Combination of a monofocal and one type of extended depth-of-focus (zonal refractive) intraocular lens (COMEDI) in bilateral cataract surgery protocol: a monocentric, randomised, parallel group trial in cataract surgery.

INTRODUCTION: Modern intraocular lens (IOL) designs for cataract treatment can be broadly classified into three focal range categories; monofocal, extended depth-of-focus (EDOF) and multifocal IOLs.Monofocal IOLs allow spectacle independence for one focus, typically distance. In contrast, EDOF IOLs provide a greater range of vision, extending spectacle independence to intermediate distance, while multifocal IOLs enable spectacle independence at all distances with the drawback of positive dysphotopsias and reduced contrast perception.EDOF lenses are an attractive compromise with fewer dysphotop

[Intraocular lens explantation and secondary implantation of a light-adjustable lens. Video article].

INDICATIONS: The operation is carried out to exchange previously implanted intraocular lenses (IOL) because of deviation from the target refraction with aniseikonia and anisometropia, in cases of contact lens intolerance, dissatisfaction with the IOL due to side effects and particularly problems with multifocal lenses. OBJECTIVE AND PRINCIPLES: The aim of this surgical approach is to retain the original implantation site of the exchange lens and capsular bag integrity with maximum sparing of the zonular fibers and secondary IOL implantation with stable positioning for fine adjustment of resid

Developments in the correction of presbyopia II: surgical approaches.

PURPOSE: To discuss the various static and dynamic surgical approaches which attempt to give presbyopes good vision at far, intermediate and … om the still-active ciliary muscle, zonule and capsule, or other sources. CONCLUSIONS: At present, multifocal intraocular lenses appear to offer the most consistent and reliable surgical approach to surgical presbyopic correction. They have obvious advantages in convenience and stability over optically-similar, simultaneous-image presbyopic contact lenses but this must be bal …

Pseudophakic mini-monovision.

Cataracts and presbyopia represent leading global causes of visual impairment, and demand for spectacle independence after cataract surgery continues to rise. Pseudophakic mini-monovision, a refined adaptation of traditional monovision employing milder anisometropia (-0.75 to -1.50 diopter), has emerged as an effective and versatile strategy to address this need. Compared with conventional monovision, mini-monovision better preserves stereopsis and binocular vision while providing satisfaction and spectacle independence approaching that of multifocal intraocular lenses (IOLs). Success is influ…

Non-contact lensless holographic reconstruction of diffractive intraocular lenses profiles.

A lensless compact arrangement based on digital in-line holography under Gabor's regime is proposed as a novel contactless method to assess the profile of multifocal intraocular lenses (MIOLs) which are conformed by several diffractive rings. Diffractive MIOLs are a widely adopted ophthalmologic option for the correction of presbyopia in patients undergoing cataract surgery. The MIOL optical design might introduce non-negligible optical performance differences between lenses as well as the introduction of undesirable photic phenomena (such as halos and glare) affecting the vision of users. The…

Pitfalls of Using NIR-Based Clinical Instruments to Test Eyes Implanted with Diffractive Intraocular Lenses.

The strong wavelength dependency of diffractive elements casts reasonable doubts on the reliability of near-infrared- (NIR)-based clinical instruments, such as aberrometers and double-pass systems, for assessing, post-surgery, the visual quality of eyes implanted with diffractive multifocal intraocular lenses (DMIOLs). The results obtained for such patients when using NIR light can be misleading. Ordinary compensation for the refractive error bound to chromatic aberration is not enough because it only considers the best focus shift but does not take into account the distribution of light energ…

[Pseudo-accommodation of diffractive multifocal lenses and monofocal lenses].

35 patients were examined for their pseudoaccommodation 6 to 18 months after cataract extraction and implantation of a diffractive multifocal intraocular lens (MIOL). While in 15 patients a MIOL had been implanted in both eyes 20 patients had a MIOL in one eye only. 45 type 3M 815 LE and 5 3M 825 XE lenses were implanted. The mean age was 58.2 years +/- 9.6 years (33 to 76). Only patients without any pathology except cataract and preoperative refractive errors < +/- 5 diopters of spherical and +/- 1 diopter of astigmatic refraction were eligible for implantation of MIOLs. Corneal Topography for Intraocular Lens Selection in Refractive Cataract Surgery.

The purpose of this review is to evaluate the usefulness of corneal topography to select premium intraocular lenses (IOLs), including aspherical IOLs, toric IOLs, and multifocal IOLs, in refractive cataract surgery. Corneal topography can detect corneal regular astigmatism, corneal irregular astigmatism (higher-order aberrations [HOAs]) including spherical aberration, and corneal shape abnormalities after corneal refractive surgery. Surgeons can explain to the patients with significant corneal HOAs about its effect on postoperative visual function before surgery. Multifocal IOLs should not be

Outcome of a Mix-and-Match Approach with a Monofocal Aspherical and a Bifocal Extended Depth-of-Focus Intraocular Lens to Achieve Extended Monovision in Cataract Patients.

BACKGROUND: Extended monovision is a novel mix-and-match approach that has been recently introduced. It involves implanting an aspherical monofocal intraocular lens (IOL) for distance vision in the dominant eye, and a bifocal extended depth-of-focus (EDOF) IOL in the nondominant eye. The target refraction for the nondominant eye is - 1.25 diopters (D), and provides good intermediate vision at 80 cm, with an additional 1.5 D power for near vision at 36 cm. This study evaluates the visual outcomes and patient-reported experiences with this extended monovision strategy in cataract patients. PATI
