Pulmonary venous flow from fetal to neonatal period.

There are few reports about the effect of fetal or transitional circulation on the pulmonary venous flow. The purposes of this study were to investigate flow patterns of the pulmonary vein serially from fetal to neonatal period and to determine the relationship between pulmonary venous flow and other parameters from aortic and mitral valve. Pulmonary venous flow velocity was analyzed in 21 normal term human fetuses. Postnatal follow-up studies were performed at 1, 6, 24 h, 3 days, 1 week and 1 month. In each time point, pulsed Doppler echocardiography was used to interrogate right upper pulmon

A unique stenosis in saphenous vein graft visualized by optical coherence tomography.

We present a case of a unique stenosis in a 12-year-old saphenous vein graft (SVG), to the right coronary artery, which was visualized by optical coherence tomography (OCT), before percutaneous coronary intervention. The patient was an 80-year-old man in whom the stenosis was documented by area-detector coronary computed tomography. OCT imaging demonstrated that the culprit lesion was a venous valve containing a thrombus before preintervention imaging. Coronary stenting was performed with a distal protection device, and pathologic examination of the aspirate verified the OCT findings. Coronary

Managing leg ulceration in intravenous drug users.

Chronic venous leg ulceration is a long-term condition commonly associated with lower-limb injecting and chronic venous hypertension caused by collapsed veins, incompetent valves, deep vein thrombosis and reflux. It is not usually a medical emergency, but intravenous (IV) drug users with leg ulcers can attend emergency departments (EDs) with a different primary complaint such as pain or because they cannot access local primary care or voluntary services. Leg ulceration might then be identified during history taking, so it is important that ED nurses know how to assess and manage these wounds.

Effects of mitral stenosis on pulmonary venous flow as measured by Doppler transesophageal echocardiography.

Pulmonary venous flow as assessed by transesophageal echocardiography (TEE) is influenced by changes in left atrial pressure and function. In mitral stenosis (MS), normal left atrial hemodynamics are altered because there is a prolonged decay in diastolic pressure from the left atrium to the left ventricle and atrial function may be altered because of atrial fibrillation. To assess the effect of the prolonged atrial diastolic pressure decay caused by MS on pulmonary venous flow, we studied 27 patients with MS (mitral valve range 0.7 to 2.4 cm2) by pulsed-wave Doppler TEE of the left or right u

Stripping the long saphenous vein: peroperative retrograde saphenography in patients with and without venous ulceration.

Peroperative retrograde saphenography was used to study 80 incompetent long saphenous veins (LSV) in 60 patients with primary varicose veins. In 53 limbs presentation was uncomplicated (group A, 40 patients); in 27 limbs there was ulceration (group B, 20 patients). All limbs in group B had a normal deep system as shown pre-operatively by ascending and descending venography. Varicose changes were found in 52 (65 per cent) of 80 LSV; the overall mean length of normal vein seen was 16 cm. The incompetent LSV appears unlikely to be suitable for arterial replacement. All valves identified on operat

Interventional Treatment of Severe Tricuspid Regurgitation: Early Clinical Experience in a Multicenter, Observational, First-in-Man Study.

BACKGROUND: Transcatheter caval valve implantation is under evaluation as a treatment option for inoperable patients with severe tricuspid regurgitation (TR). The procedure involves the catheter-based implantation of bioprosthetic valves in the inferior vena cava and superior vena cava to treat symptoms associated with TR. This study is the first to evaluate the feasibility, safety, and efficacy of this interventional concept. METHODS AND RESULTS: Twenty-five patients (mean age, 73.9±7.6 years; women, 52.0%) with severe symptomatic TR despite optimal medical treatment deemed unsuitable for su

Hemodynamic assessment of venous problems.

Objective assessment of venous function based on venous refilling time recorded by photoplethysmography (PPG) was done in 89 patients with postphlebitic syndrome (47), primary varicose veins (29), and after an episode of acute deep vein thrombosis (13). Limbs with symptomatic postphlebitic changes had a shortened venous refilling time of 15.6 +/- 11 seconds (normal greater than 20 seconds). When venographic findings (ascending and descending) in 47 patients with postphlebitic syndrome were analyzed, it was noted that femoral valve incompetence demonstrated by descending venography was a common

[Normal and pathological anatomy and physiology of the veins of the lower limbs].

Veins of the lower limbs are divided into two regions: 1) the superficial region, composed of two saphenous veins, internal and external, and of their tributaries; 2) the deep region, composed of veins that are satellites (and homonyms) of the arteries (anterior and posterior tibial; peroneal; popliteal; superficial, deep and common femoral; external, epigastric and common iliac). The two networks are linked by perforating (or communicating) veins and by the arches of the two saphenous veins. Valvules are found in the venous network below the inguinal ligament; their number decreases with incr

Thrombectomy for acute deep vein thrombosis: prevention of postthrombotic syndrome.

