Penile prosthesis implantation (PPI) is a definitive treatment option for patients with medically refractory erectile dysfunction (ED). It is a safe, reliable operation with high patient satisfaction and few complications. We report a novel case of an adult patient with exstrophy-epispadias complex who underwent PPI for ED and discuss the surgical challenges presented by the unique anatomic constraints of this condition. The bladder exstrophy and classic epispadias represent a spectrum of congenital anomalies with different degrees of anterior midline defect. Although the anomaly usually occurs sporadically there are some cases indicative of a strong genetic component. We present the clinical data of two siblings with bladder exstrophy and epispadias complex (BEEC), who were the product of consanguineous union. All previous reports of familial BEEC in the literature have been reviewed.

Epispadias is a rare condition in women. A postmenopausal 53-year-old woman with a known problem of epispadias came to our outpatient clinic complaining of persistent severe urinary stress incontinence without overactive bladder symptoms. She had previously undergone an inefficacious tension-free vaginal tape (TVT) procedure in another institution. Five male patients with epispadias and total urinary incontinence were managed in the present work. Urethral mucosal tube was performed by folding one lip of the bladder neck and urethral wall to encircle the mucosal tube snugly, and the other lip was used to overlap the first one at a distance of 3 cm length. This technique provided an effective functioning segment that maintained continence. Our results since we have adopted this technique were excellent in four cases.

The preputial island flap technique introduced by Duckett for repair of severe hypospadias has met with much clinical popularity. We report the use of a modification of this technique for repair of epispadias in 3 boys with exstrophy of the bladder and in 1 with penile epispadias. The results have been excellent in 3 cases and good in 1. Minimum followup has been 18 months.

BACKGROUND: Bladder exstrophy is a rare malformation. Prenatal diagnosis is usually an incidental finding on routine ultrasound examination. Triple-X syndrome (karyotype 47,XXX) is the most frequent sex chromosome aneuploidy in live-born females (approximately 1 in 1000). The diagnosis is often not made because women with 47,XXX karyotype have no or hardly any clinical symptoms during life.
METHODS: Prenatal diagnosis of triple X karyotype is usually an incidental finding when an invasive prenatal diagnosis is performed for other reasons.

Diphalia is an extremely rare urogenital condition presenting with penile duplication. Although it is typically diagnosed in the neonatal period due to associated congenital anomalies, some cases may present late without any related complications. Every case of diphallia presents a distinct set of symptoms and associations. In this case report, we underscore an interesting clinical presentation of diphallia in a 24-year-old male and the significance of individualized management of the condition.

Isolated epispadia is uncommon anamoly, occurring in approximately 1 out of 120,000 male births. To date, only 22 cases of epispadia have been reported with an intact prepuce and 8 cases with a phimotic intact prepuce.
