Alcaligenes xylosoxidans conjunctivitis.

PURPOSE: To report a case of chronic unilateral conjunctivitis caused by Alcaligenes xylosoxidans. METHODS: A previously healthy 47-year-old woman presented with left eye purulent discharge and irritation beginning 3 months earlier. The patient had previously been treated with bacitracin and olopatadine without improvement. Bacterial staining, culture, and antibiotic sensitivities were performed from a conjunctival swab. RESULTS: The cultures revealed heavy growth of A. xylosoxidans. The organism was resistant to aminoglycosides, fluoroquinolones, and cephalosporins. The patient was started

[Viral and chlamydial conjunctivitis].

Viral conjunctivitis is the most common type of conjunctivitis. It is contagious and is predominantly seen as adenovirus-related conjunctivitis and keratoconjunctivitis (ADV). These infections are the most frequent ocular surface infections and can lead to conjunctival scarring and corneal opacities. Other types of viral conjunctivitis are often secondary to systemic infection and typically resolve spontaneously. Chlamydial conjunctivitis, on the other hand, is dominated by trachoma, which remains endemic in 42 countries and is still the leading cause of infectious blindness worldwide. In indu

Bleb-related endophthalmitis during an epidemic of viral keratoconjunctivitis.

There is an annual epidemic of viral conjunctivitis at the onset of the monsoon (rainy) season in North India. This affects a large population, with many patients developing ocular symptoms and then getting better without any treatment as there is no specific therapy for adenoviral infections. However, this creates a dangerous situation for patients with operated trabeculectomy with a filtering bleb as conjunctivitis can lead to blebitis and bleb-related endophthalmitis with super-added bacterial/fungal infections. While the viral infection is self-limiting and most patients get well without t

Swine conjunctivitis outbreaks associated with Mycoplasma hyorhinis.

Conjunctivitis is an uncommon finding in commercial swine herds, and the etiology of the disease is rarely studied. We investigated cases of conjunctivitis in 3 wean-to-finish swine farms. Eye swabs and tissues were obtained from clinically affected pigs (8-22 wk of age), from unaffected pigs in contact with affected pen-mates, and from age-matched pigs from an unaffected herd. Real-time PCR (rtPCR) testing for Mycoplasma hyorhinis demonstrated consistent detection and high bacterial load in samples from affected herds (clinically affected animals and non-clinical pen-mates). Ct values in affe

Ocular streptococcal infections: A clinical and microbiological review.

Streptococcus is a diverse bacterial genus that is part of the ocular surface microbiome implicated in conjunctivitis, keratitis, endophthalmitis, dacryocystitis, and orbital cellulitis that can lead to decreased visual acuity and require surgical intervention. The pathophysiology of S. pneumoniae is well-established and the role of the polysaccharide capsule, pneumolysin, neuraminidases, and zinc metalloproteinases in ocular infections described. Additionally, key virulence factors of the viridans group streptococci such as cytolysins and proteases have been outlined, but there is a paucity o

Chlamydia suis survival in dust: First insights.

Chlamydia (C.) suis, a zoonotic intracellular bacterium, is described as a causative agent for conjunctivitis, particularly in nursery and fattening pigs. Chlamydiaceae are claimed to survive drying and to persist in dust. The objective of this study was to evaluate the viability of Chlamydia in dust sampled in a fattening pig farm with a high appearance of chlamydial-induced conjunctivitis. Dust was collected and stored at room temperature. To evaluate bacterial load and survival over time, quantitative PCR (Chlamydiaceae, C. suis) and isolation in cell culture were performed every week for u

Clinical evaluation of trimethoprim-containing ophthalmic solutions in humans.

We studied trimethoprim in combination with sulfacetamide and polymyxin B and also in combination with polymyxin B alone (without the sulfacetamide) to determine the efficacy and safety of these new antibiotic combinations in the eyes of patients with bacterial conjunctivitis or blepharitis. Patients were selected for the study if they showed at least three of the following criteria: (1) symptoms of a surface ocular infection; (2) a purulent discharge; (3) a polymorphonuclear neutrophilic response on Giemsa stain; (4) a history of recent exposure to an infected individual; (5) a history of an

Fucithalmic, chloramphenicol or no treatment after squint surgery in children. A single blind randomised study.

One hundred and four children undergoing strabismus surgery were randomised post-operatively to one of three treatment groups; 1) fucithalmic drops twice daily; 2) chloramphenicol (chlormycetin) ointment twice daily; 3) no treatment. Allocation to the 'no treatment' group had to be stopped after 3 of the first 8 patients in the group developed severe mucopurulent conjunctivitis. A parental questionnaire and clinical assessment, carried out in a single blind fashion revealed that nearly all parents were able to apply the prescribed treatment and that both treatments were effective in preventing

[Tear lactoferrin content in normal Chinese adults and various ocular diseases].

The authors found the tear lactoferrin content in normal Chinese adults to be 1.46 +/- 0.32 mg/ml (mean +/- 1 SD), and the mean counting rate of its unsaturated binding power of iron 63,660 +/- 17,010/min/ml (mean +/- 1 SD), with very significant positive correlation between the two, irrespective of sex or eye distinction. Tear lactoferrin began to decrease after 40 years of age, and remarkably so after 70. The authors deemed the measurement of tear lactoferrin a reliable and sensitive marker for the diagnosis of keratoconjunctivitis sicca. In patients with bacterial corneal ulcer and acute ca

How do general practitioners manage eye disease in the community?

A survey of the management of eye disease in the community was carried out in two general practices over a three-month period. During this time there were 238 consultations by patients with ocular symptoms, making up 2.3% of all consultations and giving an annual consultation rate for eye disease of 66 per 1000 persons at risk. The four commonest diagnoses were bacterial conjunctivitis, allergic conjunctivitis, meibomian cyst, and blepharitis, and these accounted for more than 70% of the consultations. A variety of topical and systemic treatments were used, with topical chloramphenicol prescri

Epidemiologic study of conjunctivitis-otitis syndrome.

We conducted a prospective cross-sectional epidemiologic study of conjunctivitis-otitis syndrome during two 3-week periods of consecutive winters. A representative sample of 184 pediatricians and ear, nose and throat specialists in the Paris area recruited 2901 children from 6 to 36 months of age presenting with acute otitis media, of whom 465 (16%) also had purulent conjunctivitis. In sampled children, the culture of the conjunctival exudate yielded bacterial pathogens in 419 patients: Haemophilus influenzae, 371 (89%); Streptococcus pneumoniae, 72 (17%); and Branhamella catarrhalis, 23 (5%).
