[Application of interventional radiology for stenosis of vascular anastomosis in living-donor liver transplantation].

Stenosis of vascular anastomosis is a significant complication leading to graft loss after liver transplantation. For the diagnosis of portal vein stenosis, clinical signs of portal hypertension such as ascites and thrombocytopenia, stenosis and/or poststenotic dilatation on ultrasonography (US), and jet flow, rambling, or scarcity flow of the intrahepatic portal vein on Doppler US are useful. Three-dimensional computed tomography is used to confirm the indications for interventional radiography (IVR) to treat portal stenosis. For the diagnosis of hepatic vein stenosis, clinical signs such as

Endoscopic Removal of Impacted Barium Stools Using a Snare and a Long Hood: A Rare Complication Following Upper Gastrointestinal Barium X-ray Radiography (With Video).

Barium sulfate is widely used as a radiographic contrast medium in upper gastrointestinal barium X-ray radiography for cancer screening in Japan. Although generally considered safe, rare complications such as bowel obstruction due to impacted barium stools can occur. We report the case of a healthy 41-year-old woman who developed colonic obstruction following upper gastrointestinal barium X-ray radiography. Endoscopic treatment was attempted using a snare and a long hood to fragment and remove the barium mass. The impacted stools were successfully removed endoscopically, and there were no seri

Congenital radioulnar synostosis: is prenatal diagnosis possible? - A case report.

OBJECTIVES: The main objectives of this case report are to discuss prenatal ultrasound findings of congenital radioulnar synostosis and to review the literature. CASE REPORT: A patient was diagnosed with congenital radioulnar synostosis at eight months old when parents noticed limited motions in the child's left forearm. The parent denied any traumatic or family history of bony malformations. Physical examination by a pediatric orthopedics specialist and digital radiography revealed proximal radioulnar synostosis. The case report includes perinatal course, comparison between the postnatal rad

Small bowel obstruction in an adolescent with pelvic inflammatory disease due to Chlamydia trachomatis.

A 19-yr-old adolescent, who was hospitalized because of pelvic inflammatory disease (PID) due to Chlamydia trachomatis, developed bile-stained emesis. A mild amount of free fluid in the pelvis was found on abdominal ultrasound but there was no sonographic evidence of a pelvic mass or of a tubo-ovarian abscess. Plain radiography and computer tomography (with contrast) of the abdomen revealed a high-grade partial small bowel obstruction. Conservative treatment, which included intravenous fluids and antibiotics together with continuous bowel decompression via nasogastric tube, led to resolution o

Work-related musculoskeletal symptoms among Saudi radiologists: a cross-sectional multi-centre study.

BACKGROUND: Musculoskeletal disorders are common health problems worldwide. Several factors cause these symptoms, including ergonomics and other individual considerations. Computer users are prone to repetitive strain injuries that increase the risk of developing musculoskeletal symptoms (MSS). Radiologists are susceptible to developing MSS because they work long hours analysing medical images on computers in an increasingly digitalised field. This study aimed to identify the prevalence of MSS among Saudi radiologists and the associated risk factors. METHODS: This study was a cross-sectional,

Pleural gas gangrene secondary to esophageal injury by an ingested fish bone.

We report a 73-year-old woman with right pleural gas gangrene, treated successfully by thoracoscopic debridement. The clinical course and chest computed radiography and thoracoscopy findings suggested that her condition resulted from a relatively rare esophageal injury after she accidentally ingested a fish bone. Video-assisted thoracoscopic intervention has proved useful in cases involving pleural gas gangrene.

Recurrent Primary Spontaneous Pneumothorax in an 18-Year-Old Man: A Case Report and Review of Considerations for Prophylactic Contralateral Pleurodesis.

Primary spontaneous pneumothorax (PSP) is defined as the accumulation of air in the pleural space originating from within the lung parenchyma without preceding trauma, which can lead to a stable or tension pneumothorax. Clinical suspicion is key for diagnosis, with chest radiography serving as the primary diagnostic tool. Management ranges from needle aspiration to tube thoracostomy with consideration of surgical intervention for recurrent cases. We present an 18-year-old African-American man who experienced three episodes of radiography-confirmed PSP occurring over a seven-month period. This

Image-guided Sports Medicine and Musculoskeletal Tumor Interventions: A Patient-Centered Model.

The spectrum of effective musculoskeletal (MSK) interventions is broadening and rapidly evolving. Increasing demands incite a perpetual need to optimize services and interventions by maximizing the diagnostic and therapeutic yield, reducing exposure to ionizing radiation, increasing cost efficiency, as well as identifying and promoting effective procedures to excel in patient satisfaction ratings and outcomes. MSK interventions for the treatment of oncological conditions, and conditions related to sports injury can be performed with different imaging modalities; however, there is usually one o

As fast as an X-ray: real-time magnetic resonance imaging for diagnosis of idiopathic scoliosis in children and adolescents.

