Severe foreign body reaction with spontaneous extrusion of glue after cyanoacrylate closure of the great saphenous vein.

Noninfectious inflammatory complications-such as phlebitis and systemic hypersensitivity reactions-can occur after cyanoacrylate closure for lower extremity varicose veins. In most cases, these reactions resolve within approximately 1 month, but in rare situations they may persist and become difficult to manage. This report describes a case of phlebitis after cyanoacrylate closure in which a severe foreign body reaction, rather than hypersensitivity or allergy, led to the formation of multiple fistulae. Through these fistulae, the cyanoacrylate polymer was eventually expelled, resulting in gra…

Complications Related to the Securement Device in Peripheral Intravenous Catheters: A Randomized Study.

Studies have not demonstrated the effectiveness of the different types of dressings in reducing the rate of complications. The purpose of this study was to determine which type of dressing is most beneficial in reducing the rate of complications. A total of 281 patients requiring a peripheral intravenous catheter were randomized to receive partially reinforced dressings or fully reinforced dressings (dressings with integral catheter securement). Patients were followed throughout their entire catheter course, and complications included infection, occlusion, phlebitis, accidental dislodgement, e…

Peripheral venous nutrition in surgical patients: techniques, indications and results.

Peripheral venous nutrition (PVN) has been proposed to avoid complications resulting from central venous catheterization in surgical patients. Using lipid emulsions 3 standard diets (with final osmolarity not higher than 900 mOsm/1, non-protein calories from 1675 to 2177, electrolytes 110 mEq, Cal/N ratio from 130/1 to 170/1) were worked out. They are suitable for total nutrition of non-hypercatabolic patients and for nutritional postoperative maintenance of patients undergoing major digestive tract surgery. The three diets were tested in 118 patients. Frequency and type of phlebitis and the n…

[Eales disease: a case report in Burkina Faso].

The authors report the first case of Eales disease described in Burkina Faso. A 21-year-old man presented myodesopsia with acute visual acuity decrease in the right eye at 1/50, vitreous haemorrhage, and retinal phlebitis of the large trunks. In the left eye, visual acuity was 10/10 P2, with phlebitis associated with retinal haemorrhages, vascular occlusion, and retinal ischaemia. Laboratory investigations found a positive TST at 20mm but the Koch bacillus was not isolated. The patient received treatment for tuberculosis, corticosteroids, retinal argon laser photocoagulation, and vitrectomy in…

[Severe pulmonary embolism disclosing a deficiency in protein C].

A constitutional deficit in protein C is a rare disorder. Our report concerns a 35 year old non-smoking male, presenting with a past history of uncomplicated phlebitis of the lower limbs. In the course of a new episode of phlebitis, the patient presented with a severe pulmonary embolus treated by embolectomy under extracorporal circulation, with interruption of the inferior vena cava. The plasma level of protein C (Elisa technique) measured before treatment with antivitamin K was 40% (normal 70-140%). The absence of associated hepatic or haematological anomalies confirmed the constitutional ch…

Exploration of the Current State of Peripheral Intravenous Catheter Complications and Documentation: A Point Prevalence Study.

Peripheral intravenous catheters (PIVCs) are the most commonly used invasive devices in acute care hospitals, with nurses being primarily re … m sites having the longest dwell time. Overall inter-rater reliability (IRR) for an infiltration or phlebitis score of 0 was high (97.4% and 92%, respectively). However, overall agreement was only 77.16% for infiltration and 40.07% for phlebitis, with significant disagreement as scores increased. Study findings support that there was strong compliance with …

Aseptic Venous Abscess Caused by a Foreign Body Granuloma Following Cyanoacrylate Glue Embolization for Chronic Great Saphenous Vein Reflux: A Case Report.

This report describes the case of an 88-year-old male who underwent cyanoacrylate glue embolization (CAE) for right great saphenous vein (GSV) reflux. Three months postoperatively, the patient presented with phlebitis and a subcutaneous abscess along the occluded right GSV. Surgical removal of the GSV and associated skin fistula was successful. Cultures of the excised tissue showed no evidence of bacterial infection. Histological analysis demonstrated a foreign body granuloma and phlebitis complicated by an aseptic abscess. The postoperative course was uneventful, with no recurrence at 1.5 yea…

Octyl-butyl cyanoacrylate glue versus splint for stabilising short peripheral intravenous catheters in neonates: a retrospective cohort study.

