RATIONALE: Zoledronic acid is one of the most commonly used intravenous, highly potent amino diphosphonate salts worldwide and is commonly used in patients with primary or secondary osteoporosis, most of whom are well tolerated. There are currently no reports of severe sepsis induced by zoledronic acid. Here we present the first case of severe sepsis induced by zoledronic acid. We reviewed the literature and found that there is currently a lack of reports on severe sepsis induced by zoledronic acid or other diphosphonates.

Ollier's disease is characterized by multiple skeletal enchondroma. There are published data regarding Ollier's disease being associated with vascular malformations and non-skeletal neoplasms. We report a case of Ollier's disease in a young male associated with osteochondroma, low grade glioma in the insular cortex of brain and Gilbert's syndrome. Technetium-99m methylene diphosphonate whole body bone scan is a sensitive investigation to ascertain the complete extent of skeletal involvement particularly the asymptomatic sites.

Sphenoidal sinus carcinoma is a rare cause of hypercalcemia of malignancy. We report on a 37-year-old male with sphenoidal sinus carcinoma with intracranial extension who developed hypercalcemia of malignancy with progressing disease and demonstrated diffuse metastatic visceral calcifications of lungs, myocardium, stomach, kidneys and thyroid on follow-up 99mTc-methylene diphosphonate bone scan. In the absence of extensive skeletal metastases, bone scan help confirm humoral nature of hypercalcaeimia.

Metastatic calcification, a known complication of prolonged end-stage renal disease, is herein described for the first time in a 10-month-old boy with acute renal failure, manifesting as a painful and swollen arm. Imaging revealed diffuse calcification and technetium-99 methylene diphosphonate (99Tc-MDP) uptake around the humerus and axilla. Calcium and vitamin D restriction, followed by intravenous administration of sodium thiosulfate caused a full symptomatic, radio- and scintigraphic improvement.

Bone uptake of 99 m Tc diphosphonate may allow early diagnosis of renal osteodystrophy. Our study deals with 53 patients treated by repeated hemodialysis and reveals scanning abnormalities in 75% of them. Bone scanning provides information which are not visualized radiologically, especially scattered or located pulmonary calcifications (15% in our series). It is valuable in the follow-up of bone changes and is part of the necessary investigations before carrying out parathyroidectomy.

Twenty-four patients with increased skeletal:renal uptake ratios of 99mTc-methylene diphosphonate were studied. Increased uptake was central in metastatic prostate carcinoma, peripheral in hematologic disorders, and heterogeneous in Paget disease and fibrous dysplasia. There was no discernible redistribution of skeletal uptake in patients with renal failure. Absence of both renal and bladder activity was not observed in patients with normal renal function. An increased ratio was always abnormal and frequently indicated diffuse bone disease.

Patients undergoing bone scintigraphy or about to have breast surgery were studied with either 99mTc-polyphosphate or 99mTc-diphosphonate. Seven of 8 patients with carcinoma demonstrated a focal area of increased uptake. The 9 controls, who were clinically free of breast disease, had normal images. In the benign group, increased uptake was seen in patients with gynecomastia, lactating breasts, or acute inflammation. Most patients showed correlation with mammography and thermography.

Sixty children with Legg-Perthes disease (19), bone tumor (27), osteogenesis imperfecta (7), osteomyelitis (5) and transient synovitis (2) were studied using 99mTc labeled diphosphonate. A number of benign or malign bone diseases of children need early detection in order to institute the best form--the fine form--of treatment. We recommend the bone scintigraphy in the initial screening of children with signs and symptoms of bone pathology.

Elevated 99mTc-methylene diphosphonate (MDP) uptake in the left lung was demonstrated in a 41-year-old man with chronic lymphocytic leukemia. Lung infection was considered because the patient also had fever and pancytopenia. However, the thoracic CT performed the next day did not reveal abnormality which could explain the cause of left lung MDP activity. The repeated thoracic CTs weeks later demonstrated multiple ground-glass opacity in the left lung.

