A model for quality improvement in the neonatal intensive care unit.

This article describes a global approach for improving care in the neonatal intensive care unit, using as a case study the process to decrease mortality in extremely low-birth-weight neonates at Lee Memorial Health System, The Children's Hospital of Southwest Florida. Key components to the development of an integrated quality improvement (QI) program--information systems, use of evidence-based medicine, and organizational structure and culture--are presented. The challenge of demonstrating true improvement and the need to better understand the cost of care associated with QI initiatives also a

Outbreaks of influenza A virus infection in neonatal intensive care units.

BACKGROUND: Outbreaks of nosocomial influenza virus infections have been described rarely during childhood and even less so in the neonatal period. METHODS: We report 30 neonates admitted to 2 neonatal intensive care units with nosocomial influenza A virus infection, which occurred in 2 outbreaks during 1999. Risk factors for infection were evaluated, and control measures were adopted. Virus was detected by indirect immunofluorescence antibody screen. Any infant with nasopharyngeal aspirate positive for influenza A virus was considered infected. RESULTS: Of 95 infants screened 30 were positi

Perspectives on the Use of Echinocandins in the Neonatal Intensive Care Unit.

The neonatal intensive care unit (NICU) population, especially low birth weight and critically ill neonates, is at risk of invasive Candida infections, which are associated with high mortality rates and unfavorable long-term outcomes. The timely initiation of an appropriate antifungal treatment has been demonstrated to enhance the prognosis. Factors that should be considered in the choice of an antifungal agent include the causative Candida strain, the presence and location of deep tissue infection, any previous use of antifungal prophylaxis, and the presence of implanted devices. Amphotericin

Improved survival of ventilated neonates with modern intensive care.

The records of 909 infants who required mechanical ventilation over a 42-month period were reviewed; 68.3% of the infants survived. The highest survival rate was in infants with persistent fetal circulation and hyaline membrane disease, and the lowest in infants with congenital anomalies and miscellaneous diseases. The survival rate for each year of the 3 1/2-year study increased from the previous year. There was also a dramatic increase in the survival rate in infants with a birth weight greater than 1,250 gm.

Echocardiography and management of sick neonates in the intensive care unit.

To evaluate the influence of echocardiographic examination in the clinical management of the sick neonate, 241 patients, admitted to the neonatal intensive care unit of a tertiary referral center that had echocardiograms with data available for review, were enrolled in a retrospective study. Asymptomatic murmurs (45%) followed by extracardiac anomalies/dysmorphic features (24%) were the most common clinical indications for requesting an echocardiogram. Congenital structural abnormalities (33%), hemodynamically significant patent arterial duct (3%), persistent pulmonary hypertension of the newb

Advances in Genetic Testing of Neurologically Abnormal Neonates in the Neonatal Intensive Care Unit.

Recent advances in cost and speed with which genome sequencing can be achieved have changed the approach to genetic testing in neonatal intensive care units (NICUs). In this article, we review the impact of these changes on the genetic evaluation of the neurologically abnormal neonate admitted to a NICU and share our center's approach to this important patient population, developed through multidisciplinary collaboration and reflective of a growing body of literature. We emphasize the importance of early identification of treatable disorders and other actionable findings.

Low birth weight and nosocomial infection of neonates in a neonatal intensive care unit.

A one year prospective surveillance of nosocomial infections (NI) in a neonatal intensive care unit (NICU) was performed. Among 229 neonates the infection rate was 27.1%, the infection proportion 20.1%, and the incidence density 21.9 infections per 1000 patient days. Infants were stratified into four birth weight categories. Degrees of infection ranged from 44.4% in the < or = 1000 g group to 10.1% in the > 2500 g group. Differences between the groups were statistically significant (P < 0.01). The mean birth weight of infants with NI was significantly lower than that of infants without NI (171

Risk factors for the development of neonatal sepsis in a neonatal intensive care unit of a tertiary care hospital of Nepal.

BACKGROUND: Sepsis is an overwhelming and life-threatening response to bacteria in bloodstream and a major cause of neonatal morbidity and mortality. Understanding the etiology and potential risk factors for neonatal sepsis is urgently required, particularly in low-income countries where burden of infection is high and its epidemiology is poorly understood. METHODS: A prospective observational cohort study was conducted between April 2016 and October 2017 in a level three NICU at a tertiary care hospital in Nepal to determine the bacterial etiology and potential risk factors for neonatal seps

Incidence of Hypoglycaemia and its Determinants in Neonates Admitted in Neonatal Intensive Care Unit.

INTRODUCTION: Baby in mother's womb depends on placental circulation for glucose. During birth, the clamping of the umbilical cord abruptly curtails the glucose supply from the placenta and thus neonates are more prone to develop hypoglycaemia and is preventable if we aware of the maternal and neonatal risk factors. This study aimed to identify the incidence of hypoglycaemia and its risk factors. METHODS: A prospective observational design was carried out among 115 neonates in neonatal intensive care unit (NICU) during December 2021 to January 2022. Convenience sampling technique was used to

Clinical Profile of Neonates with Hypernatremic Dehydration in an Outborn Neonatal Intensive Care Unit.

This hospital-record review describes the clinical profile of hypernatremic dehydration in neonates. 49 neonates (3.4% of the total admitted newborns) developed hypernatremic dehydration between January 2014 and August 2015. The major presenting complaints were fever (34.6%), poor feeding (42.8%), loose stools (40.8%) and lethargy (26.5%). The mean (SD) time needed for correction of hypernatremia was 38.6 (15.1) hours. Exclusively breastfed neonates had lesser complication rates of hypernatremic dehydration.

The uncertain 'best interests' of neonates: decision making in the neonatal intensive care unit.

Neonatology is a recently specialized area of medicine in which medical advances continue to save the lives of many ailing infants. However, like most areas of health care, it is not a perfect science, and hence not all infants can be saved, no matter how advanced the technology or how well-trained the physician. Inevitably, decisions regarding when to forego treatment must be made. It is essential to scrutinize the bases upon which these decisions are reached. This article examines the development of the scientific, legal and philosophical theories in Canada, the United States and the United

Significant improvement in survival outcomes of trisomy 18 with neonatal intensive care compared to non-intensive care: a single-center study.

BACKGROUND: Trisomy 18 syndrome, also known as Edwards syndrome, is a chromosomal trisomy. The syndrome has historically been considered lethal owing to its poor prognosis, and palliative care was primarily indicated for trisomy 18 neonates. Although there have been several reports on the improvement of survival outcomes in infants with trisomy 18 syndrome through neonatal intensive care, few studies have compared the impact of neonatal intensive care on survival outcomes with that of non-intensive care. Therefore, we compared the survival-related outcomes of neonates with trisomy 18 between i
