Inhibition of the ERK pathway prevents HIVgp120-induced REM sleep increase.

Approximately 35% of HIV-infected subjects, both children and adults, exhibit alterations in the sleep-waking cycle. HIV surface glycoprotein gp120 has been postulated to contribute to this abnormality. For example, it has been reported that HIVgp120 modifies sleep in freely-moving rats and that it also activates the ERK pathway in brain slices. The goal of this work was to determine if sleep changes induced by HIVgp120 in normal rats are mediated by the MAPK pathway. Our results show that a single intraventricular administration of HIVgp120 selectively increases REMS and that such an increase

REM sleep deprivation increases early morning pineal melatonin in castrated rats.

Recently nighttime melatonin levels have been shown to be attenuated in depressive patients or patients with dementia of the Alzheimer type. On the other hand, depression can be transiently relieved by deprivation of rapid eye movement sleep. Since exogenous melatonin administration increases rapid eye movement sleep and slow wave sleep in the rat, could rapid eye movement sleep deprivation then inversely influence endogenous melatonin production? We found indices that in castrated Wistar rats 4 days of rapid eye movement sleep deprivation by the cuff pedestal method elevates the pineal conten

Basal electromyographic amplitude in rapid eye movement (REM) and non-rapid eye movement (NREM) sleep as a predictor of disease progression in isolated REM sleep behavior disorder.

STUDY OBJECTIVES: This study examined whether basal electromyographic (EMG) amplitude during sleep is altered in patients with isolated REM sleep behavior disorder (iRBD) and its relationship to disease progression, as well as changes across the sleep cycle. METHODS: Baseline polysomnograms collected from 95 iRBD patients (15 converters, 80 non-converters) and matched 24 controls were analyzed. EMG amplitude was calculated from submentalis recordings across all sleep stages using 3-s mini-epochs, with REM sleep further classified as REM sleep without atonia (RWA) and non-RWA (RA) according to

The Syn-sleep trial protocol: detection of cutaneous phosphorylated alpha-synuclein in REM sleep behavior disorder.

BACKGROUND: Idiopathic rapid eye movement sleep behavior disorder (iRBD) is a prodromal neurodegenerative disease of misfolded alpha-synuclein (P-SYN) with a high risk of phenoconversion to a clinically apparent synucleinopathy (including Parkinson's disease, multiple system atrophy, or dementia with Lewy bodies) over 15 years. OBJECTIVES: To determine rates of cutaneous P-SYN deposition in iRBD, to quantify changes in PSYN deposition over time, and to determine if P-SYN deposition patterns and amounts predict phenoconversion to a specific type of synucleinopathy. CLINICAL TRIAL PROTOCOL: In

Rapid eye movement sleep: Who needs it? Creativity mechanisms and psychiatric applications of REM sleep enhancement.

Rapid eye movement sleep (REMS) has historically been associated with anecdotal 'creative insights', possibly due to the fantastical and ostensibly illuminating nature of its associated phenomena (dreams). REMS, characterised by rapid eye movements, muscle atonia, and high-energy neuronal activity, has been linked to memory consolidation and information processing, particularly regarding the formation of novel associations or reintegration of consolidated memories into new cognitive networks. However, studies in these domains have largely used methodology which deprived subjects (animal or hum

Rapid Eye Movement (REM)-Predominant Central Sleep Apnea With Alpha-Delta Sleep Pattern: A Case Report.

Central sleep apnea (CSA) occurring predominantly during rapid eye movement (REM) sleep represents an unusual clinical phenomenon, as CSA typically occurs during non-rapid eye movement (NREM) sleep. We present the case of a 32-year-old male with severe obstructive sleep apnea (OSA), with an apnea-hypopnea index (AHI) of 65.5/hr, who exhibited central apneas mainly during REM sleep (central apnea index: 35.9/hr in REM vs. 5.2/hr in NREM), along with concurrent alpha-delta sleep pattern during slow-wave sleep. The patient was taking tramadol 100 mg and pregabalin 300 mg daily for chronic back pa

Correlations with REM sleep behavior disorder severity in isolated rapid eye movement sleep behavior disorders patients.

OBJECTIVES: The pathogenesis of isolated rapid eye movement sleep behavior disorders (iRBD) is poorly understood. The severity of RBD may reflect its pathogenesis. METHODS: We compared motor function and non-motor symptoms (NMSs) between iRBD patients and healthy volunteers. We correlated motor function, NMSs, and striatal dopaminergic activity with RBD severity using video-polysomnography. RESULTS: Twenty-one iRBD patients and 17 controls participated. The Unified Parkinson's Disease Rating Scale part III scores were higher in patients compared to controls (p < 0.001). There was no differen

EEG Power Spectral Analysis of Abnormal Cortical Activations During REM/NREM Sleep in Obstructive Sleep Apnea.

Objective: To characterize electroencephalogram (EEG) power in different frequency bands during rapid eye movement (REM) sleep and non-rapid eye movement (NREM) sleep in patients with obstructive sleep apnea (OSA). Methods: Retrospective data on 151 patients were collected and divided into three groups: primary snoring group (AHI < 5/h), mild-moderate OSA group (6 ≤ AHI < 30/h), and severe OSA group (AHI ≥ 30/h). EEG recordings in the frontal, central, and occipital regions were extracted from both REM and NREM sleep, to compute the normalized spectral power densities in the delta, theta, alph

Melatonin Effects in REM Sleep Behavior Disorder Associated with Obstructive Sleep Apnea Syndrome: A Case Series.

