Trapezial topography in thumb carpometacarpal arthritis.

Objective Contradictory reported arthritic patterns of the metacarpal surface of the trapezium include preferential volar wear, radial wear, and dorsal-ulnar sparing. We investigated whether a predominant wear pattern exists in surgical trapeziectomy for advanced thumb carpometacarpal (CMC) arthritis. Methods We examined 36 intact trapezia from 34 thumb CMC arthroplasty patients over an 18-month period. The first metacarpal articular surface revealed three consistent morphology patterns: (1) saddle, (2) dish, and (3) cirque. The saddle represented cartilage loss with preservation of the normal…

Thumb injuries in intercollegiate men's lacrosse.

BACKGROUND: Men's intercollegiate lacrosse is played at a fast pace and with significant force. Glove protection is required. However, the thumb is at risk because of contact with opponents' sticks, the ball, other players, and the ground or artificial surface. PURPOSE: To characterize patterns of hand injuries in men's intercollegiate lacrosse and to compare them with those in similar intercollegiate stick-handling sports that require gloves. STUDY DESIGN: Descriptive epidemiology study. METHODS: The National Collegiate Athletic Association (NCAA) Injury Surveillance System was utilized to…

Thumb infection caused by Streptococcus pseudoporcinus.

Streptococcus pseudoporcinus, a recently described organism found in the genitourinary tract of women, was isolated from a thumb wound in a male patient subsequent to trauma. This case describes a rarely reported non-genitourinary tract clinical isolate of S. pseudoporcinus.

Two different types of bowler's thumb.

Two rare cases of bowler's thumb (traumatic neuropathy of the ulnar digital nerve of the thumb) were treated surgically with encouraging results. These had different types of lesion, involving nodular neuroma and epineural mass, which were distinguished by magnetic resonance imaging. The surgical outcome for active bowlers is unclear and postoperative protection from the repetitive trauma of throwing is important. Possible technical solutions include changing the size and weight of the ball, the spacing between holes, the fitting of thumb to ball, and the slope and size of thumb holes. It is i…

Great toe-to-thumb microvascular transplantation.

Great toe to thumb transplantation offers the ideal reconstruction because it replaces the absent thumb with nearly identical anatomy. Functional donor site morbidity is minimal as compared with the aesthetic one; however, in comparison to the hand, the functional and aesthetic gain is tremendous. Anatomic and operative details are discussed along with preoperative and postoperative management. Postoperative functional evaluation of the thumb and donor site is assessed. Forty years of experience with this composite tissue transplantation has proven its status as the gold standard for thumb rec…

Percutaneous trigger thumb release in children.

The objective of this study was to determine whether percutaneous release of the A1 pulley is an adequate treatment of trigger thumb in children. Twenty-three children under the age of 6 years with 27 trigger thumbs fixed in flexion were evaluated prospectively. All were treated with a percutaneous release of the A1 pulley in an ambulatory setting and followed for at least 1 year. Interphalangeal and metacarpophalangeal joint range of motion, pinch strength, static two-point discrimination sensibility, and triggering were examined and compared with the unaffected thumb at the end of follow-up.…

Thumb polydactyly: clinical outcome after reconstruction.

PURPOSE: To evaluate clinical and cosmetic outcomes of reconstruction in thumb polydactyly and prognostic value of the Wassel classification. METHODS: Between 1993 and 2000 inclusive, out of the patients with thumb polydactyly (involving 80 thumbs) operated on, 34 patients (36 thumbs) were available for review and underwent clinical and radiological assessment. Outcomes in terms of the Tada score and complications were recorded. RESULTS: The mean age of patients at the time of operation was 2.8 (range, 0.6-47) years. The mean follow-up period was 5 (range, 2.4-10) years. According to the Was…

Thumb digital neuropathy caused by splinting.

Healthcare professionals are currently faced with a great variety of splints and splinting materials. Choices range from prefabricated produ … equently reported. We report a case in which a metal wrist cock-up splint caused compression of the thumb ulnar digital nerve. Preventive measures for such complication are included.

Little finger-to-thumb microvascular transfer.

This case report documents a unique thumb reconstruction performed at the authors' institutions. The amputated dominant right thumb of a manual laborer was electively reconstructed with microvascular transfer of the previously partially amputated little finger from the same hand. At 1 year postoperatively, the patient returned to work with excellent grip and pinch strength, thumb opposition to all digits, and 8 mm of static two-point discrimination. The technical details of the operation are described and compared with other analogous reports in the literature.

Gamekeeper's thumb. Ligament localisation by echography.

Twenty-one patients presenting clinically with a lesion of the ulnar collateral ligament complex of the metacarpo-phalangeal joint of the thumb underwent prospective diagnostic echography, in order to recognize the possible dislocation of the ligament over the adductor aponeurosis (Stener's lesion). Seven patients were treated surgically, the others conservatively. The accuracy of the echography was controlled by surgery and by the follow-up. We determined 15 true negatives, 4 true positives, 2 false positives, there was no false negative. Ultrasound imaging is a non-invasive, cost-effective d…

Transcarpal carpometacarpal dislocations, excluding the thumb.

During a 9-year period, we treated 13 patients with major fractures of bones of the distal carpal row creating instability of the corresponding carpometacarpal joints. Diagnosis of the injury was made early in only eight patients whose treatment consisted of open reduction, pin or screw fixation, and immobilization from 6 to 8 weeks. They showed no complications and had an excellent functional recovery. Diagnosis in five (38%) patients was delayed 1 week to 8 months after the accident. At initial presentation, symptoms included pain, loss of grip strength, and mild limitation of finger motion.

Thumb avulsion: results of replantation/revascularization.

During a 9-year period, 39 thumb avulsions required emergency replantation/revascularization at our institution. Amputation was complete in 28 (72%) cases and involved the dominant thumb in 31 (80%). Farm machinery was the source of injury in 33 (85%) cases. Follow-up ranged from 7 months to 6 years. Only 10 of the revascularization attempts (26%) were successful; the remaining attempts required amputation at the level of the injury. In two (18%) of the 11 partial amputations and in eight (29%) of the 28 complete amputations, revascularization was successful. In eight (31%) of the 26 thumbs in…

Trigger thumb of de Quervain's disease.

Triggering of the thumb due to de Quervain's Disease has not been reported in an adult, and only one case in a child has been documented in the literature. Two cases of trigger thumb that were due to stenosing tenosynovitis in which the extensor pollicis brevis tendon triggered in a separate fibro-osseous tunnel within the first dorsal compartment are cited.

Thumbs-out classification: a novel tool for detection of thumb-in- palm deformity in children and youth with cerebral palsy.

PURPOSE: To develop a simple screening tool, the Thumbs-Out Classification (TOC), for detecting thumb-in-palm deformity during clinical follow-ups of children with cerebral palsy (CP), and to evaluate its reliability. METHODS: The TOC was developed through expert consensus and consists of three levels of radial thumb abduction: normal (green), slightly reduced (yellow), and severely reduced (red). Classification cut-offs were established using goniometric measurements from 20 typically developing children. These cut-offs were then applied to classify active and passive radial thumb abduction …
