Studies in a number of laboratories have confirmed a deficiency in lymphocyte PHA mitogen response in patients with rheumatoid arthritis. Improvement in T cell function in these deficient patients had been noted with levamisole treatment. Agranulocytosis, which must be differentiated from leukopenia, was seen as the most severe side effect of levamisole in the treatment of rheumatoid arthritis (RA). By sending a detailed questionnaire to each investigator known to have been confronted with leukotoxic side effects, data were collected on 88 patients with agranulocytosis, 43 patients with leukopenia and three with thrombocytopenia. Many patients with steroid-sensitive nephrotic syndrome develop a relapsing course; therefore, alternative treatment may be necessary to avoid steroid toxicity. In this issue, a multicenter controlled study in relapsing steroid-sensitive nephrotic syndrome shows the effectiveness of levamisole. Time to first relapse was significantly increased compared with placebo. In addition, possible differential treatment effects were suggested for subgroups: patients with frequent relapses might have a superior response to those with steroid dependency.

During levamisole therapy, 14 of 20 patients with previously unresponsive rheumatoid arthritis had significant improvement (P less than 0.05) in clinical measures of disease activity, erythrocyte sedimentation rate, and rheumatoid factor titer in a 32-week double-blind placebo controlled crossover trial. BACKGROUND: Levamisole is frequently used as a steroid-sparing agent in children with steroid-sensitive nephrotic syndrome. Side effects, such as neutropenia, gastro-intestinal upset and skin rash, have been reported. In the current issue of Kidney International, Sinha et al. report results of a randomized controlled trial comparing levamisole and mycophenolate mofetil for the second-line treatment of children with frequent relapses of nephrotic syndrome. This study suggests that these two agents may both be safe and effective. The present commentary addresses strengths and limitations of this important study.
