Mycobacteria, survival, and universal stress proteins.

Universal stress proteins (USPs) have remained an enigma since their first description by Nystrom and Neidhardt in 1992. Despite being upregulated under diverse stresses and found across a range of bacterial species, decades of studies suggested only general and potentially redundant protective functions for USPs. Recent studies have uncovered that USPs are critical regulators of bacterial survival processes in Actinobacteria, most notably in Mycobacterium tuberculosis, one of the most prolific and lethal of human pathogens. This brief review places these recent studies in the context of earli

Intracellular Pseudomonas aeruginosa: internalization and survival.

Pseudomonas aeruginosa is an opportunistic human pathogen that can colonize multiple tissues and establish chronic infections with a high rate of relapse. P. aeruginosa survives intracellularly in multiple cell types, including in biopsies from human patients. Intracellular bacteria are partially protected from the immune system, from antibiotics with poor penetration across eukaryotic membranes, and, in some cases, also from antibiotics that penetrate eukaryotic membranes, such as fluoroquinolones. This suggests that P. aeruginosa can establish an intracellular reservoir that may contribute t

Factors Driving Pancreatic Cancer Survival Rates.

OBJECTIVES: To analyze trends driving the increase in the 5-year survival rate of pancreatic cancer over the last decade with a focus across disease stages. METHODS: Pancreatic cancer survival data was analyzed by disease stage, age, and race/ethnicity using the most recent data from the Surveillance, Epidemiology, and End Results (SEER) program. Stage distribution and survival rates were combined to evaluate the contribution of each stage to the overall survival rate. RESULTS: The most recent SEER 17 data registry reports an overall 5-year survival rate for pancreatic cancer of 12%, with ra

Medical comorbidities prognosticate survival in glioblastoma.

BACKGROUND: Medical comorbidities are prevalent in the glioblastoma population, although their effects on survival are not well established. METHODS: We retrospectively extracted known prognostic factors and medical comorbidities at the time of glioblastoma diagnosis for 867 patients who presented to Rhode Island Hospital from 2005-2022. RESULTS: All established prognostic factors were represented. Median age was 65 years (range <1-93). A higher number of comorbidities was associated with older age (p < 0.0001). Having >5 comorbidities doubled the hazard ratio compared to ≤1. While comorbidi

Prolonged allograft survival in liver transplantation.

INTRODUCTION: Donor age has traditionally been considered a factor associated with allograft failure in liver transplantation. We sought to examine the characteristics and outcomes of all liver allografts with a cumulative age of over 80 years within the US to better understand liver senescence. METHODS: Using the UNOS STARfile, allografts with a cumulative age (sum of age at transplant plus post-transplant survival) of octogenarian, 90-99 nonagenarian, and 100 years or greater (centurion) were identified from all adult transplant recipients between 1990 and 2022. Donor and recipient data as

Early inflammatory biomarkers and melanoma survival.

BACKGROUND: Few studies have investigated the role of inflammatory markers in predicting cutaneous melanoma survival. The aim of the study was to identify, if any, early inflammatory markers in the prognosis of all stages of primary cutaneous melanoma. METHODS: We conducted a 10-year cohort study among 2,141 melanoma patients from the same geographic area (Lazio) with primary cutaneous melanoma diagnosed between January 2005 and December 2013. In situ cutaneous melanoma was excluded from the analysis (N = 288), leaving 1,853 cases of invasive cutaneous melanoma. The following hematological ma

Saxagliptin promotes random skin flap survival.

BACKGROUND: Flap necrosis is a common issue encountered in clinical flap transplantation surgery. Here, we assessed the effects of saxagliptin, a dipeptidyl peptidase-4 inhibitor, on flap survival and explored the underlying mechanisms. METHODS: A dorsal McFarlane flap model was established in 36 rats, which were randomly divided into a high-dose saxagliptin (HS) group (saxagliptin, 30 mg/kg/day, n = 12), low-dose saxagliptin (LS) group (saxagliptin, 10 mg/kg/day, n = 12), and control group (n = 12). On day 7, flap survival was examined by eye in six rats from each group, along with determina

Survival Trends and Syndromic Esophageal Atresia.

