Drug-induced hypoglycemia. A review of 1418 cases.

The present review catalogues 1418 reported cases of drug-induced hypoglycemia. The main findings are that sulfonylureas (especially chlorpropamide and glyburide), either alone or with a second hypoglycemic or potentiating agent, still account for 63% of all cases; that alcohol, propranolol, and salicylate, either singly or with another hypoglycemic drug, are the next most frequent offenders (19% of the total); and that one older drug (quinine) and three new ones (pentamidine, ritodrine, and disopyramide) have caused an additional 7% of all episodes of severe hypoglycemia. The clinical factors

Drug-induced immunodeficiency associated with nodal Kaposi's sarcoma.

A 40-year-old male showed a syndrome of acquired immunodeficiency after a prolonged use of antidiabetic sulfamides. The lesions were revealed by the biopsic examination of inguinal lymph nodes. Immunodeficiencies are usually associated with malignant lymphoma: our case was, however, associated with Kaposi's sarcoma.

The response of plasma triglyceride, cholesterol, and lipoprotein lipase to treatment in non-insulin-dependent diabetic subjects without familial hypertriglyceridemia.

The effects of treatment on plasma total triglyceride, total cholesterol, and plasma postheparin lipase activities have not been evaluated in non-insulin-dependent diabetic (NIDD) subjects without a coexisting familial lipid disorder. In 49 untreated NIDD subjects, there was a linear relationship between glycosylated hemoglobin (GHb) and triglyceride (r = 0.35, P less than 0.02). This correlation was improved after adjusting for the effects of obesity by a partial correlation analysis. After therapy, there was a significant relationship between the change in GHb and the change in triglyceride.

Effects of sulfonylureas on membrane-bound low Km cyclic AMP phosphodiesterase in rat fat cells.

The effects of sulfonylureas and a biguanide on membrane-bound low Km cyclic AMP phosphodiesterase and lipolysis were examined in rat fat cells. Pharmacologically active sulfonylureas, such as tolbutamide (10 mM), acetohexamide (10 mM) and glibenclamide (200 microM) activated the phosphodiesterase when incubated with fat cells and suppressed lipolysis induced by isoproterenol. However, neither of these actions was observed in the presence of a pharmacologically inactive sulfonylurea, carboxytolbutamide (10 mM) and a biguanide, buformin (500 microM). Tolbutamide (0.5-10 mM) activated the enzyme

[Comparative evaluation of the hypoglycemic effect of the 2d-generation sulfonylurea preparations maninil and diamicron in diabetes mellitus].

Glycemia level and diurnal glucosuria were studied in the morning, afternoon and evening in a group of 27 patients with insulin-dependent diabetes mellitus before and after diamicron therapy for 1.5 to 3 months. A similar investigation was performed in a group of 20 patients, suffering from diabetes mellitus of the second type before and following maninil treatment during 1.5 to 2 months. It was shown that diamicron exerts a more pronounced sugar-lowering effect in comparison with that of maninil. It promotes stable glycemia development and should take an appropriate place among hypoglycemics

[Porphyria cutanea tarda and blood-sugar-suppressing derivatives of sulfonylurea].

A screening test for urinary porphyrin excretion was performed amound 1433 diabetes patients (552 males and 881 females). In three patients (0.21)% the diagnosis of porphyria cutanea tarda was ascertained. In spite of the increasing usage of sulfonylurea antidiabetic drugs, previously reported as potential porphyria-inducing agents, the authors could not observe an increased number of porphyria cases among the diabetics compared to the proportions in similar groups examined 10-12 years ago. The sulfonylurea Oradian was the likely etiopathogenic factor in only one out of three patients. The abo

PP2A activation overcomes leptomeningeal dissemination in group 3 medulloblastoma.

Leptomeningeal dissemination (LMD) is the primary cause of treatment failure in children with group 3 medulloblastoma (MB). Building on our previous work on protein phosphatase 2A (PP2A) activation in MB, here we present preclinical and molecular data on the effects of two novel classes of PP2A activators on disease processes of LMD in group 3 MB. The PP2A activators used in this study are ATUX-6156 and ATUX-6954 (diarylmethylcycloamine sulfonylureas), and ATUX-1215 and ATUX-5800 (diarylmethyl-4-aminotetrahydropyran-sulfonamides). Treatment with these compounds led to suppression of the endoge

Herbicides in landfill environments: multi-source characterization and priority risk pollutants in leachate and adjacent waters.

This study investigated the occurrence, distribution, and ecological risks of 29 herbicides in water bodies surrounding a landfill in North China. Analysis revealed that 24 herbicides were ubiquitously detected, though at trace concentrations (ng L-1 level). The herbicide compositions varied significantly among the leachate, groundwater, and surface water samples. Spatial distribution, together with principal component analysis and Pearson's correlation analysis, indicated diverse sources of herbicide contamination in the surrounding waters. However, the landfill is effective in preventing her

Acetohydroxyacid synthase: a target for antimicrobial drug discovery.

