Comparison of Survival in Recipients of Marginal and Standard Cadaveric Donor Kidneys.

BACKGROUND: Renal transplantation is the main treatment for end-stage renal disease and has rapidly increased in number over the past 3 decades, leading to an increasing need for organs. The disparity between organ access and the number of patients on the waiting list has led transplant surgeons to use marginal cadaveric donors. As a consequence, the concept of "marginal cadaveric kidney donor" has been developed. METHODS: We retrospectively evaluated 120 patients who received kidneys from cadaveric donors between September 2009 and August 2021. The donors were divided into standard and margi

Fear, fascination and the sperm donor as 'abjection' in interviews with heterosexual recipients of donor insemination.

The background to this article is the medical regulation of sperm donation in the UK and the recent policy change so that children born from sperm, eggs or embryos donated after April 2005 have the right to know their donor's identity. I draw upon data from interviews with ten women and seven joint interviews with couples who received donor insemination from an anonymous sperm donor and were the parents of donor insemination children. I explore the symbolic presence of the donor and his potential to disrupt social and physical boundaries using the theoretical conceptions of boundaries and poll

Optimal management of brain-dead organ donor.

Maintaining a brain stem-dead (BSD) donor is specialized science. It is a daunting task as they are fragile patients who need to be handled with utmost care owing to extreme haemodynamically instability and need the best of monitoring for maintenance of organs. To ensure a successful transplant, a BSD donor first needs to be identified on time. This requires scrupulous monitoring of neurologically compromised patients who tend to be the most frequent organ donors. Once the donor is identified, an all-out effort should be made to legally obtain consent for the donation. This may require numerou

Living kidney donor experiences: implications for counselling.

This study adds to previous, mostly quantitative, investigation into the experiences of living related kidney donors. Such investigation is important so that potential donors are supported effectively and donation programmes remain relevant and specific to need. Exploration takes place into donor decision-making processes and the most effective forms of professional support. A non-probability sampling technique highlighted eight living related kidney donors who were interviewed using a semi-structured interview format. Raw data was analysed through the qualitative technique of Interpretative P

[Anonymity and gamete donation].

In France the gamete donation is based on the major principles: anonymity, no payment, solidarity and this mode of procreation can be used only if a medical indication is present in recipient couples. In prerequisite and during the revision of the law of bioethics, a wide debate took place about the anonymity of gamete donation. The objectives of this article is to review the concept of the anonymity and its links with the questions of the origin, the secret of the modalities of the conception and the mourning of the fertility, children, donors and the recipient couples waiting for gamete dona

Prevalence of markers of transfusion transmissible diseases in voluntary and replacement blood donors.

BACKGROUND: Transfusion of safe blood requires a safe donor. The voluntary donor movement encompasses the concept of a donor who is free from transfusion transmissible infections. It is now mandatory to screen blood for hepatitis B surface antigen, antibodies to HIV-1 and HIV-2, antibodies to hepatitis C virus, syphilis and malarial parasites. METHODS: Between 1996 and 2002, 235 461 donors were screened for markers of hepatitis B virus, and HIV-1 and HIV-2 using commercially available ELISA kits, VDRL test for syphilis and Geimsa stain for the malarial parasite, respectively. A total of 56 47

An iridium(III) complex-based luminogenic probe for high-throughput screening of hydrogen sulfide donors in living cells.

The scarcity of suitable high-throughput screening technology for hydrogen sulfide (H2S) donors has hampered the discovery of H2S donors. In this study, a long-lived cyclometalated iridium complex was rationally designed as a mitochondria-targeted H2S probe to monitor the real-time dynamic change of H2S. By using the time-resolved emission spectroscopy (TRES) technique, an anti-interference high-throughput screening system was developed to monitor H2S in living cells with decreased false negative results. As a proof-of-concept, three natural products were identified as potential H2S donors fro

Intentional and successful use of a marginal donor heart with surgically-corrected interventricular communication.

We describe a case of heart transplantation (HTX) performed using a heart from a 20-year-old donor who underwent surgical closure of a ventricular septal defect during childhood. Our 26-year-old patient was successfully discharged to a rehabilitation centre on day 20 post-transplantation. To our knowledge, this is the first report of an HTX performed with a 'redo' donor heart with previous surgical correction of a congenital heart defect. The widespread use of HTX as a therapeutic option is currently limited by the increase in number of patients listed annually for this procedure. The concomit

Embryo donation parents' attitudes towards donors: comparison with adoption.

BACKGROUND: Embryo donation produces a family structure where neither rearing parent is genetically related to the child, as in adoption. It is not known how embryo donation parents view the donors compared with how adoptive parents view the birth parents. METHODS: 21 couples with an embryo donation child aged 2-5 years were compared with 28 couples with an adopted child. Parents were administered a semi-structured interview, assessing knowledge of the donors/birth parents, frequency of thoughts and discussions about the donors/birth parents and disclosure of the donor conception/adoption to …

Predictors of success with the use of donor sperm.

