The Use of Whole Exome Sequencing in a Cohort of Transgender Individuals to Identify Rare Genetic Variants.

Approximately 0.5-1.4% of natal males and 0.2-0.3% of natal females meet DSM-5 criteria for gender dysphoria, with many of these individuals self-describing as transgender men or women. Despite recent improvements both in social acceptance of transgender individuals as well as access to gender affirming therapy, progress in both areas has been hampered by poor understanding of the etiology of gender dysphoria. Prior studies have suggested a genetic contribution to gender dysphoria, but previously proposed candidate genes have not yet been verified in follow-up investigation. In this study, we

Perspectives From Nurses and Physicians on Training Needs and Comfort Working With Transgender and Gender-Diverse Youth.

INTRODUCTION: Nurses and physicians receive minimal training about providing competent care to transgender and gender-diverse (TGD) patients, and training specific to TGD youth is particularly lacking. This qualitative study examined health care providers' experiences and attitudes about working with TGD youth to identify specific training needs. METHOD: Semistructured interviews were conducted with 14 nurses and physicians who work with adolescents. Thematic analysis was used to characterize participants' responses. RESULTS: Five themes summarized participants' responses to interview questi

A multisite, longitudinal study of risk factors for incarceration and impact on mental health and substance use among young transgender women in the USA.

BACKGROUND: Transgender women are disproportionately incarcerated in the US relative to the general population. A dearth of research has explored the factors that predict incarceration among transgender women or the longitudinal impact of incarceration on the health of this population. METHODS: Between 2012 and 2015, 221 transgender women ages 16-29 from Boston, MA and Chicago, IL were prospectively assessed at baseline, 4, 8 and 12 months. Mixed effects models were used to identify risk factors for incarceration and examine whether incarceration predicts somatic, anxiety and depressive sympt

Characterizing social cohesion and gender identity as risk determinants of HIV among cisgender men who have sex with men and transgender women in Côte d'Ivoire.

PURPOSE: HIV prevalence has been previously estimated among cisgender men who have sex with men (MSM) in Côte d'Ivoire; however, limited data exist relating to the role of social cohesion and gender identity within this population. This study aims to examine these factors as risk determinants of HIV among MSM in Côte d'Ivoire. METHODS: We conducted a cross-sectional study using respondent-driven sampling for recruitment with a structured sociobehavioral instrument and testing for HIV. After respondent-driven sampling adjustment, chi-squared tests and bivariate logistic regression and multivar

Health and Care Utilization of Transgender and Gender Nonconforming Youth: A Population-Based Study.

BACKGROUND: Transgender and gender nonconforming (TGNC) adolescents have difficulty accessing and receiving health care compared with cisgender youth, yet research is limited by a reliance on small and nonrepresentative samples. This study's purpose was to examine mental and physical health characteristics and care utilization between youth who are TGNC and cisgender and across perceived gender expressions within the TGNC sample. METHODS: Data came from the 2016 Minnesota Student Survey, which consisted of 80 929 students in ninth and 11th grade (n = 2168 TGNC, 2.7%). Students self-reported g

Gender incongruence and the brain - Behavioral and neural correlates of voice gender perception in transgender people.

The phenomenon of gender incongruence is hypothesized to arise from a discrepant sexual development of the brain and the genitals, contingent on genetic and hormonal mechanisms. We aimed at visualizing transgender identity on a neurobiological level, assuming a higher functional similarity to individuals of the aspired rather than assigned sex. Implementing a gender perception paradigm featuring male and female voice stimuli, behavioral and functional imaging data of transmen were compared to men and women, and to transwomen, respectively. Men had decreased activation in response to voices of

Why don't key populations access HIV testing and counselling centres in Nepal? Findings based on national surveillance survey.

OBJECTIVES: To assess the demographic, behavioural, psychosocial and structural factors associated with non-utilisation of HIV testing and counselling (HTC) services by female sex workers (FSWs) and men who have sex with men/transgender (MSM/TG). METHODS: This study involved a cross-sectional design. We used the national surveillance survey data of 2012, which included 610 FSWs and 400 MSM/TG recruited randomly from 22 and three districts of Nepal, respectively. Adjusted prevalence ratio (aPR) and 95% confidence interval (CI) using modified Poisson regression was used to assess and infer the

Prevalence and types of rectal douches used for anal intercourse among men who have sex with men in Brazil.

INTRODUCTION: Rectal douching (RD) is practised among men who have sex with men (MSM), and various products and materials are used. There have been no studies in Brazil on this practice and its risks in the transmission of sexually transmitted infections and HIV. METHOD: Between June and August 2015, 401 MSM over the age of 18 were interviewed about their sexual practices associated with RD over the last 3 months. RD was associated with the reported sexual behaviour, and descriptive statistical analyses were conducted on the same. RESULTS: Among the respondents, 85.6% identified themselves a

Laboratory Evaluation of a Point-of-Care Downward-Flow Assay for Simultaneous Detection of Antibodies to Treponema pallidum and Human Immunodeficiency Virus.

Combining the detection of syphilis and HIV antibodies into one point-of-care test integrates syphilis screening into already existing HIV screening programs, which may be particularly beneficial in settings such as antenatal care. Using the INSTI Multiplex downward-flow immunoassay, we tested 200 stored serum samples from high-risk patients enrolled in a longitudinal study on HIV infection and syphilis in Peruvian men who have sex with men and transgender women. This rapid assay detected HIV and Treponema pallidum serum antibodies with sensitivities of 100% (95% confidence interval [CI], 95.9

Mental health impacts of anti-trans stigma on transgender people in Australia: a mixed-methods study.

