Unregulated skin-lightening cream use causing topical steroid-induced dermatitis and nodulocystic acne.

A woman in her early 30s developed severe facial erythema, telangiectasia, burning sensation and progressive hyperpigmentation after 12 months of using an over-the-counter skin-lightening product labelled 'skin-lightening cream' purchased from a local beauty store. Laboratory analysis confirmed the presence of clobetasol propionate 0.05% w/w, which was not disclosed on the product label. Following discontinuation, she developed painful nodulocystic acne as a delayed complication. Treatment with topical calcineurin inhibitors, emollients, photoprotection and oral doxycycline resulted in partial…

Hydroxychloroquine-induced erythroderma.

Erythroderma is characterized by diffuse erythema and scaling of the skin involving more than 90% of the total body skin surface area. Drug-induced erythroderma has rarely been reported with hydroxychloroquine. We report a case of a 50-year-old female patient, with systemic lupus erythematosus, who developed itchy lesions all over the body 1 month after starting treatment with hydroxychloroquine. Drug-induced erythroderma was suspected. Hydroxychloroquine was withdrawn and the patient was treated with emollients, mid-potency corticosteroids, and oral antihistamines. A biopsy was done which con…

The use of cleansers as therapeutic concomitants in various dermatologic disorders.

The choice of a mild cleansing agent is important in the adjunctive management of various skin conditions, such as atopic dermatitis, acne vulgaris, rosacea, photoaging, retinoid-induced irritant dermatitis, and sensitive skin. There are 3 major categories of cleansing agents: soaps, synthetic detergents, and lipid-free cleansing agents. The irritancy potential of cleansing agents is a function of a number of factors, including the pH, type of surfactants, and amount of skin residue. Furthermore, the presence of humectants and emollients also can influence the overall mildness of a cleansing a…

The Clock Is Ticking: Everything We Have Learned About Food Allergy Prevention in the Last Decade.

This article covers common risk factors for the development of food allergy (FA), and the current evidence-base for each. Non-modifiable risk factors contribute to FA development, including genetics, gender, and mode of delivery. The article explores the current research on maternal diet and nutrition during pregnancy and lactation, specifically exploring supplementation with vitamin D, omega-3 fatty acids, and prebiotics/probiotics. Infant factors for development of FA, including emollient use and breastfeeding, are also reviewed. The article concludes with a discussion on early introduction

Propylene Glycol.

Propylene glycol (PG), an emollient and emulsifier found in cosmetics, medications, and food, has been granted the dubious honor of being named the American Contact Dermatitis Society's Allergen of the Year for 2018. Allergic and irritant contact dermatitis and systemic cutaneous reactions to PG, which has become an increasingly common ingredient, have been documented. Propylene glycol is as contentious as it is ubiquitous because it acts as both a weak sensitizer and an irritant, confounding the results of positive patch tests. This report serves to increase awareness about PG sensitization a

Evaluating the Efficacy and Safety of 4% 5-Fluorouracil Cream in Patients with Actinic Keratosis: An Expert Opinion.

Actinic keratosis is a lesion that develops in sun-exposed areas of the skin and is considered to be a precancerous condition or an early in situ squamous cell carcinoma. Treatment of actinic keratosis is important for reducing skin cancer risk, with treatment choice based on patient-, lesion- and treatment-related considerations. Of the topical treatments used for field-directed therapy, those containing 5-fluorouracil are among the most effective and widely prescribed. The most recently developed topical 5-fluorouracil preparation (Tolak®; Pierre Fabre, France) contains 4% 5-fluorouracil in

Efficacy of a moisturizer for pruritus accompanied by xerosis in patients undergoing dialysis: A multicenter, open-label, randomized verification study.

Xerosis and pruritus are common in patients undergoing dialysis. These symptoms are treated with moisturizers, but limited evidence supports the efficacy of such treatment. Our exploratory study suggested the effectiveness of a heparinoid-containing product for xerosis in dialysis patients. We conducted a multicenter, open-label, randomized, before-after, parallel-group comparative study to verify the exploratory study results (Clinical Trial Registry: UMIN000029360). Seventy-one Japanese patients undergoing dialysis with chronic kidney disease and xerosis were randomly assigned to receive a h

Distribution profiles of diphenhydramine and lidocaine in scalp, axillary, and pubic hairs measured by micro-segmental hair analysis: good indicator for discrimination between administration and external contamination of the drugs.

PURPOSE: Drug distribution in scalp hair can provide historical information about drug use, such as the date and frequency of drug ingestion. We previously developed micro-segmental hair analysis, which visualizes drug distribution at 0.4-mm intervals in individual hairs. The present study examines whether the distribution profiles of drugs can be markers for the administration or external contamination of the drugs using scalp, axillary, and pubic hairs. METHODS: A single dose of anti-itch ointment containing diphenhydramine (DP) and lidocaine (LD) was topically applied to the axillary or pu

Effectiveness of skin cleanser and protectant regimen on incontinence-associated dermatitis outcomes in acute care patients: A cluster randomised trial.

Skin cleansers and protectants protect skin from incontinent matter to reduce the risk of incontinence-associated dermatitis (IAD), but their effectiveness treating established IAD in the tropics is unknown. We conducted an open-label cluster randomised trial to compare the effectiveness of a combined regimen of (1) specialised skin cleansers with disposable body wipes and (2) either an acrylic terpolymer (T1) or zinc oxide (T2) skin protectant against disposable body wipes and zinc oxide protectant (control) in promoting IAD healing and reducing the risk of deterioration. Eighty-four patients

Allergic contact dermatitis to ethylenediamine. A continuing problem.

Ethylenediamine would be an infrequent sensitizer if it were not present in a widely prescribed topical corticosteroid preparation that is applied to inflamed skin. Occupational allergic contact dermatitis to ethylenediamine is not common. The educational efforts of American dermatologists such as Dr. Alex Fisher have helped lead to the deletion of ethylenediamine from Mycolog cream. It is unfortunate that the same pharmaceutical company continues to use ethylenediamine in its products in other countries, as the continued popularity of these creams may allow ethylenediamine to remain a common