OBJECTIVE: This study was undertaken to assess the effect of multiple factors that influence the success rate and time to conception among couples undergoing donor sperm insemination. STUDY DESIGN: A retrospective analysis of 960 cycles of frozen donor sperm insemination was performed at the University of Florida. Cycle pregnancy rates and cumulative probability of pregnancy were compared using several variables. RESULTS: The pregnancy rate was 12.1% per treatment cycle, and the cumulative probability of pregnancy exceeded 80% for the entire cohort. Seventy percent of pregnancies resulted in

Perspectives on sperm donor anonymity: insights from donor-conceived adults in Belgium.

STUDY QUESTION: Are donor-conceived adults in Belgium interested in obtaining donor information, and do these interests vary based on their family backgrounds? SUMMARY ANSWER: Donor-conceived adults express a significant interest in obtaining donor-related information, with the highest interest reported by offspring from heterosexual couples compared to those from lesbian couple-parented or single-parent families. WHAT IS KNOWN ALREADY: In Belgium, sperm donation is mainly anonymous, but the rise of direct-to-consumer genetic testing challenges this anonymity. STUDY DESIGN, SIZE, DURATION:

Pregnancy loss in lesbian and bisexual women: an online survey of experiences.

BACKGROUND: Although pregnancy loss is a distressing health event for many women, research typically equates women's experiences of pregnancy loss to 'married heterosexual women's experiences of pregnancy loss'. The objective of this study was to explore lesbian and bisexual women's experiences of miscarriage, stillbirth and neonatal death. METHODS: This study analysed predominantly qualitative online survey data from 60 non-heterosexual, mostly lesbian, women from the UK, USA, Canada and Australia. All but one of the pregnancies was planned. Most respondents had physically experienced one ea

Shifting from donor to donor-recipient matching perspective in defining indications for machine perfusion in liver transplantation.

Optimization of donor-recipient matching is a common concept in liver transplantation. In emergency transplant for acute liver failure, outcome is influenced by timing, patient clinical condition, and graft quality. Although factors like advanced donor age have been linked to a poorer outcome, use of suboptimal or marginal grafts can be inevitable in very unstable patients, if no other graft is available. We present a case of a liver transplant performed in an extremely sick patient suffering from HBV-related fulminant hepatitis, in which a compatible graft from a 76-year-old deceased donor be

Expanded donor criteria due to age: an effort rewarded.

The life expectancy of the current Spanish population is 78 years, with some 20% of the population aged over 65 years. The continuing increase in the demand for transplants has led to widening of the criteria for acceptance of donors without worsening the results, but without reducing transplant waiting lists or mortality. This has resulted in the need to include the concept of the expanded criteria donor (ECD). We undertook a retrospective study of 77 donors from a hospital with no neurosurgery service from January 2000 to December 2005. Four of the donors younger than 60 years of age (n = 38

Possible CMV transmission via intrauterine insemination: case report.

BACKGROUND: Maternal cytomegalovirus (CMV) infection at conception can cause congenital CMV (cCMV) infection and substantial morbidity. Although CMV screening is mandated for sperm donors, the risk of acquiring CMV from donor sperm is unknown. Experience with HIV may lead clinicians to expect that standard sperm-washing reduces CMV transmission risk for procedures including intrauterine insemination. However, limited data suggests that CMV may differ importantly from other herpesviruses and from HIV after sperm washing. CASE PRESENTATION: A 29-year-old CMV immunoglobulin (Ig)M- and IgG-negati

Anterolateral thigh free flap for "head-to-toe" reconstruction.

Since its conception the anterolateral thigh (ALT) free flap has become a cornerstone in the reconstruction of complex head and neck defects. It has quickly gained further uses because of its variability in size, malleability, 2-team approach to the operation, and low-morbidity donor site. The ALT lends itself well to reconstructing complex defects throughout the body, from the lower extremities up to the head and neck, and a variety of indications, from oncologic defects to burns and traumatic injuries. Twenty patients (18 male, 2 female; average age 57.74 years [range, 17-86 years]) had ALT

Donation Care Work: Family Members' Contributions to Deceased Organ Donation Following Determination of Death Using Circulatory Criteria.

Family members of potential deceased organ donors are often portrayed as gatekeepers to the individual experience of organ donation. We argue the success of deceased donation as an institution depends on often-unrecognised family labour that occurs after consent. Building on feminist notions of care work, we propose the concept of 'donation care work' to conceptualise the combination of emotion work, end-of-life care, and death work that family members perform to support efforts for donation while facing imminent loss. We interviewed 38 family members of potential deceased organ donors in Cana
