Reliability of comprehensive echocardiography evaluation of patent ductus arteriosus among extremely preterm neonates across a national network: A prospective observational study.

BackgroundPrior studies on the reliability of targeted neonatal echocardiography (TNE) among extremely low gestational age neonates (ELGANs) with a patent ductus arteriosus (PDA) have been limited to evaluation of pre-defined images by a small set of study observers. The objective of this study was to investigate the interobserver reliability of comprehensive echocardiography measures of PDA size/shunt volume and ventricular performance among TNE-neonatologists in a large national network.MethodsWe conducted a prospective observational study. TNEs performed for the evaluation of PDA among ELGA…

Feeding practices for infants with gastroschisis: A survey of neonatal intensive care units in Canada.

BackgroundFeeding practices for infants with gastroschisis are often based on institutional protocols, clinician judgement, and usual practi … ructured interviews were conducted with 14 of 17 centers (82% response rate). Participants included neonatologists (12/14 centers) and dieticians (10/14 centers). Pediatric surgeons from 3/14 centers participated at the request of neonatologists. None of the centers reported using a gastroschisis feeding protocol routinely, although two centers noted occasional …

Deep-Learning-Based Multi-Class Classification for Neonatal Respiratory Diseases on Chest Radiographs in Neonatal Intensive Care Units.

INTRODUCTION: Accurate and timely interpretation of chest radiographs is essential for assessing respiratory distress and guiding clinical m … hospitals in Korea. Ground truth for manual classification of these conditions was generated by 20 neonatologists and validated by others from different hospitals. The dataset, consisting 34,598 for training, 4,370 for validation, and 4,370 for testing, was used to train a modified ResNet50-based deep-learning model for automatic classification. RESULTS: The a …

Breaking barriers: A Pediatrician's perspective on enhancing early diagnosis of developmental dysplasia of the hip.

BACKGROUND: Detecting Developmental Dysplasia of the Hip (DDH) early is vital for normal hip development (90% success). Taiwan's free baby check-ups target early diagnosis through neonatal exams and selective sonography. The study assesses a program's impact on high-risk patient selection and early DDH diagnosis. METHODS: In May 2022, a Quality Improvement (QI) program for Developmental Dysplasia of the Hip (DDH) was initiated, involving neonatologists, orthopedics, and administrators. A guideline-based bundle was developed, focusing on early detection. Pediatricians enrolled high-risk infant…

[Correction of Acidosis in Neonatal Intensive-care Medicine: A National Survey].

BACKGROUND: Metabolic acidosis is a common problem of patients on neonatal intensive care units. Only little data exists in literature and t … d 63 times with "zero". In perinatal asphyxia newborns with gestation age over 36+0 weeks 4% of the neonatologists would frequently perform a correction of acidosis, 74.3% would do it rarely and 21.8% never. In syndrome of persistent fetal circulation 28.4% would correct acidosis frequently, 42.0% would correct it rarely and 29.5% would never correct it. In case …

Cord blood full blood count parameters in Lagos, Nigeria.

INTRODUCTION: Full blood count (FBC), one of the most frequently requested for laboratory investigations, is a simple, fast and cheap test and is a reliable indicator of health. Due to its usefulness in the assessment of health status of individuals, its parameters in cord blood, a major source of haemopoietic stem cell transplantation and an ideal source for laboratory investigations for newborns were determined to provide a useful guide to local neonatologists and stem cell transplant physicians. METHODS: Three millilitres of umbilical cord blood was collected from 130 normal birth weight n…

Diagnostic practices and disease surveillance in Canadian children with congenital central hypoventilation syndrome.

OBJECTIVE: To assess the diagnostic and surveillance practices of Canadian pediatric subspecialists for children with congenital central hypoventilation syndrome (CCHS). METHODS: The present analysis was a prospective cross-sectional study. A web-based survey was sent to 303 pediatric subspecialists in Canada: 85 pediatric respirologists, 77 pediatric neurologists and 141 neonatologists. The survey included 36 questions about the current diagnostic and surveillance management of pediatric CCHS. Differences in responses among respirologists, neurologists and neonatologists were evaluated for e…

A decision aid for considering indomethacin prophylaxis vs. symptomatic treatment of PDA for extreme low birth weight infants.

BACKGROUND: Decision Aids (DA) are well established in various fields of medicine. It can improve the quality of decision-making and reduce decisional conflict. In neonatal care, and due to scientific equipoise, neonatologists caring for extreme low birth weight (ELBW) infants are in need to elicit parents' preferences with regard to the use of indomethacin therapy in ELBW infants. We aimed to develop a DA that elicits parents' preferences with regard to indomethacin therapy in ELBW infants. METHODS: We developed a DA for the use of the indomethacin therapy in ELBW infants according to the Ot…

Genetics of craniofacial malformations.

