Imaging of acute thoracic injury: the advent of MDCT screening.

Chest radiography remains the primary screening study for the assessment of victims of chest trauma, but computed tomography (CT), particula … CT has been well documented to offer major advantages over chest radiography in both screening for thoracic injuries and in characterizing such injuries. This capacity has been enhanced by the application of multichannel data acquisition. The greater sensitivity of MDCT has been well demonstrated in diagnosing vascular and diaphragmatic injuries. This article revi …

Risk factors for blunt thoracic aortic injury in children.

BACKGROUND/PURPOSE: Because blunt thoracic aortic injury is rare in children, a high index of suspicion is needed to identify this injury. The purpose of this study was to use a large national trauma database to define the risk factors for blunt thoracic aortic injury in children. METHODS: Using the National Trauma Database, the authors compared patient demographics, mechanism of injury, and associated injuries between children sustaining blunt trauma with and without a thoracic aortic injury. Factors independently associated with this injury were identified using multivariate methods. RESUL

Thoracoscopic management of thoracic duct injury: Is there a place for conservatism?

Thoracic duct injury is a rare but serious complication following chest surgeries and major neck dissections. Clinically, it can present as cervical chylous fistula, chylothorax or chylopericardium. Without treatment, the mortality is up to 50% and thus, early aggressive therapy is indicated. Traditional conservative management includes low-fat diet, parenteral nutrition, careful monitoring of fluid and electrolytes, and drainage of the neck wound or chylothorax. Patients with failed conservative management require definitive treatment in the form of ligation of the thoracic duct, which has tr

An isolated long thoracic nerve injury in a Navy Airman.

A palsy of the long thoracic nerve of Bell is a cause of scapular winging that has been reported after trauma, surgery, infection, electrocution, chiropractic manipulation, exposure to toxins, and various sports-related injuries that include tennis, hockey, bowling, soccer, gymnastics, and weight lifting. Scapular winging can result from repetitive or sudden external biomechanical forces that may either exert compression or place extraordinary traction in the distribution of the long thoracic nerve. We describe an active duty Navy Airman who developed scapular winging secondary to traction to

Centrifugal pump support for repair of thoracic aortic injury.

Spinal cord ischemia and the potential for paraplegia are always of concern during operations for repair of traumatic thoracic pseudoaneurysm. Patients who experience hypotension, loss of intercostal blood supply, or normothermic aortic cross clamp times greater than 30 minutes are more likely to become paraplegic. The preservation of renal perfusion is also a concern.

[Late diagnosis of a bronchial fracture after thoracic injury].

We report a case of bronchial fracture diagnosed in a child who had sustained a chest injury two and a half months earlier. Immediately after the injury, clinical and roentgenographic findings usually suggest the diagnosis, which is then confirmed by fiberoptic endoscopy. Because latent forms are frequent (10%), fiberoptic bronchoscopy should be widely used in patients having sustained a motor vehicle accident responsible for a significant chest injury. A follow-up visit two months after the injury ensures prompt detection of secondary complications.

Traumatic pneumoperitoneum following combined abdominal and thoracic injury.

Pneumoperitoneum following blunt abdominal trauma in the absence of other signs of severe intraabdominal injury is a rare finding. Although the vast majority of all cases of pneumoperitoneum are due to a ruptured intraabdominal hollow viscus, free abdominal air may result from significant barotrauma to the thorax. This type of secondary pneumoperitoneum can occur in the absence of chest x-ray evidence of a pneumothorax or pneumomediastinum. The complications associated with a missed visceral injury warrant an exploratory laparotomy, even if an extraabdominal source for the pneumoperitoneum is

Colonic dysfunction in patients with thoracic spinal cord injury.

Severe constipation is a debilitating concomitant of complete traumatic thoracic spinal cord injury. In order to investigate the pathophysiology of this symptom, we studied colonic compliance, as well as motor and myoelectrical activity, in the fasting and postprandial states and after neostigmine stimulation in 9 patients with clinically and electrophysiologically documented complete thoracic spinal cord injury. Electrophysiologic studies, including nerve conduction velocities, cortical and spinal somatosensory-evoked responses, and bulbocavernosus reflex responses, as well as urinary bladder

The fourth column in thoracic vertebral fractures: concomitant thoracic cage injuries and mortality in 27,814 inpatient cases from the german national DRG database.

PURPOSE: Population-level data on fourth column fractures and prognostic implications remain lacking. This study aimed to characterise the epidemiology of thoracic vertebral fractures in Germany, including the prevalence of fourth column injuries and geriatric patients, comorbidity burden and the in-hospital mortality across distinct fracture populations. METHODS: A Nationwide cross-sectional administrative inpatient data analysis of all adult thoracic vertebral fracture cases (ICD-10-GM S22.0x/S22.1) from the 2024 German Diagnosis Related Groups database was conducted across three groups: th

Early body representation EEG signals in cervical vs. thoracic spinal cord injuries with neuropathic pain.