BACKGROUND: Idiopathic scoliosis is common in adolescence. Due to the rapid growth of the spine, it must be monitored closely with radiographs to ensure timely intervention when therapy is needed. As these radiographs continue into young adulthood, patients are repeatedly exposed to ionizing radiation. OBJECTIVE: This study aimed to investigate whether real-time magnetic resonance imaging (MRI) is equivalent to conventional radiography in juvenile idiopathic scoliosis for determining curvature, rotation and the Risser stage. Additionally, the time requirement should be quantified. MATERIALS

Intracavitary electrocardiography-guided positioning of central vascular access device can spare unnecessary ionizing radiation exposure in pediatric patients.

BACKGROUND: Most hospital protocols-including those of our own institute-require the use of radiography to validate tip position in every central vascular access device placement. This study evaluated whether unnecessary ionizing radiation exposure could be spared in the pediatric population when intracavitary electrocardiography is used to guide catheter placement. MATERIAL AND METHODS: Retrospective study of intracavitary electrocardiography-guided central vascular access device placements in our pediatric surgery department between 2013 and 2018. We evaluated the operating time, success in

Threading the needle: Tips and tricks for challenging percutaneous abdominopelvic biopsies.

The growing use of biomarker testing for personalized oncologic care has increased the demand for complex and repeated percutaneous biopsies in abdominal and interventional radiology practices. Challenging biopsies can be daunting, requiring a careful assessment of feasibility, safety, and risk tolerance. Several strategies can help optimize the chances of achieving success. Such strategies include leveraging the advantages of the modality chosen for image guidance, ensuring patient comfort, and the use of special techniques, such as IV contrast, navigational tols, and biopsy needle modificati

Assessing clinical complexity in fluoroscopy-guided interventional procedures using the modified Delphi method: a pathway to define diagnostic reference levels (DRLs).

AIM: Diagnostic reference level (DRL) implementation for interventional radiology (IR) is challenging, considering exposure-value variation for the same procedure, due to different complexity levels. This study aims to define clinical criteria to assess the complexity level for three fluoroscopy-guided interventional procedures (FGIPs) in Portugal: percutaneous transhepatic biliary drainage (PTBD), transarterial chemoembolisation (TACE), and transjugular intrahepatic portosystemic shunt (TIPS). MATERIALS AND METHODS: The modified Delphi method, a formal and standard group consensus process th

Unique usage of a partially covered metal stent for drainage of a pancreatic pseudocyst via endosonography-guided transcystgastrostomy.

Pancreatic pseudocysts are frequent complications of pancreatitis episodes. The current therapeutic modalities for drainage of pancreatic pseudocysts include surgical, percutaneous, and endoscopic drainage modalities. Endosonography-assisted endoscopic drainage of these pseudocysts with the placement of multiple plastic or fully covered self-expanding biliary metal stents is becoming more commonly carried out. The present case report discusses the unique and successful drainage of a pancreatic pseudocyst with the placement of a partially covered self-expanding metal stent.

Use of a straight, side-hole delivery sheath for improved delivery of Amplatzer ASD occluder.

The Amplatzer ASD occluder may be difficult to position in some patients with a large atrial septal defect (ASD) or deficiency of one or more atrial septal rims. We developed a method to modify a Mullins transseptal sheath to enhance delivery. The resulting sheath is straight and has an exit orifice essential in the side of the distal portion of the sheath-a straight, side-hole (SSH) delivery sheath. We have used this modified delivery sheath in 140 successive patients with excellent results. The techniques of sheath modification and delivery of the device using the modified sheath are describ

CT diagnosis of acquired small bowel volvulus.

Small-bowel volvulus is an uncommon but important cause of small-bowel obstruction and often results in ischemia or infarction. Clinical examination and plain film radiography may be nondiagnostic, leading to delay in surgical intervention with subsequent increase in morbidity and mortality. We present two patients in whom the diagnosis of strangulating small-bowel volvulus was made by computed tomography (CT), allowing rapid surgical correction of this potentially life-threatening condition.

The spectrum of heterotaxic syndromes.

The heterotaxic syndromes may manifest as complex disorders with multiple anomalies, which have been described in the polysplenia/asplenia syndromes, or as a single isolated anomaly. Clinically insignificant anomalies may be incidentally discovered and should be recognized. Early diagnosis in the prenatal and neonatal periods allows for early intervention and correction, particularly of cardiac anomalies. Chest radiography, sonography, CT, radionuclide imaging, and MR imaging are useful diagnostic tools in evaluating these patients.
