The question of what counts as a successful outcome of the process of genetics counselling has recently become central because of the increasing calls for efficiency in health care, and for means of measuring efficiency. Angus Clarke has drawn attention to this trend, and has argued against both a measure in terms of the number of terminations of pregnancy performed as a result of counselling, and an assessment in terms of the contribution of genetics counselling to a national eugenics policy. OBJECTIVE: To assess echocardiography as an investigation for the detection of occult gene carriers in tuberous sclerosis.
PATIENTS: Sixty parents of children with tuberous sclerosis who had been extensively investigated for signs of the disease and 60 age and sex matched controls.
PROCEDURE: Blind study by two experienced echocardiographers and blind interpretation of video recordings by an adult cardiologist.
SETTING: Cardiology department of a district general hospital.
RESULTS: Two parents and three controls had bright echodense areas interpreted as possible rhabdomyomas.

An investigation into the causes of sensorineural hearing loss in young children has been used as a basis for determining risk factors for parents of children with a hearing loss of unknown origin having a further affected child and for the defect to be passed on to a subsequent generation. The degree of hearing loss likely to be found if further children are affected, is examined.

Basis knowledge of genetics is necessary for the pediatrician, but he also must be experienced in talking with grieving parents. Grief reactions should be facilitated and by this help to the parents the best basis for future genetic counselling is established. The clinical diagnosis is a prerequisite for genetic counselling. Parents seeking help must be prepared to find their own decision. If the counsellor makes typical mistakes he provokes more disappointment than relief. If he succeeds his help can preserve or encourage future lives.

The impact of information-guidance type of genetic counseling was evaluated for the family planning of 2082 consultands. The understanding of the risks, parental decision, and the number of induced and spontaneous abortions were evaluated by the use of questionnaires. A survey is reported of the genetic advice given to 272 families with osteogenesis imperfecta. An appreciable number of families were not given advice by anyone even though they had asked for it, and only one quarter of the families had been referred to a clinical genetic specialist. Overall approximately one third of the information recollected as having been received was incorrect but genetic specialists were clearly more successful than other doctors.