BACKGROUND: Anti-trans stigma has escalated globally, yet there is limited empirical evidence examining its mental health impacts on trans populations. METHODS: In a May 2024 mixed-methods cross-sectional online survey, trans people aged ≥16 years living in Australia reported the frequency of seven anti-trans stigma experiences in the previous month, including exposure to anti-trans remarks in the news media or online; personal experience of online bullying, harassment or threats; having thought about, seen or heard about anti-trans rhetoric by Australian politicians and high-profile individu

A National Multidisciplinary Survey of Current Practice Trends in Breast Cancer Screening After Gender Affirming Mastectomy: A Call for Inclusive Care.

BACKGROUND: Gender-affirming mastectomy (GAM) often preserves residual breast tissue, conferring an unknown but nonzero risk of breast cancer. However, current breast screening guidelines are inconsistent and lack evidence-based specificity for this population. To date, no study has evaluated how plastic and breast surgeons approach post-GAM cancer surveillance or their confidence in doing so. Therefore, this study aimed to evaluate the practices and perspectives of plastic and breast surgeons regarding breast cancer screening following GAM. METHODS: A 12-item electronic survey was distribute

Transperineal Ultrasound Evaluation of Pelvic Floor Anatomy in Transgender Women After Penile Inversion Vaginoplasty: A Pilot Study.

AIMS: Penile inversion vaginoplasty is the most commonly performed gender-affirming surgery in transgender women and results in the creation of a neovaginal canal while preserving several pelvic structures, including the urethra, prostate, and elements of erectile tissue. However, postoperative pelvic floor anatomy in transgender women has not been well characterized using functional imaging techniques. The aim of this study was to describe pelvic floor anatomy on transperineal ultrasound in transgender women after penile inversion vaginoplasty and to evaluate the feasibility and interobserver

Penile implants after phalloplasty in transgender individuals: a consensus-based surgical clinical protocol concerning pre-, peri-, and postoperative care.

BACKGROUND: Individuals undergoing phalloplasty as part of their gender-affirming medical transition are subsequently able to undergo insertion of a penile implant to achieve rigidity in the neophallus for penetrative intercourse. These procedures require specific knowledge and surgical skills, as the anatomy of the neophallus differs from that of the native penis. Currently, there is no universally accepted guideline for preoperative assessment, surgical technique, or postoperative care. AIM: To address this gap, we developed consensus-based protocol informed by expert opinions to standardiz

Brief Report: Disparities in PrEP Use Among HIV-Negative Black and Latina Women of Transgender Experience and Transfeminine People in Los Angeles County from 2019 to 2020 to 2023-2024.

This study examined PrEP uptake and the role of gender-affirming care among Black and Latina women of transgender experience and transfeminine people (WTE/TFP) in Los Angeles County from 2019 to 2020 and 2023-2024. PrEP use increased substantially among Latinas (28% to 49%) but declined among Black participants (32% to 21%). PrEP uptake was more prevalent among participants using hormones or reporting methamphetamine use. Having trusted gender-affirming providers increased PrEP use more for Black than Latina participants. Our findings highlight the importance of integrating HIV prevention with

Persistence on Oral HIV Pre-Exposure Prophylaxis Among Key Populations at a Centre in Buenos Aires: A Survival Analysis of a Real-World Cohort.

INTRODUCTION: Although HIV pre-exposure prophylaxis (PrEP) has been rapidly scaled up in Argentina to reach key populations with high HIV burden, evidence on real-world persistence is lacking. Understanding who remains engaged in care, and why others discontinue, is essential to ensure the effectiveness of PrEP. This study evaluated oral PrEP persistence and identified demographic and behavioural factors associated with discontinuation in a cohort of individuals initiating PrEP in Buenos Aires. METHODS: We performed a retrospective cohort study including all users initiating daily or event-dr

Reducing HIV Disparities Among Gay, Bisexual, and Other MSM and Transgender Women in Guatemala: A Blueprint for Action.

Guatemalan gay, bisexual, and other men who have sex with men (GBMSM) and transgender women experience profound HIV disparities, with prevalence rates far exceeding those of the general population. Limited access to and suboptimal use of HIV prevention and treatment services hinder progress towards the Joint United Nations Programme on HIV and AIDS goal to end the epidemic by 2030. Our binational community-based participatory research partnership developed a blueprint of twelve recommendations to reduce HIV disparities in Guatemala. Recommendations include enhancing HIV surveillance, increasin

Perceptions of Long-Acting Injectable Pre-Exposure Prophylaxis Among Men Who Have Sex With Men and Transgender Individuals in Europe Using Structural Text Modeling Technique: Cross-Sectional Study.

BACKGROUND: Oral pre-exposure prophylaxis (PrEP) effectively prevents HIV but remains unevenly accessible across Europe. Long-acting PrEP (LA-PrEP), recently approved in Europe, offers new HIV protection options. However, no qualitative evidence is available to inform people's perceptions of this novel modality. OBJECTIVE: This study provides the first large-scale, qualitative evidence in English from 20 European countries on how men who have sex with men (MSM) and trans* individuals perceive LA-PrEP. METHODS: We analyzed open-ended responses from 3123 HIV-negative MSM and trans* individuals