OBJECTIVES: REM sleep behavior disorder (RBD), with its main clinical symptoms of nightmares with dream-enacting behavior, is considered as a possible precursor of neurodegenerative disease. Obstructive Sleep Apnea Syndrome (OSAS) is known to be capable of provoking RBD-like symptoms by apneic event related arousals. The two sleep related pathologies must coincide in a relevant number of individuals because of overlapping prevalence in similar age groups. Until now RBD symptoms coexisting with OSAS are rarely described in scientific literature and in fact considered as OSAS mimicking RBD. MET

The relationship between sleep quality, daytime sleepiness, and rapid eye movement obstructive sleep apnea (REM-OSA).

PURPOSE: The prevalence of rapid eye movement obstructive sleep apnea (REM-OSA) varies among reports. It remains unclear whether or not patients with REM-OSA experience more severe daytime sleepiness and poorer sleep quality than those with sleep-stage-independent obstructive sleep apnea (IND-OSA). We investigated the prevalence of REM-OSA in a Korean population sample and determined whether or not REM-OSA was associated with poor sleep quality and daytime sleepiness. METHOD: In this retrospective study. we defined "REM-OSA 1" as an apnea-hypopnea index (AHI) ≥ 5 and AHIREM/AHINREM ratio ≥ 2.

Association of neurocognitive functioning with sleep stage dissociation and REM sleep instability in medicated patients with schizophrenia.

Many patients with schizophrenia present with impaired cognitive functioning and sleep disturbances. Dissociated stages of sleep represent instability within distinct sleep regulatory cerebral networks. Previous studies found increased rates of rapid eye movement (REM) sleep abnormalities in patients with schizophrenia and a positive association with psychopathology. In this study, we examined if sleep stage dissociation and REM sleep instability was associated with neurocognitive functioning in a sample of medicated patients with schizophrenia. The analyses were performed on 31 baseline polys

Validation of the Mayo Sleep Questionnaire to screen for REM sleep behavior disorder in a community-based sample.

OBJECTIVE: To validate a questionnaire focused on REM sleep behavior disorder (RBD) in a community-based sample. BACKGROUND: RBD is a parasomnia manifested by recurrent dream enactment behavior during REM sleep. While confirmation of RBD requires the presence of REM sleep without atonia on polysomnography (PSG), a screening measure for RBD validated in older adults would be desirable for clinical and research purposes. METHODS: We had previously developed the Mayo Sleep Questionnaire (MSQ) to screen for the presence of RBD and other sleep disorders. We assessed the validity of the MSQ by com

Validation of the Mayo Sleep Questionnaire to screen for REM sleep behavior disorder in an aging and dementia cohort.

OBJECTIVE: To validate a questionnaire focused on rapid eye movement sleep (REM) sleep behavior disorder (RBD) among participants in an aging and dementia cohort. BACKGROUND: RBD is a parasomnia that can develop in otherwise neurologically-normal adults as well as in those with a neurodegenerative disease. Confirmation of RBD requires polysomnography (PSG). A simple screening measure for RBD is desirable for clinical and research purposes. METHODS: We had previously developed the Mayo Sleep Questionnaire (MSQ), a 16 item measure, to screen for the presence of RBD and other sleep disorders. W

Rethinking the lexicon of REM sleep behavior disorder: a two-axis proposal.

REM sleep behavior disorder (RBD) is a parasomnia occurring during REM sleep, with different etiologies. The current international lexicon identifies a clinically oriented definition, which is 'clinically isolated RBD', when a clear causative disorder is not identified, and a pathophysiology-based definition, termed 'secondary RBD', when a known cause of RBD is evident. However, these two terms reflect different aspects of the disease and may be misleading and confusing, especially when defining disorders biologically, e.g., with biomarkers. This viewpoint proposes a new RBD lexicon based on e

Genetic associations between post-traumatic stress disorder and REM-sleep behavior disorder.

Isolated/idiopathic rapid-eye movement (REM) sleep behavior disorder (iRBD) is, in most cases, an early form of α -synuclein-related neurodegenerative diseases, including Parkinson's disease and dementia with Lewy bodies. Clinical reports suggest that iRBD is more common in individuals with Post-Traumatic Stress Disorder (PTSD) compared to those without PTSD. We conducted polygenic risk score (PRS), genetic correlation, and Mendelian randomization analyses to explore potential genetic and/or causal associations between PTSD and iRBD. Dopamine transporter imaging binding status was also examine

Altered network efficiency in isolated REM sleep behavior disorder: A multicentric study.

INTRODUCTION: Isolated rapid eye movement (REM) sleep behavior disorder (iRBD), characterized by abnormal movements during REM sleep, is a prodromal stage of dementia with Lewy bodies (DLB) and Parkinson's disease (PD). While iRBD shows emerging brain changes, their impact on structural connectivity and network efficiency, and their predictive value, remain poorly characterized. METHODS: In this international prospective study, 198 polysomnography-confirmed iRBD patients and 174 controls underwent diffusion magnetic resonance imaging and were analyzed.