A case of progressive diaphyseal dysplasia (Engelmann's disease) is described in a 23-year old woman. The disease is characterized by limb pain, muscular wasting, a waddling gait and easy fatiguability. It is diagnosed radiologically by cortical thickening, a narrowed medullary cavity, and increased diameter of the diaphyses of the long bones. The extension of the bone lesions was evaluated by scintigraphy using 99mTc methylene diphosphonate (MDP) and 99mTc colloids. Corticosteroid treatment resulted in clinical improvement and reduction of the diaphyseal uptake of MDP and colloids.

Bone scans with 99mtechnetium diphosphonate were performed on 2 patients with gynecomastia induced by diethylstilbestrol therapy for adenocarcinoma of the prostate. Neither patient had evidence of bone metastases but both scans revealed increased isotope concentration over the anterior rib cage at the lateral margin of the chest wall, corresponding in location to the hypertrophic breasts. This observation may be related to similar radionuclide uptake in normal and abnormal female breasts. One should not mistake the finding of gynecomastia for metastases of the ribs.

The composition of 99mTc-ethane-1-hydroxy-1, 1-diphosphonate complexes, prepared with the reducing agent NaBH4 and by electrolytic reduction, was analysed by reversed-phase ion-pair chromatography and soft-gel permeation chromatography. The results were compared with corresponding data for complexes prepared with the traditional reducing agent tin(II). Significant differences between the chromatograms of the three preparations were found. They were interpreted in terms of the occurrence of different complexes in the three preparations.

We report on 4 patients with secondary hyperparathyroidism of intestinal origin presenting a characteristical pattern of distribution, namely focal accumulations of 99mTc-methylene diphosphonate on locations typical of Looser's zones. In accordance with other authors we demonstrate the higher sensitivity of skeletal scintigraphy as compared to radiography which is well known from other disorders of bone. We also refer to the problems of differential diagnosis arising when multiple foci are visible on bone scan.

Malignant otitis externa is a severe, rare infective condition of the external auditory canal and skull base. The diagnosis is generally made from a range of clinical, laboratory, and imaging findings. Technetium 99m methylene diphosphonate bone scintigraphy is known to detect osteomyelitis earlier than computed tomography. The authors present a patient with bilateral malignant otitis externa where the extent of skull base involvement was determined on 3-phase bone scintigraphy with single photon emission computed tomography/computed tomography.

We present a case of thymic carcinoid, in which primary and metastatic lesions of lymph nodes and bones could be detected by [(18)F]fluoro-2-deoxy-D-glucose (FDG)-PET, but not by (123)I-meta-iodobenzylguanidine ((123)I-MIBG) SPECT, or by (99m)Tc-methylene diphosphonate ((99m)Tc-MDP) bone scintigraphy. FDG-PET may be a useful tool for managing thymic carcinoids in patients with negative results on (123)I-MIBG SPECT or (99m)Tc-MDP imaging.

Limitation of movement of vocal cords developed in a 38-year-old man after he received a 45% second- and third-degree burn with ossification about the cricoarytenoid joints, which indicated that the lesion had been caused by heterotropic ossification. To the best of our knowledge, this is the first report in medical literature of heterotopic ossification as a mechanical cause of vocal cords dysfunction in the patient with critical burns. Diphosphonate (Didronel) treatment has improved the function of the vocal cords after 11 months of therapy.

A 41 year old man was admitted to hospital for acute alcoholic rhabdomyolysis and renal failure. Calcium deposits in the damaged muscles of the inferior limbs were detected by technetium-99 m methylene diphosphonate. Repeat bone imaging studies showed a decrease in the deposits with time. The mechanisms responsible for the rhabdomyolysis and the characteristic disturbances in serum calcium and phosphorus concentrations during such renal failure are recalled. The interest of 99mTc-MDP is reviewed. Early dialysis is advocated, in particular in case of extensive muscular damage.
