Extinction theory & anorexia nervosa: Deepening therapeutic mechanisms.

By virtue of adopting the core symptomatic fear (i.e., a fear of weight gain) as a primary treatment target, the treatment of AN centrally involves exposure-driven processes. However, exposure trials targeting the fear of weight gain in AN have been sparse, yielding mixed results to date. In translating extinction theory to the treatment of AN, it is likely that the absence of a clear distinction between what constitutes the core feared cue and the core feared outcome has stymied the application of exposure treatments in AN. This review considers several configurations of the core fear associa

Agoraphobia With and Without Panic Disorder: A 20-Year Follow-up of Integrated Exposure and Psychodynamic Therapy.

The aim of the current study was to compare the 20-year outcome in panic disorder with agoraphobia (PD with AG) and agoraphobia without panic disorder (AG without PD) patients after inpatient psychological treatment. Of 53 eligible patients having completed a medication-free integrated exposure and psychodynamic treatment, 38 (71.7%)-25 PD with AG and 13 AG without PD patients-attended 20-year follow-up. AG without PD patients improved less than PD with AG patients did on primary outcome measures. In the PD with AG group, there were large uncontrolled effect sizes (<-2.30). More of the AG with

Testing the efficacy of pure versus guided mirror exposure in women with bulimia nervosa: A combination of neuroendocrine and psychological indices.

BACKGROUND AND OBJECTIVES: Body exposure improves body image problems in women with eating disorders. However it has almost always been combined with other interventions. Thus, the efficacy of body exposure alone (i.e., pure exposure) remains largely unclear. We aimed to compare the efficacy of two body exposure techniques through psychological and neuroendocrine indices recorded within and between successive sessions. METHOD: Twenty-nine women with high body dissatisfaction and diagnosis of bulimia nervosa were randomly assigned to one of two treatment groups: Pure Exposure (n = 14) or Guide

Approach behavior can mitigate predominately univalent negative attitudes: evidence regarding insects and spiders.

Three experiments tested whether disliking of predominately univalently negative attitude objects could be reduced by a procedure pairing approach behaviors with subliminally presented images of the objects. Experiment 1 demonstrated that participants who approached images of insects rated insects less negatively than participants who did not approach insect pictures. Experiment 2 extended this effect to spiders and used an implicit measure of spider attitudes. Experiment 3 examined the consequences of an approach induction for affect during actual approach behavior in a sample of individuals

Decline in cue-provoked craving during cue exposure therapy for smoking cessation.

INTRODUCTION: Based on the principles of Pavlovian learning and extinction, cue exposure therapy (CET) involves repeated exposure to substance-associated cues to extinguish conditioned cravings and reduce the likelihood of relapse. The efficacy of CET is predicated on successful extinction, yet the process of extinction in CET trials has rarely been demonstrated. This study explored the extinction process using a cue-reactivity paradigm in smokers undergoing multiple CET sessions as part of a comprehensive smoking cessation treatment. METHODS: The sample comprised 76 moderately dependent, tre

It was as big as my head, I swear! Biased spider size estimation in spider phobia.

The current study tested the association between fear and perception in spider phobic individuals (n=57) within the context of a treatment outcome study. Participants completed 5 post-treatment Behavioral Approach Tasks (BATs) in which they encountered a live spider and were asked to provide spider size estimates. Consistent with predictions, results indicated that high levels of fear were associated with magnified perception of phobic stimuli. Specifically, we found a significant positive correlation between size estimates and self-reported fear while encountering spiders. Together with previ

Immersive Virtual Reality Exposures for the Treatment of Childhood Anxiety.

Exposure-based cognitive behavior therapy (CBT) has demonstrated efficacy and is recommended as a front-line treatment for childhood anxiety. Unfortunately, challenges exist that impact the effective implementation of exposure-based CBT in clinical practice. One of the primary challenges is the accessibility and availability of exposure stimuli (e.g., spiders, storms, heights) in CBT sessions. Immersive virtual reality (VR) has shown promise as a scalable and sustainable solution to address this clinical need, but remains largely untested in youth with anxiety disorders. Here, we examine the u

Running for extinction? Aerobic exercise as an augmentation of exposure therapy in panic disorder with agoraphobia.

Exposure-based Cognitive Behavioral Therapy (eb-CBT) represents the most evidence-based psychotherapeutic approach in anxiety disorders. However, its efficacy may be limited by a delay in onset of action and a substantial number of patients does not respond sufficiently to treatment. In this context, aerobic exercise was found to be effective in reducing clinical anxiety as well as to improve (elements of) disorder-specific CBT in some mental disorders. We therefore investigated the effect of aerobic exercise supplementary to an eb-CBT in panic disorder and agoraphobia (PD/AG). 77 patients wit

A meta-analysis of dropout rates from exposure with response prevention and pharmacological treatment for youth with obsessive compulsive disorder.

OBJECTIVE: Beliefs that exposure with response prevention (ERP) is excessively distressing and will result in client dropout from treatment are commonly-cited reasons for clinicians not providing evidence-based treatment. This meta-analysis examined treatment attrition for ERP for youth with obsessive compulsive disorder (OCD) compared with other treatment modalities. METHOD: A systematic literature search identified 11 randomized controlled trials (RCTs) comparing ERP to active or waitlist control conditions, 9 comparing pharmacotherapy to control, and 3 comparing ERP to pharmacotherapy for

Group exposure and response prevention for college students with social anxiety: A randomized clinical trial.

