Otosclerosis and Dysplasias of the Temporal Bone.

Many bone dysplasias, some common and others rare, may involve the temporal bone causing conductive, sensorineural, or mixed hearing loss, vestibular dysfunction, or skull base foraminal narrowing, potentially affecting quality of life. Some conditions may affect only the temporal bone, whereas others may be more generalized, involving different regions of the body. High-resolution computed tomography may detect subtle osseous changes that can help define the type of dysplasia, and MR imaging can help define the degree of activity of lesions and potential associated complications.

Superficial malignant peripheral nerve sheath tumor arising from diffuse neurofibroma in a neurofibromatosis type 1 patient.

Malignant peripheral nerve sheath tumors (MPNST) are regarded as sarcomas that arise from peripheral nerves or that display differentiation along the lines of the various elements of the nerve sheath. These tumors occur in deep soft tissues, but superficial primary MPNST with a cutaneous or subcutaneous origin have rarely been reported. A 70-year-old woman presented with a 3-4-year history of a slowly enlarging soft nodule on the left side of her neck. The histopathological diagnosis of the nodule was low-grade MPNST arising from diffuse neurofibroma. There was increased cellularity, but no ne

Protrusio acetabuli: its occurrence in the completely expressed Marfan syndrome and its musculoskeletal component and a procedure to arrest the course of protrusion in the growing pelvis.

Protrusio acetabuli [center-edge (C/E) angle > 60 degrees, teardrop collapsed, and pelvis invaded by acetabulum beyond ilioischial line (Köhler's)] was identified in 22 patients with classic Marfan syndrome. All had a preponderance of the syndrome's cardinal features (positive pedigree and musculoskeletal, ocular, and cardiovascular manifestations). Fifty-two immature patients with the syndrome's skeletal expressions predominating were studied for deep acetabuli. Half of these individuals (26 patients) had true protrusio acetabuli. Thirty-one percent (16 patients) had abnormal acetabular deepe

[Application of the Ilizarov technic in the lengthening of the humerus].

Lengthening of the humerus has been considered to be particularly risky because of the danger of neurological damage. The Ilizarov technique makes it possible to avoid complications. The authors have performed eight such lengthenings, with gains in length of 8 to 16 cms. Only one case was complicated by an ulnar and radial paralysis which recovered secondarily.

Canine hypertrophic osteodystrophy, a study of the spontaneous disease in littermates.

All members of a weimaraner litter had clinical and radiographic signs of hypertrophic osteodystrophy shortly after weaning. Three dogs were necropsied. Radiographic metaphyseal densities, which are used to make a clinical diagnosis of hypertrophic osteodystrophy, were found to result from elongation of the calcified cartilage lattice of the primary spongiosa. Intertrabecular acute inflammation was associated with necrosis, failure to deposit osseous tissue on the calcified-cartilage lattice, and trabecular microfractures. This process led to metaphyseal infraction and separation of the epiphy

[Skeletal dysplasias-Multidisciplinary orthopedics].

Skeletal dysplasias (SD) are a heterogeneous group of rare hereditary diseases with pronounced skeletal involvement and include over 750 phenotypes, which are associated with disease-triggering variants in more than 550 genes. In addition to orthopedic treatment, a network of various disciplines is necessary to ensure high-quality diagnostics and treatment for SD. This is ideally carried out at a center specialized in the treatment of rare bone diseases (European Reference Network on Rare Bone Diseases). The close networking of orthopedics with pediatrics and other disciplines is important to

Case Report: Surgical timing for Blount's disease: a case report and systematic review.

BACKGROUND: Currently, Blount's disease is treated in a variety of ways, but the optimal timing of treatment and the choice of optimal treatment regimen have yet to be determined. We report a case of a patient who failed multiple surgical treatments and underwent 3D-printed osteotomy guide-assisted proximal tibial orthopedic external fixation in adulthood to restore normal lower limb mechanical axis and suggest optimal treatment modalities in light of the systematic literature. METHODS: A case of Blount's disease patient who was misdiagnosed and missed and underwent multiple surgical treatmen

Dysplasia Epiphysealis Hemimelica Presenting as Multiple Loose Bodies: A Case Report.

CASE: A 11-year-old boy with no medical history presented with a protective limp and worsening mechanical pain in his left knee. No recent traumatic or infectious history was reported. Radiographs and ultrasonography showed multiple intra-articular loose bodies with osteocartilaginous signal. Dysplasia epiphysealis hemimelica (DEH) was confirmed by magnetic resonance imaging (MRI) and computed tomography (CT) scan. This is the first report that describes the presence of loose bodies in a knee without previous surgery as a possible case of DEH. CONCLUSION: We emphasize the use of CT scan and M

Blount disease: an update.

Blount disease is a developmental disorder associated with childhood obesity. Based on whether the deformity is first noted before or after 4 years of age, early-onset and late-onset forms of Blount disease have been described. Besides physeal abnormalities of the proximal tibia, compensatory changes in the intra-articular morphology of the medial compartment of the affected knee are often noted on MRI scan. Both guided growth and acute and gradual correction via a proximal tibial osteotomy have roles in the surgical management of these patients. In order to optimize clinical outcome, frequent

The use of anterolateral bowing of the lower leg in the diagnostic criteria for neurofibromatosis type 1.

Neurofibromatosis type 1 is diagnosed clinically based on the presence of two of seven criteria developed by a panel of experts in 1987. The sixth criterion focuses on skeletal findings and is as follows: "A distinctive osseous lesion such as sphenoid dysplasia or thinning of long bone cortex, with or without pseudarthrosis." The wording for this criterion is misleading. In particular, "thinning of long bone cortex" is not the characteristic radiographic presentation, and no mention of long bone bowing is included. The distinctive clinical feature of long bone dysplasia in neurofibromatosis ty

[Impact of angiodysplasia on bones].

Vascular malformations usually have an impact on bones because of hemodynamic disturbances in the muscular and osteoperiosteal systems. In fact, even when these structures are not involved by arterial or venous malformations, the blood flow is altered in them. The consequences for bones affect the longitudinal growth of the skeleton of the malformed limb. Growth rarely becomes slower. On the contrary, the condition usually results in an acceleration of bone growth, which results in an excess in the length of the limb presenting with the vascular malformation. This overlengthening may be variab

[Correction of tibial varus deformity due to Blount disease using an epi-metaphyseal distraction].

The possibility to correct tibial varus deformity due to Blount disease by epi-metaphyseal distraction was presented. The correction was achieved in 2 patients; in one of them leg length discrepancy was eliminated. The method is not invasive, offers continuous rehabilitation and a possibility of equalization of the extremities.

Stability in correction of hypertelorbitism and Treacher Collins syndromes.

In the authors' experience, correction of hypertelorbitism either by facial bipartition or by moving only the orbits has proved to be very stable. This is based on the design of the osteotomies and the use of the frontal bar. On the contrary, the rotation of the midfacial segment combined with mandibular lengthening for the correction of Treacher Collins has a strong tendency to relapse because of the backward pull of the soft tissues. Primary stability in these cases depends on the quality of bone grafts, their placement, and skeletal fixation.
