Tracheal intubation in children with Morquio syndrome using the angulated video-intubation laryngoscope.

PURPOSE: There are a number of syndromes with proven or suspected instability of the cervical spine especially in pediatric patients. It is a challenge for the anesthesiologist to intubate these patients with as little movement of the cervical spine as possible. A new device to facilitate this task is the angulated video-intubation laryngoscope (AVIL). CLINICAL FEATURES: The AVIL is a curved endoscopic intubation laryngoscope with angulated distal tip. The video-view from the distal blade tip improves glottic visualization during difficult direct laryngoscopy. We report three sisters with Mor

[Diagnosis and therapy of laryngeal diseases in the history of medicine. I: The pre-laryngoscopic era].

BACKGROUND: ANATOMY, PHYSIOLOGY AND PATHOLOGY: Hippocrates and Aristotle did not yet have a clear idea of the anatomy and physiology of the larynx. 500 years later Galenos carried out subtle studies on animals and elaborated a fairly precise description of the different cartilages, muscles and nerves of the larynx; he was the first to demonstrate the superior and the recurrent nerves of the larynx. He performed numerous experiments on animals with temporary or permanent interruption of the nervous function and thus developed a correct opinion of the vocal physiology. However, he did not draw c

Video-assisted endoscopic laryngosurgery using a direct laryngoscope and a long rigid endoscope.

Endolaryngeal microscopic laryngosurgery (microlaryngosurgery) using a direct laryngoscope is the preferred surgical method for treating laryngeal lesions under general anesthesia. However, this method does not provide a wide-angle view of the larynx and does not allow detailed observations of the ventricle and subglottis of the laryngeal cavity, resulting in blind areas. Video-assisted endoscopic laryngosurgery using a direct laryngoscope and a long transurethral rigid endoscope was therefore utilized to allow clear observations and complete resection of laryngeal lesions in these blind areas

"External counterpressure" and "internal distention" for optimal laryngoscopic exposure of the anterior glottal commissure.

External laryngeal counterpressure and internal laryngeal distention produce forces that are helpful for enhancing laryngoscopic exposure of the anterior glottis. These principles were formally described in the early 20th century, but are seldom used today. Hand pressure has been the typical source for external counterpressures. Since this maneuver is unstable if provided by an assistant and wasteful if provided by the surgeon, it is often neglected. Current phonomicrosurgical techniques require wider glottal exposure; therefore, a reexamination of the value of external counterpressure and int

Indirect laryngoscopic evaluation of vocal cord function in patients undergoing transhiatal esophagectomy.

Vocal cord function was assessed by indirect laryngoscopy before and after transhiatal esophagectomy for carcinoma of the middle and lower esophagus. Unilateral vocal cord palsy was found in 16 (34 percent) of 47 patients, but three of these patients were not hoarse and coughed normally. Four patients were hoarse postoperatively but had normal vocal cord function on indirect laryngoscopic examination. All vocal cord palsies but one were on the left side and the left side of the neck was used for esophageal mobilization with the only exception of the one patient who had a right cord palsy after

Surgical treatment of large epiglottic cysts with a side-opened direct laryngoscope and snare.

Two cases of large epiglottic cysts are reported. Histopathological examination revealed a retention cyst in one case and an epidermoid cyst in the other. We designed a side-opened direct larngoscope and snare for treating epiglottic cysts. This direct laryngoscope is characterized by an opening from the anterior tip to the posterior end of the right side. The removal of the cysts was accomplished by using this direct laryngoscope and a snare under inhalation anesthesia by fiber optic guided endotracheal intubation. The side-opened direct laryngoscope enabled easy insertion of the snare throug

Effectiveness of indirect versus direct laryngoscopes for nasotracheal intubation in adults undergoing elective surgery: a systematic review and network meta-analysis.

PURPOSE: It is not clear whether indirect laryngoscopes are superior to direct laryngoscopes for nasotracheal intubation and which laryngoscopes are better for nasotracheal intubation. We sought to undertake a ranking of the effectiveness of intubation devices in patients undergoing nasotracheal intubation. METHODS: We conducted a systematic review and network meta-analysis (NMA). We searched electronic databases for prospective randomized studies comparing different tracheal intubation devices in patients undergoing nasotracheal intubation. The primary outcome was intubation time. The second…

[Comparison of GlideScope® Cobalt and McGrath® Series 5 video laryngoscopes with direct laryngoscopy in a simulated regurgitation/aspiration scenario].