BACKGROUND AND OBJECTIVES: Presence of a syndrome (or association) is predictive of poor survival in esophageal atresia (EA). However, most reports rely on historical patient outcomes, limiting their usefulness when estimating risk for neonates born today. We hypothesized improved syndromic EA survival due to advances in neonatal care. METHODS: A retrospective single-center review of survival in 626 consecutive patients with EA from 1980 to 2017 was performed. Data were collected for recognized risk factors: preterm delivery; birth weight <1500 g; major cardiac disease; vertebral defects, ana

Anal cancer survival: a socioeconomic analysis.

INTRODUCTION: Squamous cell carcinoma is the most common variant of anal malignancy. Certain disease-related factors have been established in determining survival. These include tumour size, differentiation and nodal involvement. Other factors such as HIV status, human papillomavirus infection, smoking and socioeconomic disparity may have important roles, however few data are available on the UK population. We aim to correlate social deprivation and survival of anal cancer patients at a tertiary centre. MATERIALS AND METHODS: All consecutive cases diagnosed with anal squamous cell carcinoma a

Survival trends in elderly myeloma patients.

OBJECTIVE: Myeloma is primarily a disease of the elderly, but older patients experience poorer outcomes. Effective treatments may not be offered to older patients over fears about toxicity, particularly for those with comorbidities. We aimed to characterise the trend in survival disparity between older and younger patients and assess to what extent comorbidity might explain these disparities. METHODS: We examined records for 56 010 patients diagnosed with myeloma in England between 1998-2014. The Hospital Episode Statistics database provided information on comorbidity. Net survival was estima

Marginal variable screening for survival endpoints.

When performing survival analysis in very high dimensions, it is often required to reduce the number of covariates using preliminary screening. During the last years, a large number of variable screening methods for the survival context have been developed. However, guidance is missing for choosing an appropriate method in practice. The aim of this work is to provide an overview of marginal variable screening methods for survival and develop recommendations for their use. For this purpose, a literature review is given, offering a comprehensive and structured introduction to the topic. In addit

Factors influencing dialysis arteriovenous graft survival.

PURPOSE: Dysfunction and loss of patency of dialysis arteriovenous grafts (AVGs) are serious causes of morbidity in patients on dialysis. Various risk factors associated with shorter AVG patency have been blamed, but the results of the studies were controversial. The aim of this study was to assess if associated diseases, biochemical markers and other parameters associated with atherosclerosis influence patency of AVGs in a large vascular access centre. METHODS: We conducted a retrospective study that included patients with AVGs patent for at least 3 weeks after creation. We included variable

Survival of Korean Huntington's Disease Patients.

OBJECTIVE: The survival of Huntington's disease (HD) patients is reported to be 15-20 years. However, most studies on the survival of HD have been conducted in patients without genetic confirmation with the possible inclusion of non-HD patients, and all studies have been conducted in Western countries. The survival of patients with HD in East Asia, where its prevalence is 10-50-fold lower compared with Western populations, has not yet been reported. METHODS: Forty-seven genetically confirmed Korean HD patients from independent families were included in this retrospective medical record review

Quantile association for bivariate survival data.

Bivariate survival data arise frequently in familial association studies of chronic disease onset, as well as in clinical trials and observational studies with multiple time to event endpoints. The association between two event times is often scientifically important. In this article, we examine the association via a novel quantile association measure, which describes the dynamic association as a function of the quantile levels. The quantile association measure is free of marginal distributions, allowing direct evaluation of the underlying association pattern at different locations of the even

Genotype impacts survival in Marfan syndrome.

AIMS: The aorta in Marfan syndrome (MFS) patients is variably affected. We investigated the assumed genotype-effect on protein production as a risk factor for a severe aortic phenotype in adult MFS patients. METHODS AND RESULTS: We collected clinical and genetic data from all 570 adults with MFS who had been included in the Dutch CONgenital CORvitia registry since the start in 2001. Mean age was 36.5 ± 13.5 years (51.2% male, 28.9% prior aortic surgery, 8.2% prior aortic dissection). Patients were prospectively followed for a mean duration of 8.2 ± 3.1 years. Men had more frequently aortic su

Changing pattern in malignant mesothelioma survival.

UNLABELLED: Survival for mesothelioma has been shown to be poor, with marginal improvement over time. Recent advances in the understanding of pathophysiology and treatment of mesothelioma may impact therapy to improve survival that may not be evident from available clinical trials that are often small and not randomized. Therapies may affect survival differently based on mesothelioma location (pleural vs peritoneal). Data are conflicting regarding the effect of asbestos exposure on mesothelioma location. OBJECTIVES: We examined survival in a large cohort of mesothelioma subjects analyzed by t
