Childhood sleep disorders: practical management for the pediatrician.

PURPOSE OF REVIEW: Sleep disorders significantly impact pediatric cognitive development and behavioral health. As pediatric obesity and neurodevelopmental diagnoses rise, primary care providers require updated frameworks to differentiate between anatomical, behavioral, and circadian-driven sleep disturbances to ensure timely intervention. RECENT FINDINGS: Pediatric obstructive sleep apnea (OSA) is primarily linked to adenotonsillar hypertrophy, with the CHAT study highlighting that surgical intervention significantly improves quality of life and behavior. Behavioral insomnia remains prevalent

Characterizing Sleep in Adolescents and Adults with Autism Spectrum Disorders.

We studied 28 adolescents/young adults with autism spectrum disorders (ASD) and 13 age/sex matched individuals of typical development (TD). Structured sleep histories, validated questionnaires, actigraphy (4 weeks), and salivary cortisol and melatonin (4 days each) were collected. Compared to those with TD, adolescents/young adults with ASD had longer sleep latencies and more difficulty going to bed and falling asleep. Morning cortisol, evening cortisol, and the morning-evening difference in cortisol did not differ by diagnosis (ASD vs. TD). Dim light melatonin onsets (DLMOs) averaged across p

Loneliness mediates the association between sleep deficiency and fighting in school-aged adolescents: a 2-year longitudinal study.

Sleep deficiency is becoming increasingly common among adolescents and may lead to aggressive behaviour. This study investigated the association between sleep deficiency and fighting among 1946 school-aged adolescents (mean age: 12.2 ± 0.47 years) in Ningbo city through a two-year longitudinal study and explored the mediating roles of loneliness, sadness, and nervousness. A total of 552 (28.3%) reported sleep deficiency at baseline. During the follow-up period, 347 (17.8%) adolescents engaged in fighting. Sleep deficiency was significantly associated with an increased likelihood of fighting (O

Transoral robotic OSA surgery.

Continuous positive airway pressure (CPAP) is the primary treatment of obstructive sleep apnea/hypopnea syndrome (OSA). Most sleep physicians are in agreement that a certain number of OSA patients cannot or will not use CPAP. Although other conservative therapies such as oral appliance, sleep hygiene and sleep positioning may help some of these patients, there are many who fail all conservative treatments. As experts of upper airway diseases, we often view an airway clearly and help OSA patients understand the importance of assessment and treatment for OSA. Surgery for OSA is not a substitute

Necrotizing ulcerative gingivitis, periodontitis, and stomatitis: clinical staging and predisposing factors.

Necrotizing ulcerative gingivitis (NUG), necrotizing ulcerative periodontitis (NUP), and necrotizing stomatitis (NS), collectively termed necrotizing gingivostomatitis (NG), represent a dramatic, but rare oral infection associated with diminished systemic resistance, including HIV infection. Over a 5-year period, 68 consecutive NG patients from a population with known HIV status were evaluated and treated. Lesions were staged (modified Pindborg), and clinical findings and predictor variables were compared to 68 random control subjects without NG. Most cases (52%) were stage 1, with necrosis of

[Narcolepsy: etiology, clinical features, diagnosis and treatment].

Narcolepsy is a chronic hypersomnia characterized by excessive daytime sleepiness (EDS) and manifestations of disrupted rapid eye movement sleep stage (cataplexy, sleep paralysis, and hypnagogic/hypnopompic hallucinations). Mechanisms underlying narcolepsy are not fully understood. Experimental data indicate that the disease is caused by a loss of hypocretin neurons in the hypothalamus, likely due to an autoimmune process triggered by environmental factors in susceptible individuals. Most patients with narcolepsy and cataplexy have very low hypocretin-1 levels in the cerebrospinal fluid. An ap