BACKGROUND: Video laryngoscopy has become increasingly important in airway management in the pre- and intrahospital settings. However, using video laryngoscopes in the presence of body fluids can make visualization of the airway difficult. A simulated regurgitation model was used to compare two video laryngoscopes (VL) with direct laryngoscopy. MATERIALS AND METHODS: A total of 72 physicians participated in this randomized trial. The hypopharynx from an Airway Management Trainer was filled with artificially warmed turbid liquid. In addition, the cervical spine of the manikin was immobilized. …

Comparison of intubating conditions in supine (sniffing) and 25° back-up position using channelled and non-channelled video laryngoscopes - A randomised controlled study.

BACKGROUND AND AIMS: The 25°back-up position is proposed to increase the efficacy of preoxygenation and provide better tracheal intubating conditions when using a direct laryngoscope. This study aimed to compare the ease of tracheal intubation between supine and 25° back-up positions when using two video laryngoscopes (VLS). METHODS: One hundred adults with normal airways and belonging to the American Society of Anesthesiologists physical status classes I and II, requiring general anaesthesia, were randomised in groups K and M. The trachea was intubated using King Vision and McGrath VLS in ei…

Comparison of Macintosh Laryngoscope, King Vision®, VividTrac®, AirAngel Blade®, and a Custom-Made 3D-Printed Video Laryngoscope for Difficult and Normal Airways in Mannequins by Novices-A Non-Inferiority Trial.

Background: Endotracheal intubation (ETI) is a cornerstone of airway management. The gold standard device for ETI is still the direct laryngoscope (DL). However, video laryngoscopes (VLs) are now also widely available and have several proven advantages. The VL technique has been included in the major airway management guidelines. During the COVID-19 pandemic, supply chain disruption has raised demand for 3D-printed medical equipment, including 3D-printed VLs. However, studies on performance are only sparsely available; thus, we aimed to compare 3D-printed VLs to the DL and other VLs made with …

Neuromuscular Block and Video Laryngoscope to Facilitate Intubation-A Survey of Current Practice in Denmark and Sweden.

BACKGROUND: Neuromuscular blocking agents (NMBA) provide optimal conditions for tracheal intubation. A high dose of opioid can be used as an alternative but may give suboptimal conditions for intubation. The use of a video laryngoscope for intubation might eliminate this potential difference. METHODS: A survey examining clinical practice regarding tracheal intubation with and without NMBA and use of video laryngoscopes was conducted in Sweden and Denmark. RESULTS: 1771 out of 3181 (56%) of invited anaesthetists responded. Overall, 1365/1771 (77%) preferred using NMBAs for non-acute tracheal …

[Pros/Cons: Airway Management for Separate-side Ventilation: Always Use a Video Laryngoscope?].

The optimal technique for airway management during intubation with a double-lumen tube (DLT) in thoracic anesthesia remains unclear. While efforts are now being made to use a video laryngoscope for routine use with single-lumen tubes, its use with DLTs remains a topic of debate. This article presents an analysis from two perspectives in the style of a debate, in this case with a slight wink as a plea in court, comparing video laryngoscopy (VL) with direct laryngoscopy (DL) for routine use. The "defense position" highlights evidence demonstrating improved visibility of the glottis, higher succe

The Advantage of Using Video Laryngoscope in Puncture and Incisional Drainage of Peritonsillar Abscess: A Case Report.

UNLABELLED: This case report demonstrates the use of a video laryngoscope to aid in the incision and drainage of a peritonsillar abscess in a 30-year-old male. This technique, which has not been widely discussed in the literature, provided enhanced visualization, overcoming challenges like poor access due to trismus and poor lighting. The video laryngoscope improved safety, offered clearer guidance, and provided valuable teaching opportunities, particularly for less experienced physicians and residents. This case contributes to the literature by illustrating how the video laryngoscope can func

[Use of video laryngoscope for tracheal intubation in patient with oral cavity mass: case report].

BACKGROUND AND OBJECTIVES: When planning the management of a predicted difficult airway, it is important to determine which strategy will be followed. Video laryngoscopy is a major option in scenarios with factors suggesting difficult airway access. It is also indicated in rescue situations, when there is tracheal intubation failure with direct laryngoscopy. The objective of the present report was to show the efficacy of using the video laryngoscope as the first device for a patient with a large tumor that occupied almost the entire anterior portion of the oral cavity. CASE REPORT: An 85 year

McGRATH MAC video laryngoscope for insertion of a transoesophageal echocardiography probe: A randomised controlled trial.

BACKGROUND: Transoesophageal echocardiography (TOE) probe insertion in anaesthetised patients can cause pharyngeal and oesophageal injuries. Kim et al. have shown that insertion assisted by a Macintosh laryngoscope can reduce such complications but it may sometimes be difficult to observe the passage of a TOE probe. The McGRATH MAC (McGRATH) has been shown to provide a better view of the glottis, piriform fossa and oesophageal inlet during tracheal intubation than the Macintosh. OBJECTIVE: We hypothesised that the McGRATH provided better visualisation of the oesophageal inlet and was useful a