The field of craniofacial malformations is comprehensive and does not allow to discuss all craniofacial malformations which have been described as single entities. Many of the syndromes with craniofacial malformations are ultrarare. In this review we have chosen craniofacial malformation syndromes which are of relevance for the pediatrician, especially neonatologist: different types of craniosynostoses, oculo-auriculo-vertebral spectrum, Pierre Robin sequence and Treacher Collins syndrome. These syndromes will be described in detail. Diagnostic and therapeutic options will be discussed.

Decision-making at the borderline of viability: Who should decide and on what basis?

Parents and medical staff usually agree on the management of preterm labour at borderline viability, when there is a relatively high risk of long-term neurodevelopmental problems in survivors. If delivery is imminent and parents and staff cannot agree on the best management, however, who should decide what will happen when the baby is delivered? Should the baby be resuscitated? Should intensive care be initiated? Three ethicists, one of whom is also a neonatologist, discuss this complex issue.

Necrotizing fasciitis of the chest in a neonate in southern Nigeria.

We discuss the successful saving of a male neonate with necrotizing fasciitis of the chest following a hot fomentation of the umbilicus with exposure of the ribs and the pleural space on the right side. He recovered 5 weeks after admission. We stressed the need to recognize necrotizing fasciitis extending from the upper anterior abdominal wall to the chest following hot fomentation of the umbilicus. The need for multidisciplinary cooperation for excellent outcome is very important, that is, neonatologist, medical microbiologist, and plastic and chest surgeons.

Postnatal Outcomes and Surgical Implications of Somatex™ Thoracoamniotic Shunting for CPAM: A Multicenter Experience.

Intrauterine thoracoamniotic shunting in fetuses with congenital pulmonary airway malformation (CPAM) was first described using Cook™ or Rocket™ shunts. With the availability of the Somatex™ intrauterine shunt, a new device with the supposed advantages of less invasive placement and less frequent dislocations, pediatric surgeons and neonatologists are increasingly confronted with a new cohort of patients. Data on postnatal findings and the impact on surgical management are scarce. We conducted a multicenter retrospective study of all children born after prenatal treatment with a Somatex™ thorac…

Prioritising neonatal medicines research: UK Medicines for Children Research Network scoping survey.

BACKGROUND: The dosing regimen and indications for many medicines in current use in neonatology are not well defined. There is a need to prioritise research in this area, but currently there is little information about which drugs are used in UK neonatal units and the research needs in this area as perceived by UK neonatologists. METHODS: The Neonatal Clinical Studies Group (CSG) of the Medicines for Children Research Network (MCRN) undertook a 2 week prospective scoping survey study to establish which medicines are used in UK neonatal units; how many babies are receiving them; and what clini…

Mortality and morbidity by month of birth of neonates admitted to an academic neonatal intensive care unit.

BACKGROUND: Clinical expertise and skill of pediatric housestaff improve over the academic year, and performance varies despite supervision by faculty neonatologists. It is possible that variation in clinical expertise of housestaff affects important clinical outcomes in infants in ICUs. OBJECTIVE: Our goal was to test the hypothesis that there is a decrease in morbidity and mortality in infants admitted to an NICU over the course of the academic year. DESIGN/METHODS: A retrospective analysis was conducted using data on infants with birthweight 401 to 1500 g and >or=24 weeks' gestation (n = …

Bilirubin encephalopathy due to Rh incompatibility.

The authors present the case of a newborn of an Rh-factorsensitized mother, who received early hospital discharge while icteric only to be readmitted at an Emergency Service at five days of age with signs of kernicterus. Despite treatment given, the neonate progressed with a clinical picture of bilirubin encephalopathy. The lack of interaction between the obstetric and neonatal teams, premature hospital discharge, and lack of concern of neonatologists with jaundice in a full-term infant are highlighted as causes of a condition that should have disappeared if there had been adequate prevention.

[Palliative care in the neonatal period. Part one: general considerations].

In France, the law dated 22 April 2005 required that all practitioners offer palliative care to patients as an alternative to unreasonable obstinacy. The practical development of palliative care during the neonatal period is not easy, even though obstetricians and neonatologists have always been aware of the ethical necessity of comfort in the dying newborn. The decision leading to palliative care begins with the recognition of patent or potential unreasonable obstinacy, followed by withdrawing treatment and technical support, and finally a palliative care plan is drawn up with the medical tea…
