Minoxidil-induced pleuro-pericardial effusion is a diagnosis of exclusion after evaluation for other known causes of pericardial effusion. When complicated by cardiac tamponade, prompt pericardiocentesis and discontinuation of minoxidil can be lifesaving. We report a rare case of minoxidil-induced pleuro-pericardial effusion with tamponade in a patient with end-stage renal disease (ESRD) on hemodialysis who improved with pericardiocentesis and drug withdrawal.

INTRODUCTION: This case was designed to be an interprofessional scenario to assist the learner in the approach to a patient with cardiac tamponade. This resource provides information and materials for a high-fidelity simulation scenario that is appropriate for learners at multiple levels. Each learner has the opportunity to interact with embedded patient actors. This scenario includes the opportunity to conduct a bedside echocardiogram and perform an ultrasound-guided pericardiocentesis.
METHODS: A 51-year-old female presents to the emergency department with a chief complaint of shortness of …

Surgical pericardiotomy is often preferred as a primary option in patients with malignant pericardial effusions. Recent series have revealed that prolonged drainage substantially reduces pericardial effusion recurrence rates, even in the setting of malignancy. The aim of the study was to directly compare the efficacy of pericardiocentesis with prolonged drainage with the primary surgical pericardiotomy in patients with symptomatic pericardial effusion associated with a malignancy. We retrospectively evaluated 88 patients who presented with pericardial tamponade associated with a malignancy. Pe…

Coronavirus disease 2019 (COVID-19)-induced pericarditis and pericardial myocarditis are common entities; however, the development of perica … ge pericardial effusion leading to cardiac tamponade within one day of initial presentation. Urgent pericardiocentesis was performed but was complicated by rapid decompensation of the patient, which has been assumed to be ECP following pericardiocentesis and PDS. Close monitoring of acute pericarditis with pericardial effusion is required in these patients for the …

Pericardial effusion (PE) prognosis depends on the underlying etiology. We sought to assess the outcome of patients with chronic (>3 months), large (diastolic echo-free space >2 cm), idiopathic (without apparent etiology), C-reactive protein (CRP) negative PE, a topic where data are lacking. A total of n = 74 patients were enrolled in this study. Patients were treated by pericardiocentesis (n = 39) or surgical pericardial "window" (PW) (n = 13) or conservatively (n = 22). The median follow-up was 24 months (interquartile range: 15 to 38). Among those patients who had PE drained (n = 52), PE re…

Cardiac radiotherapy is rarely used in clinical practice because of concern of adverse effects on the heart. We present a case of a 64-year-old man with advanced small cell lung cancer (SCLC) treated with chemo-radiotherapy who attained partial remission initially but had disease progression to bulky cardiac metastasis and significant pericardial effusion. Severe heart failure with hepatic failure was found. Chemotherapy and pericardiocentesis were contraindicated because of the associated high risk and bleeding tendency. Emergent palliative cardiac radiotherapy resulted in rapid improvements …

We present a rare case of iatrogenic pneumopericardium, pneumoperitoneum, and Escherichia coli pericarditis after emergency pericardiocentesis for pericardial tamponade. The patient had profound bowel distention at the time of the procedure that led to iatrogenic pericardioperitoneal fistula formation along with transverse colon perforation, which manifested later after pericardial drain removal. This condition required repeat pericardiocentesis, laparoscopic colon repair, a long course of antibiotics, and an eventual pericardial window. (Level of Difficulty: Intermediate.).

Rapid accumulation of pericardial fluid can lead to tamponade, resulting in cardiac chambers' collapse, which can lead to hemodynamic and clinical instability, potentially needing emergent pericardiocentesis. Pleural effusion should also be considered as a potential, if rare, cause of cardiac chambers' collapse and possibly cardiac tamponade. This phenomenon has clinical implications because hemodynamically unstable patients with moderate to large pleural effusion may actually need thoracentesis instead of massive volume resuscitation, inotropic agents, or pericardiocentesis.

BACKGROUND: Cholesterol pericarditis is a rare condition characterized by cholesterol crystal accumulation in the pericardium, often linked to hyperlipidemia and chronic inflammatory disorders.
CASE SUMMARY: A 50-year-old woman with a history of hypercholesterolemia presented with progressive dyspnea, chest pain, and fatigue. Clinical evaluation and imaging studies revealed a massive pericardial effusion. Pericardiocentesis was performed, and fluid analysis demonstrated the presence of cholesterol crystals. The patient was treated with pericardiocentesis, nonsteroidal anti-inflammatory drugs,…

Purulent pericarditis is an extremely rare disease accounting for <1% of pericarditis cases. Purulent pericarditis with hemodyamically unstable tamponade if untreated is fatal. Furthermore, although idiopathic polymicrobial disease is documented, a combination Haemophilus parainfluenzae, Prevotella buccae, and Citrobacter freundii have not been found in the literature by the authors. What follows is a case of a 58-year-old male who presented to the emergency department (ED) with these features and underwent emergent bedside pericardiocentesis and a brief review of current pericardiocentesis te…

An experimental animal model was utilized to evaluate the role of simultaneous two-dimensional echocardiography combined with a modified electrocardiographic monitoring technique for improving the safety of pericardiocentesis. This technique avoids the hazards of placing the sharp pericardiocentesis needle against the heart in order to determine its location against the epicardial surface. This technique was evaluated in eight dogs in over 100 aspiration attempts. Cardiac trauma occurred only in a single incidence with the use of a 20-gauge cannula. A detailed description of this procedure is …

The central tendon defect type of congenital diaphragmatic hernia (CDH) is extremely rare and usually associated with a significant pericardial effusion. Prenatal diagnostic ultrasound features of this quite rare entity remain often overlooked or misdiagnosed. There is a dearth of literature about the role of prenatal intervention, often through an elective pericardiocentesis, for the prevention of lung hypoplasia and to decrease the overall neonatal morbidity and mortality. To the best of our knowledge, till date, there is only one case that was subjected to a prenatal intervention. Here, we …

Primary chylopericardium proved to be the (rare) cause of chronic, symptom-poor pericardial effusion in a 19-year-old man. Echocardiography and pericardiocentesis provided a firm diagnosis. Lymphography demonstrated extravasation into the mediastinum but no direct connection into the pericardial sac. Ligation of the thoracic duct at diaphragmatic level and pericardiostomy achieved a cure. Secondary chylopericardium requires treatment of the underlying disease, e.g. mediastinal tumor. Chylopericardium after thoracic or cardiac operation without signs of tamponade is preferably treated conservat…

Two cases are presented, one with chronic renal failure and the other with a hepatic cyst perforated in the pericardium, with a tension pericardic leakage accompanied with by electrocardiographic alternance. After correct treatment, pericardiocentesis and surgery, respectively, the electrocardiogram lost its electric alternance. The echocardiogram evaluates the importance of the pericardic liquid volume which causes, because of the particular cardiac anatomy, an alternating movement of the heart within the pericardic sac. This factor would be decisive in the origen of the electric phenomenon, …
