Intravitreal ocriplasmin: A breakthrough in the treatment of vitreomacular traction?

Vitreoretinal interface pathologies, such as vitreomacular traction syndrome, epiretinal membranes and macular holes are sight-threatening conditions and one of the important causes of vision defects and vision loss. To this date, vigilance with observation of how the vitreomacular traction resolves, or vitreoretinal surgery in more severe cases, were the only treatment options. Recent rapid progress in ophthalmology, especially in diagnostic and visualization techniques, provided better insight into the mechanisms taking place on the vitreoretinal surface, which enabled a more accurate select

[Glaucoma and retinal surgery].

In the therapeutic approach to complex glaucomas different initial situations were considered: pre-existing glaucoma, induction of glaucoma after vitreoretinal surgery and antiglaucomatous procedures. In pre-existing glaucoma and after filtering surgery maintenance of the filtering bleb requires a vitreoretinal approach for conjunctiva preservation with techniques such as pneumatic retinopexy or small gauge vitrectomy. After vitreoretinal surgery an increase in intraocular pressure (IOP) is common. Secondary glaucoma may occur after scleral buckling and after vitrectomy with or without gas or

Retinal Damage Induced by Internal Limiting Membrane Removal.

The internal limiting membrane (ILM), the basement membrane of the Müller cells, serves as the interface between the vitreous body and the retinal nerve fiber layer. It has a fundamental role in the development, structure, and function of the retina, although it also is a pathologic component in the various vitreoretinal disorders, most notably in macular holes. It was not until understanding of the evolution of idiopathic macular holes and the advent of idiopathic macular hole surgery that the idea of adjuvant ILM peeling in the treatment of tractional maculopathies was explored. Today intent

Aberrant angiogenesis, inflammation, and fibrosis in pediatric vitreoretinal surgical diseases.

Pediatric rhegmatogenous retinal detachment (RRD) is usually caused by trauma or myopia, but can also be a comorbidity of genetic or congenital diseases or a complication after previous eye surgery. Pediatric RRD is challenging due to late diagnosis, distinct ocular anatomy compared to adults, and a higher prevalence of proliferative vitreoretinopathy (PVR). Late diagnosis due to limited symptom awareness, especially in younger children, ultimately leads to poor surgical outcomes, causing chronic detachment and PVR formation. Pediatric patients are susceptible to more severe PVR due to stronge

Endoscopic evaluation and dissection of the anterior vitreous base.

The specific concept of pathological anterior vitreous base (AVB) and of its endoscopy-assisted dissection was elaborated during our 20 years' experience with endoscopy for observing the 'in vivo' anatomy and pathoanatomy of the AVB, and for dissecting the anterior vitreous cortex in over 2,000 consecutive eyes. Endoscopy provides a 360° view of the entire vitreous cavity akin to the surgeon's eye being inside the operated eye. Evaluation of the capsulozonular complex, ciliary body, and AVB is not only independent of anterior media transparency, but unimpeded by scleral depression and magnifie

[Extra-scleral balloon catheterization. Report 3. New ways of its use--prevention of hypotonia and intraocular hemorrhages in vitreo- retinal surgery].

Hypotonia and ex vacuo hemorrhages are the most grave complications of vitreoretinal surgery; the known methods of their prevention are altogether inadequate. The authors have developed extra-scleral ballooning (ESB) methods that improve the efficacy of surgery and reduce the rate and severity of complications in subretinal fluid draining during surgery for detachment of the retina and in arresting intraocular hemorrhages during closed vitrectomy. ESB application helped elevate intraocular pressure in the patients with drastic hypotonia, permitting a reliable and easy-to-perform monitoring of

Pars plana vitrectomy for vitreoretinal interface disorders coincident with intermediate stage age-related macular degeneration.

BACKGROUND AND AIMS: Currently around 67 million people in Europe are affected by some form of age-related macular degeneration (AMD). As most known types of vitreoretinal (VR) interface disorders can coexist with AMD and as we can favourably affect the former with vitreoretinal surgery, our goal was to evaluate the results of vitreoretinal interface disorder surgery with macular peeling in relation to coincident intermediate stage AMD. METHODOLOGY: This was a retrospective evaluation of eyes operated with 25-gauge pars plana vitrectomy (PPV). The monitored parameters were anatomical and func

Indications and techniques of endoscope assisted vitrectomy.

The popularization of ophthalmic endoscopy has been promoted by recent technological advancements that increase the number of indications for endoscopy. These advancements have improved the endoscope's capabilities in its two fundamental surgical advantages: (1) bypassing anterior segment opacities, and (2) visualizing anteriorly positioned structures such as the ciliary bodies and sub-iris space. In this article, the current state of the ophthalmic endoscope is reviewed alongside its growing number of applications in glaucoma, vitreoretinal, and ocular trauma surgery. We describe the role of

Artificial vitreous replacements.

Vitreoretinal pathologies remain the significant leading cause of blindness, after cataract, throughout the world. Biomaterials as a whole, and vitreous substitutes in particular, have had a major role to play in vitreoretinal surgery. History has witnessed the advent of vitreous substitutes from collagen and hyaluronic acid to gases, silicone oils and perfluorocarbon liquids and now to polymer hydrogels. Such a long list only underscores the fact that we have been found wanting in our quest for an ideal vitreous substitute. This review focuses on the merits and demerits of various vitreous su

Force-based Control for Safe Robot-assisted Retinal Interventions: In Vivo Evaluation in Animal Studies.

In recent years, robotic assistance in vitreoretinal surgery has moved from a benchtop environment to the operating rooms. Emerging robotic systems improve tool manoeuvrability and provide precise tool motions in a constrained intraocular environment and reduce/remove hand tremor. However, often due to their stiff and bulky mechanical structure, they diminish the perception of tool-to-sclera (scleral) forces, on which the surgeon relies, for eyeball manipulation. In this paper we measure these scleral forces and actively control the robot to keep them under a predefined threshold. Scleral forc

Ocular surface changes following vitreoretinal procedures.

Dry eye disease (DED) can lead to ocular discomfort as well as visual disturbances. Older adults are more likely to develop DED. They are also more likely to develop retinal diseases such as diabetic retinopathy and age-related macular degeneration, which may require vitreoretinal surgeries, lasers, and intravitreal injections. Posterior segment surgery may cause or worsen existing dry eye symptoms, albeit temporarily. Despite good anatomic and functional outcomes, ocular surface dysfunction can have a significant impact on the quality of life, lowering a patient's satisfaction with the retina