AIM: This study was conducted to compare the effectiveness of octyl-butyl cyanoacrylate glue or limb splinting used alone or in combination for stabilising neonatal short intravenous peripheral catheters (SPCs). MATERIALS AND METHODS: This retrospective cohort study analysed routinely collected data from a tertiary neonatal intensive care unit. A total of 45 754 successfully inserted SPCs were grouped into one of three stabilisation strategies: splint+glue, splint only and glue alone. The primary outcome was SPC dislodgement. Secondary outcomes included dwell time, phlebitis, infiltration and…

Complication risks associated with lower versus upper extremity peripherally inserted central venous catheters in neonates with gastroschisis.

PURPOSE: To compare the complication rates of lower extremity (LE) and upper extremity (UE) peripherally inserted central catheters (PICCs) in neonates with gastroschisis. METHODS: In this retrospective comparative study, neonates with gastroschisis admitted to a level IIId NICU between 2004 and 2013 were identified. Catheter dwell time and complication rates (infiltration, phlebitis, occlusion, migration, infection and thrombosis) between the initial UE and LE PICCs were compared. RESULTS: Forty (31%) and eighty-nine (69%) neonates with gastroschisis had their initial PICCs placed from thei…

Incidence and type of peripheral intravenous catheter complications documented in hospitalised dogs.

OBJECTIVE: To report the incidence and type of peripheral intravenous catheter complications in hospitalised dogs. METHODS: A prospective, observational trial was performed. Peripheral intravenous catheters were monitored for complications. Complications were documented and classified as extravasation, phlebitis, dislodgement, occlusion and line breakage. If phlebitis was present, the Visual Infusion Phlebitis Scale was used to assign a grade (0 to 5). Fisher's exact test was used to compare the type of complications between the critical care unit and the intermediate care unit. A univariate …

"AFGP" bundles for an extremely preterm infant who underwent difficult removal of a peripherally inserted central catheter: A case report.

BACKGROUND: There have been few reports on level 3 difficult removal of peripherally inserted central catheter (PICC) in neonates. Here, we reported a case of an extremely preterm infant who underwent level 3 difficult removal of a PICC. CASE SUMMARY: Female baby A, weighing 1070 g at 27+1 wk of gestational age, was diagnosed with extremely preterm infant and neonatal respiratory distress syndrome. She underwent PICC insertion twice. The first PICC insertion went well; the second PICC was inserted in the right lower extremity, however, phlebitis occurred on the second day after the placement.…

Variability in the Clinical Presentation of Major Catheter-Related Complications: A Case Report.

Phlebitis and infiltration have traditionally been considered distinct conditions, and numerous studies have used these terms to classify outcomes. However, classic diagnostic criteria may not consistently or accurately differentiate between the 2 clinical conditions. The current case report presents examples demonstrating that phlebitis and infiltration are heterogenous conditions with varied and overlapping signs that do not always align with conventional definitions. This variability in presentation may hinder distinguishing between these complications and may contribute to inconsistencies …

[Venous thrombosis in Behçet's disease].

Venous lesions in Behçet's disease (BD) were defined by Adamantiades and represent one of the most suggestive signs of the disease. They are occasionally the first sign of the disease and are frequently the basis for the diagnosis in a case of recurrent thrombosis in a young subject, the preferential context of BD. Involvement of superficial vessels is virtually constant. Venous vasculitis is responsible for non-specific hypersensitivity and erythema nodosa, which constitute some of the major diagnostic criteria. Ocular periphlebitis is one of the elements responsible for posterior uveitis. Th…

Disabling Outcomes After Peripheral Vascular Catheter Insertion in a Newborn Patient: A Case of Medical Liability?

BACKGROUND The positioning of peripheral venous catheters (PVC) is an invasive procedure commonly performed in pediatrics hospital wards to obtain vascular access for the administration of fluids, medications and other intravenous (IV) therapies. Many studies exist about management of peripheral venous access in adults. On the contrary, scientific evidence on the management of this procedure in children and newborns, especially regarding the optimal duration of infusion and the possible related side effects, is still poor. To minimize the risk of phlebitis, the guidelines of the US Centers for…

Vasculitis and pyrexia associated with superficial spreading gastric carcinoma.

A case of low-grade fever developing about a month before the discovery of gastric carcinoma is reported. No findings of infection or collag … ls did not exhibit any inflammatory changes. It was presumed that the gastric carcinoma had induced phlebitis, which subsequently brought about the fever. As to the pathogenetic mechanism, it was suggested that a substance produced by the carcinoma cells flowed into nearby veins to induce the phlebitis.
