Timing of Antibiotic Prophylaxis in Elective Caesarean Delivery: A Multi-Center Randomized Controlled Trial and Meta-Analysis.

OBJECTIVE: To compare the effectiveness of antibiotic prophylaxis before skin incision with that after umbilical cord clamping in elective caesarean delivery. METHODS: We conducted a randomized open-label controlled trial with two parallel arms at three hospitals in western China. Participants meeting the inclusion criteria received antibiotics 30-60 minutes before skin incision while others received antibiotics after umbilical cords clamping. For the meta-analysis, studies were identified from the database of PUBMED, Cochrane Library and EMbase and assessed using the Cochrane risk of bias to

Laparoscopic excision versus open excision for the treatment of choledochal cysts: a systematic review and meta-analysis.

In 1723, Vater first described choledochal cyst and in 1977, Todani et al classified this disease. For many years, open excision (OP) as the standard procedure made a great impact in the treatment of choledochal cyst. Since 1995, when Farello et al first reported laparoscopic choledochal cyst excision, laparoscopic excision (LA) has been used worldwide. However, its safety remains a major concern. The aim of this meta-analysis was to compare OP with LA in treating choledochal cyst and then to determine whether LA is safe and valid. The design of this study involved systematic review and meta-a

Social, economic, and health impact of the respiratory syncytial virus: a systematic search.

BACKGROUND: Bronchiolitis caused by the respiratory syncytial virus (RSV) and its related complications are common in infants born prematurely, with severe congenital heart disease, or bronchopulmonary dysplasia, as well as in immunosuppressed infants. There is a rich literature on the different aspects of RSV infection with a focus, for the most part, on specific risk populations. However, there is a need for a systematic global analysis of the impact of RSV infection in terms of use of resources and health impact on both children and adults. With this aim, we performed a systematic search of

WITHDRAWN: Patient controlled intravenous opioid analgesia versus continuous epidural analgesia for pain after intra-abdominal surgery.

BACKGROUND: There are two common techniques for postoperative pain control after intra-abdominal surgery: patient-controlled analgesia (PCA) with intravenous opioids and continuous epidural analgesia (CEA). It is uncertain which method has better pain control and fewer adverse effects. OBJECTIVES: The objective of this review was to compare PCA opioid therapy with CEA for pain control after intra-abdominal surgery in terms of analgesic efficacy, side effects, patient satisfaction and surgical outcome by meta-analysis of the relevant trials. SEARCH METHODS: We searched CENTRAL (The Cochrane L

Wound drains after incisional hernia repair.

BACKGROUND: Incisional hernias are caused by the failure of the wall of the abdomen to close after abdominal surgery, leaving a hole through which the viscera protrude. Incisional hernias are repaired by further surgery. Surgical drains are frequently inserted during hernia repair with the aim of facilitating fluid drainage and preventing complications. Traditional teaching has recommended the use of drains after incisional hernia repair other than for laparoscopic ventral hernia repair. More than 50% of open mesh repairs of ventral hernias have drains inserted. However, there is uncertainty a

SGLT2 inhibitor therapy following aortic valve intervention for aortic stenosis: a systematic review and meta-analysis of mortality, heart failure, rhythm, renal, and valve-related outcomes.

BACKGROUND: Residual mortality and heart-failure events remain common after aortic valve intervention for severe aortic stenosis. Sodium-glucose cotransporter-2 inhibitors (SGLT2i) improve cardiovascular and renal outcomes in heart-failure, diabetes, and chronic kidney disease populations, but their role following transcatheter or surgical aortic valve intervention remains uncertain. METHODS: PubMed, Scopus, ScienceDirect, Cochrane Library, citation tracking, and manually identified eligible studies were screened through June 2026 for randomized and observational studies comparing SGLT2i with

Evidence mapping of Zhenwu decoction for chronic heart failure.

OBJECTIVE: Chronic heart failure (CHF) remains associated with symptom burden, recurrent hospitalization, and reduced quality of life despite guideline-directed therapy. Zhenwu Decoction (ZWD), a classical traditional Chinese medicine formula, is used as adjunctive therapy for yang-deficiency and fluid-retention patterns. This evidence map aimed to characterize the volume, distribution, methodological quality, and evidence gaps in clinical research on ZWD for CHF. METHODS: We searched CNKI, VIP, Wanfang, CMJ, SinoMed, PubMed, Web of Science, Embase, the Cochrane Library, ClinicalTrials.gov, I

Robot-assisted versus non-robotic partial nephrectomy for completely endophytic renal tumors: a systematic review and meta-analysis.

Completely endophytic renal tumors (CERTs), which are entirely confined within the renal parenchyma, present substantial technical challenges during nephron-sparing surgery. We conducted a systematic review and meta-analysis to compare robot-assisted partial nephrectomy (RAPN) with laparoscopic partial nephrectomy (LPN) and open partial nephrectomy (OPN) specifically for CERTs. PubMed/MEDLINE, Embase, Web of Science, and the Cochrane Library were searched through August 24, 2026. Eligible comparative studies directly evaluated RAPN versus LPN and/or OPN. Continuous outcomes were summarized usi

Perioperative pulmonary infection prevention and management in adult patients after coronary artery bypass grafting: a summary of best evidence.

BACKGROUND: Coronary artery bypass grafting is an important surgical treatment for coronary heart disease. As an open thoracic procedure, pulmonary infection is a common and potentially serious complication of coronary artery bypass grafting. Its occurrence may prolong mechanical ventilation, increase hospital length of stay, and raise mortality risk. This study therefore aims to evaluate and summarise the evidence for the prevention and management of perioperative pulmonary infection in patients undergoing coronary artery bypass grafting to inform clinical practice. METHODS: This evidence sum

Anemia as a Potent and Underrecognized Driver of Venous Thromboembolism: A Systematic Review.

Background: Nutritional deficiency anemias-including iron, vitamin B12, and folate deficiencies-are common worldwide and are increasingly recognized as potential contributors to venous thromboembolism (VTE). Mechanistic and epidemiologic data suggest that anemia may promote thrombosis through hypoxia, endothelial activation, reactive thrombocytosis, and hyperhomocysteinemia. However, a focused synthesis of clinical and genetic evidence specifically linking nutritional deficiency anemia to VTE has been lacking. Methods: We conducted a systematic search of PubMed and the Cochrane Library from in