Spinal cord injury (SCI) not only causes severe sensorimotor impairments but also leads to disruptions in body representation, including body schema. While the neurological differences between cervical and thoracic injuries are well established, the impact of the level of injury on body schema is less understood. Deeper insights into how change in body schema is affected by injury severity may further individual rehabilitation strategies and outcomes for individuals with SCI. This study explores event-related potentials (ERPs) between individuals with cervical and thoracic injuries in response…

Brief report on the development of hemorrhagic pericardial effusion after thoracic surgery for traumatic injuries.

INTRODUCTION: Hemorrhagic pericardial effusion (HPE) is a rare but life-threatening diagnosis that may occur after thoracic trauma. Previous reports have concentrated on delayed HPE in those who did not require initial surgical intervention for their traumatic injuries. In this report, we identify and characterize the phenomenon of HPE after emergent thoracic surgery for trauma. METHODS: This is a retrospective review of patients who required emergent thoracic surgery for trauma at a level 1 trauma center from 2017 to 2021. Using the institutional trauma database, demographics, injury charact

Damage control surgery for thoracic outlet vascular injuries: the new resuscitative median sternotomy plus REBOA.

Thoracic vascular trauma is associated with high mortality and is the second most common cause of death in patients with trauma following head injuries. Less than 25% of patients with a thoracic vascular injury arrive alive to the hospital and more than 50% die within the first 24 hours. Thoracic trauma with the involvement of the great vessels is a surgical challenge due to the complex and restricted anatomy of these structures and its association with adjacent organ damage. This article aims to delineate the experience obtained in the surgical management of thoracic vascular injuries via the

Lessons learned from the endovascular management of blunt thoracic aortic injuries: A single-centre experience.

BACKGROUND: Blunt thoracic aortic injuries (BTAIs) remain a leading cause of death after blunt trauma. In severe injuries, thoracic endovascular aortic repair (TEVAR) has provided a less invasive alternative to conventional open repair. OBJECTIVE: To report the TEVAR-related complications and uncertainties in patients who presented with traumatic pseudoaneurysms (grade III BTAI). METHODS: From April 2004 to February 2012, 55 patients (42 male, mean age 34.7 years) with severe BTAI were treated with stent grafts. Computed tomography (CT) was used to diagnose the injuries, and follow-up scans

Reducing chest injuries in automobile collisions: rib fracture timing and implications for thoracic injury criteria.

The purpose of this study was to quantify the biomechanical response of the human thorax during dynamic shoulder belt loading representative of that seen in a severe automotive collision. Two post-mortem human surrogates (PMHSs) (one male and one female) were instrumented with 26 single-axis strain gages on the ribs, sternum, and clavicle. The thorax of each PMHS was placed on a custom spine support bracket designed to support the thorax on either side of the spinous process, thereby allowing free motion at the costovertebral joints. In addition, the support bracket raised the thorax above the

Is distal aortic perfusion in traumatic thoracic aortic injuries necessary to avoid paraplegic postoperative outcomes?

OBJECTIVES: Traumatic thoracic aorta injuries account for nearly 8,000 deaths annually in the United States. Clamp-and-sew techniques can lead to high rates of paraplegia. Use of distal aortic perfusion can lead to heparin-related complications, particularly with associated head trauma. Our objective was to evaluate whether or not an individualized approach to operative management provides acceptable neurologic outcomes. METHODS: A retrospective review (1991-2004) of patients with a traumatic thoracic aortic injury at a Level I trauma center was performed. RESULTS: A total of 67 patients fit

Hyperextension injuries of the thoracic spine in diffuse idiopathic skeletal hyperostosis. Report of four cases.

Four patients who had multisegmental ankylosis of the thoracic and lumbar spine due to diffuse idiopathic skeletal hyperostosis sustained a hyperextension fracture-dislocation. The patients had a mean age of sixty-four years (range, fifty-eight to sixty-nine years); all four patients were men. All injuries occurred between the seventh and eleventh thoracic vertebrae. All patients had intact neurological function at the time of admission to the hospital. Three patients were managed with posterior spinal arthrodesis with Cotrel-Dubousset segmental instrumentation; one patient was managed non-ope

[Functional therapy in primary unstable injuries of the thoracic and lumbar spine. Indications and limits].

There are good results in the functional conservative treatment of the primary stable lesion of the thoracic and the lumbar part of the spine. Late results show that traumatic deformation can be compensated by the spine and by trained muscles. Segmental mobility can be generally neglected. Real late instabilities are very seldom. Static complaints caused by left deformation are important especially if the spine was previously damaged. Then, primary or early secondary operative reposition, replacement and segmental spondylodesis are indicated.
