Factors affecting remanipulation in paediatric forearm fractures.

Children's forearm fractures treated in plaster after closed reduction can redisplace and require remanipulation. A retrospective review of 164 consecutive manipulations of fractures of radius and ulna in theatre over a 3-year period was carried out. After various exclusions, 114 patients entered the study. The overall rate of remanipulation was 14.04% (16 children). Presence of preoperative translation deformity predisposed to loss of position in plaster (P<0.0001). Residual deformity on intraoperative films also had a strong adverse influence on the outcome (P = 0.001). Usage of below-the-el…

Interosseous membrane reconstruction for forearm longitudinal instability.

Forearm longitudinal instability results from an axial load to the forearm with fracture or dislocation of the radial head and disruption of the interosseous membrane and that of the distal radioulnar joint. Patients may present in the acute or chronic setting with radioulnar instability as manifested by persistent or new forearm discomfort and wrist and elbow pain. Reconstruction of the interosseous membrane has been described to restore longitudinal forearm stability. We describe reconstruction of the central band of the interosseous membrane with a bone-patellar tendon-bone graft. This proc…

Forearm venous distensibility predicts successful arteriovenous fistula.

BACKGROUND: The success of a newly created arteriovenous fistula (AVF) depends on sufficient maturation of the forearm vein used. This maturation fails in up to 50%. We hypothesize that impairment of forearm venous distensibility, ie, the ability of veins to adjust to increased pressure, is related to AVF failure. METHODS: Forearm venous distensibility was measured by using strain-gauge plethysmography in 27 patients with end-stage renal failure awaiting vascular access surgery; either AVF or graft (AVG) formation. Ultrasound duplex scanning of the upper-extremity circulation was performed 4 …

[Forearm malignant epithelioid schwannoma associated with melanoma].

Tumours originating in peripheric nerves usually appear in patients with neurofibromatosis (NF) signs, presenting frequent combinations of tumours in nerves and cutaneous lesions. Nevertheless, this association is very rare in cases without NF. Therefore, the aim of the present article is to present a case of malignant melanoma and malignant schwannoma, without any described NF. A 69-year-old woman with antecedents of malignant melanoma diagnosed two years previously in the dorsum of the fifth finger of the left hand, treated by means of amputation of the finger. The patient presented a malign

Risk factors for perimenopausal distal forearm fracture.

This prospective population-based cohort study investigated factors predicting distal forearm fracture (DFF) in perimenopausal women. The study population consisted of 11,798 women from the Kuopio Osteoporosis Risk Factor and Prevention (OSTPRE) Study in Finland. Mean baseline age of these women was 52.3 (SD 2.9) years (range 47-56 years) and 68% were postmenopausal. Three hundred and sixty-eight women (3.1%) had a validated DFF during the 5-year follow-up. Previous wrist fracture, postmenopausal state, age and nulliparity were independent predictors of DFF, while hormone replacement therapy (…

[Essex-Lopresti injury combined with forearm fracture].

The Essex-Lopresti injury is uncommon and often overlooked. A case is presented in which a 22-year old male sustained an injury that was a combination of an Essex-Lopresti injury and a fracture of the forearm. The primary treatment was osteosynthesis of the forearm. Ten weeks after injury the fractured radial head was removed because of pain and poor function of the elbow. The full extent of the injury was not recognized until sixteen weeks after injury. Final treatment consisted of excision of the ulnar head. The final results when seen at eight years follow-up were good.

The radial forearm skin graft-fascial flap.

The radial forearm flap has become a versatile flap for upper extremity reconstruction. The use of the forearm flap for hand reconstruction in the patient with previously burned forearms has not been widely appreciated. In those patients whose forearms have been previously split-thickness skin-grafted on fascia, we have employed the reverse radial forearm flap as a skin graft-fascial flap for hand reconstruction and have obtained excellent functional results. Three patients at various intervals postburn are presented to demonstrate use of this flap for wrist contracture release, coverage of ar…

[Surgical treatment of aseptic forearm shaft pseudarthrosis].

Non union of a lower arm fracture (without infection) is the result of inadequate immobilization or of too extensive devastation of the bone. Within 1975 to 1985 in the accident-hospital of Tübingen we saw 131 pseudarthroses of the ulnar or radius shaft in 111 patients. Among these a hypertrophic pseudarthrosis is mostly seen after conservative treatment or operative treatment with intramedullary nailing. Atrophic pseudarthroses or defect pseudarthroses mostly occur after open fractures, comminuted fractures or insufficient plate osteosynthesis. The operative treatment was regularly a plating

Phalloplasty using the free radial forearm flap.

Our experience of phalloplasty using the radial forearm flap in five transsexual cases is reported. The results are disappointing due to the high incidence of complications. We had partial success in two cases, both complicated by a fistula, and complete failure in three cases due to vascular thrombosis.

Tilt of the radius from forearm rotational axis reliably predicts rotational improvement after corrective osteotomy for malunited forearm fractures.

