Safety and toxicity of Iopofosine I 131 (CLR 131) with external beam radiation therapy in recurrent or metastatic head and neck cancer: results of a phase 1 single-centre, open-label, single-arm, dose escalation and dose expansion study.

BACKGROUND: Re-irradiation of recurrent head and neck cancer (HNC) is often limited by tumour adherence to critical structures and/or radiation tolerance of critical normal tissues. Iopofosine I 131 (CLR 131) is a targeted small molecular phospholipid ether (PLE) drug conjugate that delivers iodine-131 selectively to tumour cells. We conducted a phase 1, single-centre, open-label study to determine whether CLR 131 given with reduced dose of external beam radiation therapy (EBRT) would be tolerable and feasible. METHODS: All participants received previous curative intent treatment with radioth

Phase- and gender-specific, lifetime, and future costs of cancer: A retrospective population-based registry study.

AbstractValid estimates of cancer treatment costs are import for priority setting, but few studies have examined costs of multiple cancers in the same setting. We performed a retrospective population-based registry study to evaluate phase-specific (initial, continuing, and terminal phase) direct medical costs and lifetime costs for 13 cancers and all cancers combined in Norway. Mean monthly cancer attributable costs were estimated using nationwide activity data from all Norwegian hospitals. Mean lifetime costs were estimated by combining phase-specific monthly costs and survival times from the

Integration of Chronic Oncology Services in Noncommunicable Disease Clinic in Rural Rwanda.

BackgroundIn rural sub-Saharan Africa, access to care for severe non-communicable diseases (NCDs) is limited due to myriad delivery challenges. We describe the implementation, patient characteristics, and retention rate of an integrated NCD clinic inclusive of cancer services at a district hospital in rural Rwanda.MethodsIn 2006, the Rwandan Ministry of Health at Rwinkwavu District Hospital (RDH) and Partners In Health established an integrated NCD clinic focused on nurse-led care of severe NCDs, within a single delivery platform. Implementation modifications were made in 2011 to include cance

Hepatocellular Carcinoma associated with Extra-hepatic Primary Malignancy: its Secular change, Clinical Manifestations and Survival.

Clinical manifestations between hepatocellular carcinoma (HCC) and extra-hepatic primary malignancy (EHPM) are lack of large-scale study. We enrolled 14555 HCC patients between 1986 and 2013 retrospectively. The EHPM was classified as prior, synchronous and metachronous group based on before, within and after 6 months of HCC diagnosis, respectively. The incidence rate of EHPM is 3.91% (95% confidence interval [CI]: 3.60-4.23%). Urogenital cancers, kidney and bladder, were at unexpected higher ranks. Older in age, Child-Pugh A cirrhosis, negativity of HBsAg and anti-HCV, and earlier BCLC stagin

The role of obesity and bariatric surgery-induced weight loss in breast cancer.

Obesity is a complex metabolic condition considered a worldwide public health crisis, and a deeper mechanistic understanding of obesity-associated diseases is urgently needed. Obesity comorbidities include many associated cancers and are estimated to account for 20% of female cancer deaths in the USA. Breast cancer, in particular, is associated with obesity and is the focus of this review. The exact causal links between obesity and breast cancer remain unclear. Still, interactions have emerged between body mass index, tumor molecular subtype, genetic background, and environmental factors that

Medical history predicts phenome-wide disease onset and enables the rapid response to emerging health threats.

The COVID-19 pandemic exposed a global deficiency of systematic, data-driven guidance to identify high-risk individuals. Here, we illustrate the utility of routinely recorded medical history to predict the risk for 1741 diseases across clinical specialties and support the rapid response to emerging health threats such as COVID-19. We developed a neural network to learn from health records of 502,489 UK Biobank participants. Importantly, we observed discriminative improvements over basic demographic predictors for 1546 (88.8%) endpoints. After transferring the unmodified risk models to the All

Forgoing life-sustaining treatment - a comparative analysis of regulations in Japan, Korea, Taiwan, and England.