Ninety-six limbs of 89 patients with acute deep vein thrombosis of the lower extremity were followed for 1 to 18 years. In the thrombectomy … ty, preserve venous valve function, and promote intrapelvic collateral circulation. Preservation of venous valves in the femoropopliteal region is a key to the prevention of postthrombotic syndrome. Thrombectomy within 5 days of the onset of symptoms is recommended for patients with iliofemoropopliteal venous thrombosis.

Vascular morphology of the bovine spermatic cord and testis. I. Light- and scanning electron-microscopic studies on the testicular artery and pampiniform plexus.

This highly coiled testicular artery within the bovine spermatic cord has a constant luminal diameter but a continuously decreasing mural thickness. The pampiniform plexus is composed of three interconnected venous networks differing in mesh sizes and calibres. The large veins of the first network display pouches and permanent constrictions, which may serve as throttle devices. The constitutents of the third network are venules or venous capillaries with diameters between 10 and 20 micron; they favor a periarterial position or even occupy the media-adventitia border of the testicular artery. Near Infrared Fluorescence Imaging to Demonstrate Reflux in the Superficial Microvenous Network of the Leg.

OBJECTIVE: Reflux within the superficial microvenous network may play a critical role in the development of skin changes which can be associated with chronic venous insufficiency. This study aimed to determine if near infrared fluorescence (NIRF) imaging could be used to accurately determine superficial venous reflux in the leg. METHODS: A total of nine limbs were examined ex vivo from patients undergoing limb amputation for peripheral arterial disease. Cannulation of the distal great saphenous vein was used to sequentially perform Xray contrast enhanced venography, NIRF imaging, and venous c

Current concepts in anticoagulant therapy.

An understanding of the international normalized ratio (INR)--which was developed to standardize reporting of the prothrombin time (PT) and provide consistent regulation of anticoagulation--is important. The recommended therapeutic range for the INR (which is calculated from the patient's PT, a mean control PT, and the international sensitivity index) for oral anticoagulant treatment of most conditions is 2.0 to 3.0. In patients with mechanical cardiac valves, the INR should be at least 2.5 to 3.5. A common cause for progression of venous thromboembolic disease and treatment failure is inadequ

The substitute "valve" operation by technique II in patients with postthrombotic syndrome.

The substitute "valve" operation with use of a silicone tendon designed to produce a valvelike mechanism in the popliteal veins of 29 patients with 31 postphlebitic limbs caused by reflux in the deep veins is described. The results have been assessed by measurements, taken during ambulation, of venous pressure before operation, 1 week after operation, and 1 year after operation and by phlebography. In 29 limbs there was clinical improvement associated with improvement in the venous pressure assessed during ambulation and phlebographic evidence of a functioning substitute valve.

MR phase-shift velocity mapping of mitral and pulmonary venous flow.

Mitral and pulmonary venous flows are important indexes in the evaluation of left ventricular diastolic function and in the assessment of mitral valve disease. We used MR phase-shift velocity mapping to measure mitral and pulmonary venous flow velocity in 10 healthy volunteers and mitral flow velocity in 5 patients with mitral valve stenosis. Normal mitral flow shows two positive peaks: one during early ventricular diastole and the other during atrial contraction. Peak mitral flow velocity (mean +/- SD) in early diastole was 68 +/- 12 cm/s and during atrial contraction 39 +/- 10 cm/s. The rati

Management of anticoagulation in patients with metastatic castration-resistant prostate cancer receiving abiraterone + prednisone.

PURPOSE: Abiraterone has been proven to be an effective agent used in the management of metastatic castration-resistant prostate cancer, significantly improving overall and progression-free survival. Due to the pharmacodynamic and pharmacokinetic properties of abiraterone, concurrent use with anticoagulation may pose a challenge for clinicians. Thrombosis within the cancer setting continues to increase patient mortality; therefore, appropriate anticoagulation through the use of a management algorithm can reduce adverse events and increase quality of life. METHODS: A review of the literature w

Does retrograde cerebral perfusion via superior vena cava cannulation protect the brain?

OBJECTIVE: The retrograde cerebral perfusion via cannulation of the superior vena cava is a widespread method for optimising protection of the brain during hypothermic circulatory arrest. METHODS: In 14 cadavers (8 females, 6 males) of the local department of pathology, an examination was performed to check the competence of the valves of the internal jugular veins. After a complete preparation of the superior vena cava, the innominate vein and both internal jugular veins, ligating all side branches, a retrograde perfusion on 7 cadavers was installed, documenting flow and pressure of each int

Percutaneous Interventricular Septal Access Guided by Subcostal Echocardiography and Fluoroscopy for Ventricular Tachycardia Ablation in a Patient with Aortic and Mitral Mechanical Valves.

Mechanical prosthetic aortic and mitral valves preclude either a retrograde aortic or transseptal approach to the left ventricular (LV) endocardium. Several operators have reported on the application of nonconventional techniques for ventricular tachycardia (VT) ablation including transventricular septal puncture, epicardial approach, transmechanical valve approach, transcoronary venous approach, and transapical approach. Incorporating transventricular access to the LV under intracardiac echocardiography (ICE) guidance has been previously attempted in VT ablation procedures in patients with bo