Forearm rotation occurs around an axis connecting the center of the radial head and the fovea of the distal ulna. The purpose of the present study was to demonstrate the usefulness of the difference between forearm and proximal radial axis in the treatment of malunited forearm fractures. We reviewed the results of eight corrective osteotomies for malunited fractures of the forearm without dislocations of the wrist or elbow. Subjects were 6 men and 2 women (mean age, 15 years; range, 10-21 years). Corrective osteotomy was performed at the fracture site. Preoperatively and at final follow-up, th…

One-bone forearm formation using vascularized fibula graft for massive bone defect of the forearm with infection: case report.

Massive long-bone defects of greater than 6 cm are difficult to treat with conventional bone grafts, and other methods are sometimes recommended, such as vascularized bone grafts or bone transport using the Ilizarov external fixator. The combination of local infection with a massive bone defect exacerbates the problem, and provides an even more negative prognosis. The authors treated a large bone defect of the forearm with local infection, using a one-bone forearm formation with a large vascularized fibula graft. They attached an adequate amount of muscle fascia to the vascularized fibula, whi…

Closed Reduction Nailing or Open Reduction Plating in Unstable Paediatric Forearm Fractures: A Case of Paediatric Forearm Fracture Implant Failure.

A significant amount of all paediatric fractures are forearm fractures involving the radius, ulnar shaft, or both. As surgical stabilisation lowers the likelihood of re-displacement, surgical intervention is currently recommended over conservative treatment of such fractures involving significant displacement and angulation. Open reduction and plating can better anatomically repair the majority of fractures. Bracing is necessary for the first six to eight weeks after nailing since nailing does not give a rigid fixation. External bracing is generally not necessary for plating. In our facility, …

The medial forearm fascia and lateral forearm fascia contribute to overhead elbow extension in displaced olecranon fractures: a biomechanical study.

BACKGROUND: In nonoperative management of displaced olecranon fractures, patients are able to maintain overhead extension despite a persistent nonunion. It has been hypothesized that this is feasible because of an intact lateral cubital retinaculum. The purpose of this biomechanical study was to determine the contribution of the medial and lateral cubital retinacula to overhead extension in the setting of a displaced olecranon fracture. METHODS: Eight fresh-frozen cadaveric upper-extremity specimens were used in this study. The triceps muscle was loaded through a pulley system operated by an

Comparison of standard forearm prosthetic loop grafts to composite semiloop forearm grafts ("semi-grafts") in hemodialysis patients: A prospective study.

INTRODUCTION: To prospectively assess the performance of composite semiloop antebrachial grafts ("semi-grafts," SGs) in hemodialysis patients. METHODS: Eighty-five patients who received 67 loop antebrachial grafts (LG-group) and 25 antebrachial semigrafts (SG-group) were enrolled. SGs were defined as those originating from the brachial artery and anastomosed with the proximal mature mid-antebrachial cephalic vein. Cephalic vein length should be at least 10 cm in length and of ≥5 mm in diameter for inclusion in the SG-group. LG-group included all possible outflow vein options of minimum diamet

Volar and dorsal mid-shaft forearm plating using DCP and LC-DCP: interference with the interosseous membrane and forearm-kinematics.

PURPOSE: To clarify if dorsal plate osteosynthesis of the forearm shaft is associated with impingement of the interosseous membrane (IOM) and if rotational kinematics of the forearm are influenced. BACKGROUND: Forearm fractures are treated by separate incisions. Osteosynthesis via one approach in proximal forearm fractures was associated with higher rates of synostosis. It has been claimed that this is due to IOM impingement. MATERIALS AND METHODS: 16 forearms of fresh frozen cadavers were dissected until only radius, ulna and the IOM remained. To preserve physiological forearm rotation, the…

Efficacy of Pull-In Suture for Tendon Avulsion Injury Associated With Severe Forearm Open Fracture, Forearm Amputation, and Hand Amputation.

BACKGROUND: The pull-in suture technique is a method of primary tendon repair that involves pulling the distal stump of the ruptured tendon into the proximal muscle belly and suturing it to the intact muscle fascia for avulsion injuries at the musculotendinous junctions associated with severe open forearm trauma. The aim of this study is to report the clinical outcomes of the pull-in suture technique for musculotendinous avulsion injuries associated with severe open forearm trauma and to evaluate its effectiveness. METHODS: This study included patients who underwent pull-in suture repair for …

Acute Forearm and Hand Compartment Syndrome in a Child Following Delayed Presentation of Forearm Trauma: A Case Report and Literature Review.

Background and Clinical Significance: Acute compartment syndrome is a rare but limb-threatening emergency in pediatric patients. While most cases follow high-energy trauma or displaced fractures, acute compartment syndrome precipitated by initially underestimated forearm injuries is uncommon and may create a significant diagnostic challenge, particularly in young children who exhibit atypical clinical presentations, such as escalating anxiety and analgesic requirements, rather than classic ischemic signs. Case Presentation: We report the case of a 4-year-old girl who developed severe forearm a…
