A flexible national living donor kidney exchange program taking advantage of a central histocompatibility laboratory: the Dutch model.

The shortage of deceased donor kidneys for transplantation has resulted in the expansion of living donation programs. A number of possibilities have been explored, since it became clear that donors do not need to be genetically related to their recipients. Apart from classical direct donation, other options such as paired exchange, list exchange, altruistic donation and domino paired exchange programs have been implemented. In the Netherlands, patients who cannot be transplanted with their potential living donor because of ABO blood group incompatibility or a positive crossmatch, have the opti

Developing a hospital emergency preparedness program.

The emergency preparedness program is not a static entity that only needs to be taken off the shelf and looked at once or twice a year. The various components of the program require that it be a daily part of the hospital's routine. Only by becoming a part of the everyday operation of the hospital can the full benefits of an emergency preparedness program be realized in an actual disaster. A part of the recovery activities after a hospital experiences an actual disaster should be to examine all phases of the emergency preparedness program. The program should be viewed as if it were being desig

Occupational safety and health: progress toward the 1990 objectives for the nation.

Occupational safety and health is 1 of 15 areas addressed in the Public Health Service's Objectives for the Nation. This area represents 104 million working men and women and the deaths, diseases, and injuries that result from exposures to hazards in their work environment. Characteristics of public health practice are compared with characteristics of occupational safety and health practice. The National Institute for Occupational Safety and Health (NIOSH), created by the Occupational Safety and Health Act, is discussed. NIOSH has developed a list of 10 leading work-related diseases and injuri

Affected Psychological Safety, Micromanagement, and the Risks of Full Time Return to Office Policies.

The objective of this article is to discuss organizational mandates for full return-to-office work. Concerns are emerging about the unintended impact on employee psychological safety, trust in employees' quality of work, well-being, and financial burden. This article outlines differences in hybrid work opportunities available to corporate and administrative staff with the limited flexibility of patient-facing healthcare professionals. It argues that rigid return-to-office mandates, particularly when combined with increased managerial oversight, can undermine autonomy, morale, engagement, and p

Greener Surgery, Greener Hospital: Organizational Strategies for Carbon Footprint Reduction in Anesthesia and Perioperative Departments.

Environmental sustainability has become an increasingly important priority in healthcare, particularly in anesthesia and perioperative services, where energy use, anesthetic gases, disposable materials, procurement, and waste contribute substantially to environmental burden. This narrative review examines how isolated environmental practices can be transformed into structured organizational strategies that can support sustained implementation and wider organizational adoption. A structured literature search was conducted in PubMed/MEDLINE and Scopus between May and August 2026, followed by rel

Organizational Tensions in the Implementation of Modifiable Off-the-Shelf Technologies in a University Hospital: Qualitative Multimethod Study.

BACKGROUND: Hospitals face increasing pressure to accelerate digital transformation. Modifiable off-the-shelf technologies (MOTs) combine standardized products with limited adaptability, offering promising opportunities for rapid digitalization. However, implementing MOTs in complex hospital settings involves multiple barriers, facilitators, and organizational dynamics that require deeper investigation. OBJECTIVE: This study identifies barriers and facilitators in the implementation of MOTs in hospitals and explores how organizational dynamics, conceptualized as tensions, emerge throughout th

Beyond attendance: participation and relational work in planned group recreational activities in a long-term care village setting.

BACKGROUND AND OBJECTIVES: Person-centered care (PCC) has shaped dementia care reforms and informed innovative long-term care models such as care villages. Recreational activities are often seen as a site where PCC is enacted, yet little empirical work has examined how participation in structured activities is enabled, mediated, and constrained through everyday social, relational, and organizational dynamics. This study examines how participation in structured group activities was produced in a care village. RESEARCH DESIGN AND METHODS: An ethnographic study was conducted in Canada's first pu

Compassion fatigue and COVID-19: A global view from nurses.

BACKGROUND: Due to the rapid spread of COVID-19 variants, the number of people hospitalized for COVID-19 may create psychological burdens on nurses. Nurses with high levels of compassion fatigue (CF) are more likely to make work errors, deliver poor-quality care, and have greater intent to leave their position. OBJECTIVES: This study utilized the social-ecological model to examine factors associated with nurses' CF and compassion satisfaction (CS) during the COVID-19 pandemic. METHODS: Data were collected from the United States, Japan, and South Korea from July to December 2020. The Professi

Early adoption of injectable naltrexone for alcohol-use disorders: findings in the private-treatment sector.

OBJECTIVE: The U.S. substance-abuse treatment system has been slow to adopt medications for the treatment of alcohol-use disorders (AUDs). The objectives of this study are to (a) determine how the inherent characteristics of injectable naltrexone (i.e., relative advantage, complexity, trialability, observability, compatibility) shape organizational-level decisions to adopt the medication and (b) identify key predictors of adoption and barriers that impede adoption. METHOD: This study uses data from a nationally representative sample of 345 privately funded U.S. substance-abuse treatment progr