Acetohydroxyacid synthase (AHAS) (EC 2.2.1.6) (also known as acetolactate synthase) is the first common enzyme in the branched chain amino acid (BCAA) biosynthesis pathway. This pathway is present in microorganisms and in plants but not in animals, making it an attractive target for both drug and herbicide discovery. The function of AHAS is to catalyze the conversion of two molecules of pyruvate to 2-acetolactate or to convert one molecule of pyruvate and a molecule of 2-ketobutyrate into 2-aceto-2-hydroxybutyrate. Three cofactors are required for the activity of AHAS: thiamine diphosphate (Th

Phenotype, genotype and glycaemic variability in people with activating mutations in the ABCC8 gene: response to appropriate therapy.

AIMS: To examine the phenotypic features of people identified with ABCC8-maturity-onset diabetes of the young (MODY) who were included in the adult 'Mater MODY' cohort and to establish their response to sulfonylurea therapy. METHODS: Ten participants with activating ABCC8 mutations were phenotyped in detail. A 2-hour oral glucose tolerance test was performed to establish glycaemic tolerance, with glucose, insulin and C-peptide measurements taken at baseline and 30-min intervals. Insulin was discontinued and sulfonylurea therapy initiated after genetic diagnosis of ABCC8-MODY. A blinded contin

55P0110, a Novel Synthetic Compound Developed from a Plant Derived Backbone Structure, Shows Promising Anti-Hyperglycaemic Activity in Mice.

Starting off with a structure derived from the natural compound multiflorine, a derivatisation program aimed at the discovery and initial characterisation of novel compounds with antidiabetic potential. Design and discovery of the structures was guided by oral bioactivities obtained in oral glucose tolerance tests in mice. 55P0110, one among several new compounds with distinct anti-hyperglycaemic activity, was further examined to characterise its pharmacology and mode of action. Whereas a single oral dose of 55P0110 did not affect basal glycaemia, it markedly improved the glucose tolerance of

Single-Dose Pharmacokinetics of Grapiprant in Rhesus Macaques (Macaca mulatta).

Grapiprant belongs to the newer piprant class of nonsteroidal anti-inflammatory drugs (NSAIDs) and is FDA approved for treatment of osteoarthritis in dogs. The compounds act as an antagonist at the prostaglandin E2 EP4 receptor and is therefore also noncyclooxygenase inhibiting. This mechanism of action reduces the potential for adverse side effects associated with cyclooxygenase inhibition and makes it a promising drug for long-term use in nonhuman primates (NHPs) with chronic inflammatory conditions. In this study, we sought to establish pharmacokinetic parameters of an oral oil suspension o

Association between dipeptidyl peptidase-4 inhibitor use and risk of Parkinson's disease among patients with diabetes mellitus: a retrospective cohort study.

BACKGROUND: How a person's Parkinson disease (PD) risk is affected by dipeptidyl peptidase-4 (DPP-4) inhibitors remains unclear. We evaluated the association of PD risk with use of these inhibitors in individuals diagnosed as having diabetes mellitus (DM). METHODS: Individuals diagnosed as having new-onset DM were enrolled into the case group and comparison group, comprising patients who received a DPP-4 inhibitor and a sulfonylurea, respectively. These groups were matched through propensity score matching on the basis of income level, gender, urbanization level, enrollment year, age, and dia

Oral antidiabetes agents for the management of inpatient hyperglycaemia: so far, yet so close.

BACKGROUND: Hyperglycaemia is an ongoing challenge in hospital settings and is associated with poor outcomes. Current recommendations for the management of inpatient hyperglycaemia suggest insulin as the main glucose-lowering treatment choice and limit the administration of oral antidiabetes agents to a small proportion of cases because of safety concerns. AIM: To present and critically appraise the available evidence on the use of oral antidiabetes agents in the hospital setting and the risk-benefit balance of such an approach in the era of cardiovascular outcomes trials. METHODS: PubMed, E

Oral Hypoglycemic Agents Added to Insulin Monotherapy for Type 2 Diabetes.

CLINICAL QUESTION: Among patients with type 2 diabetes mellitus who do not achieve optimal glycemic control with insulin monotherapy, is the addition of oral hypoglycemic agents associated with benefits (measured by lowering of hemoglobin A1c) or adverse effects? BOTTOM LINE: Adding a sulfonylurea to insulin was associated with more hypoglycemic events compared with insulin alone, but this association was not observed for metformin. Adding a sulfonylurea or metformin to insulin was associated with a decrease in hemoglobin A1c of approximately 1.0%.