OBJECTIVE: Social anxiety increases college student drop-out risk and stifles employment opportunities. Group cognitive-behavioral therapy with exposure (CBT ERP) has the potential to alleviate campus resource strain but remains under-researched with college students. The present study investigated the efficacy of group CBT ERP in a randomized clinical trial on a college campus. METHOD: Thirty-one postsecondary students were randomly assigned to an exposure-only group or an active control. RESULTS: Linear mixed-effects models indicated significant Group × Time interactions for general social

How technology influences the therapeutic process: evaluation of the patient-therapist relationship in augmented reality exposure therapy and in vivo exposure therapy.

BACKGROUND: New technologies have slowly become a part of psychologists' therapeutic office. However, many therapists still have doubts about the possibility of creating a good therapeutic relationship with patients in the presence of technology. AIMS: This study evaluates the development of the therapeutic alliance in individuals with small animal phobia disorder who were treated with Augmented Reality Exposure Therapy or In Vivo Exposure Therapy. METHOD: Twenty-two participants received an intensive session of cognitive behavioural therapy in either a technology-mediated therapeutic contex

The course of the working alliance during virtual reality and exposure group therapy for social anxiety disorder.

BACKGROUND: Psychoanalytic theory and some empirical research suggest the working alliance follows a "rupture and repair" pattern over the course of therapy, but given its emphasis on collaboration, cognitive behavioral therapy may yield a different trajectory. AIMS: The current study compares the trajectory of the working alliance during two types of cognitive behavioral therapy for social anxiety disorder - virtual reality exposure therapy (VRE) and exposure group therapy (EGT), one of which (VRE) has been proposed to show lower levels of working alliance due to the physical barriers posed

Does the form or the amount of exposure make a difference in the cognitive-behavioral therapy treatment of social phobia?

Exposure is considered to be an essential ingredient of cognitive-behavioral therapy treatment of social phobia and of most anxiety disorders. To assess the impact of the amount of exposure on outcome, 30 social phobic patients were randomly allocated to 1 of 2 group treatments of 8 weekly sessions: Self-Focused Exposure Therapy which is based essentially on prolonged exposure to public speaking combined with positive feedback or a more standard cognitive and behavioral method encompassing psychoeducation, cognitive work, working through exposure hierarchies of feared situations for exposure w

Synchrony and desynchrony in high and low arousal subjects undergoing therapeutic exposure.

A comparison of synchrony and desynchrony was made among phobic patients exposed to imaginal flooding under conditions of high and low arousal, respectively. Subjective anxiety (SUDs) and skin conductance (SC) were recorded at five minute intervals during three separate sessions. Results showed some degree of synchrony (small positive relationship) between the two measures for the high arousal group and desynchrony (no relationship) between these measures for the low arousal group. However, with the exception of two cases, one from each group, the findings were by no means clear-cut, suggestin

The impact of multiple traumatic events on habituation during exposure therapy in active-duty military and combat veterans.

Exposure therapy for posttraumatic stress disorder (PTSD) is efficacious for treating patients with multiple traumatic events. However, individuals with PTSD with exposure to multiple traumatic events are theorized to experience differences in emotional reactivity and fear extinction processes compared to those exposed to a single traumatic event. Thus, there is concern that individuals exposed to multiple traumatic events respond to exposure therapy differently than individuals reporting a single traumatic event. This study examined the impact of trauma frequency on exposure therapy processes

Baseline characteristics and treatment outcomes for people accessing a virtual partial hospital program versus a residential program for obsessive-compulsive disorder.

Exposure and response prevention (ERP) is the first-line treatment for obsessive-compulsive disorder (OCD). While previous literature supports the effectiveness of higher levels of specialized ERP-based care for OCD (i.e., partial hospital, residential), little is known about how outcomes for these levels of care compare to each other, or who is accessing different levels of specialized care. The present study compared treatment outcomes and baseline demographic, clinical, and prior treatment characteristics between adults with OCD or a related disorder accessing a virtual partial hospital pro

The Exposure Guide: A Practical Measure of Exposure Quality.

Quality monitoring is essential for the use of evidence-based interventions (EBIs) in both practice and research settings, yet few quality measures have been developed or validated for any treatment. This study examined the initial psychometric properties of a brief, practical measure of quality for exposure therapy (Exposure Guide; EG) in a sample of youth from three randomized clinical trials for pediatric OCD (N = 103 patients and 368 sessions). The EG was initially developed based on the behavioral principles underlying exposure, delivery factors linked to clinical outcomes in prior litera

Preferences for Justice-Based Exposure and Response Prevention Among Individuals With Identity-Related Obsessive-Compulsive Disorder.

Justice-based exposure and response prevention (ERP) has been touted as an alternative approach to (mis)uses and "Fear Factor" overcorrection applications of ERP for obsessive-compulsive disorder (OCD) with identity-related themes (i.e., sexual orientation, gender identity, racism, age, disability/diagnostic status, and economic-themed). Justice-based exposure maintains fidelity to the theoretical and evidence-based mechanisms of ERP while avoiding inadvertent stigmatization of marginalized communities. Justice-based ERP also avoids contributing to societal stigma while potentially increasing