BackgroundRegulations on forgoing life-sustaining treatment (LST) have developed in Asian countries including Japan, Korea and Taiwan. However, other countries are relatively unaware of these due to the language barrier. This article aims to describe and compare the relevant regulatory frameworks, using the (more familiar) situation in England as a point of reference. We undertook literature reviews to ascertain the legal and regulatory positions on forgoing LST in Japan, Korea, Taiwan, and England.Main textFindings from a literature review are first presented to describe the development of th

A systematic review of patient and healthcare professional perceptions of the barriers and facilitators to embedding exercise in the adjuvant cancer treatment pathway.

PurposeEvidence suggests that physical activity and exercise interventions can mitigate cancer treatment side effects. However, an improved understanding of how physical activity/exercise can be embedded within cancer care pathways is needed. We examined what is known about the barriers and facilitators to implementing physical activity/exercise interventions in the adjuvant cancer treatment pathway from a patient and healthcare professional perspective. The protocol was registered a priori with PROSPERO in March 2023.MethodsElectronic databases (CINAHL Plus, MEDLINE, PsycINFO and Cochrane) we

Prognostic Factors and Risk Stratification in Malignant Tumor Patients With Sepsis: A Retrospective Cohort Study.

BACKGROUND Sepsis is a life-threatening complication in cancer patients and a major contributor to ICU admission and mortality. Although tumor-related factors affect outcomes, the prognostic value of ICU-specific clinical parameters remains insufficiently explored. This study aimed to identify ICU-related predictors of mortality beyond tumor characteristics in cancer patients with sepsis. MATERIAL AND METHODS We conducted a single-center retrospective cohort study including 4301 cancer patients with sepsis. Independent predictors of 28-day mortality were identified using multivariable logistic

Development and implementation of the International AIDA Network Castleman's disease registry.

Castleman's disease (CD) consists of a wide spectrum of rare disorders classified into unicentric CD and multicentric CD (MCD), based on the diffusion of disease distribution and the severity of clinical manifestations. While unicentric CD is characterized by a single lymph node involvement, MCD is defined by multiple lymph node station involvement with more prominent systemic symptoms. MCD is further subdivided into HHV-8 associated MCD, polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, skin changes (POEMS)-associated MCD, and idiopathic MCD (iMCD), which is also

A Comprehensive Review of the Role of the Latest Minimally Invasive Neurosurgery Techniques and Outcomes for Brain and Spinal Surgeries.

Minimally invasive neurosurgical techniques offer reduced surgical trauma, lower complication rates, and faster recovery compared to traditional open procedures. Approaches such as keyhole craniotomy, endoscopic skull base surgery, endovascular neurosurgery, and percutaneous spinal techniques have advanced the management of cranial and spinal pathologies. These methods aim to minimize tissue disruption, blood loss, and hospital stay while maintaining surgical efficacy. This narrative review explores their evolution, applications, and advantages in modern neurosurgery.

Innovation in Oncology Drug Development.

Significant progress has been made in our understanding of the molecular lesions responsible for tumor cells to exhibit uncontrolled growth while circumventing normal mechanisms of apoptosis and their ability to migrate and invade normal tissues while evading recognition and destruction by the immune system. This understanding has enabled the development of therapies specifically targeted to these lesions coupled to innovative treatment regimens to most effectively use these new targeted therapies with precision in selected subpopulations of patients. Innovation at the scientific and clinical

Transferability of a US claims-based machine learning model for ATTRwt-CM identification: a retrospective evaluation in a German setting.

Cardiac amyloidosis (CA), a fatal and progressive cardiomyopathy is characterized by amyloid deposition within the myocardium. The main forms of CA include transthyretin amyloidosis (ATTR-CM; distinguished into a hereditary and wildtype form [ATTRv and ATTRwt]) and light-chain amyloidosis (AL). Transthyretin amyloidosis with cardiomyopathy (ATTR-CM) is increasingly recognized among heart failure (HF) patients, despite its underestimated prevalence as a rare disease. ATTR-CM is challenging to diagnose due to its broad phenotypic overlap with HF, yet a timely differentiation of CA is crucial, as
