Construct Choice for the Treatment of Displaced, Comminuted Olecranon Fractures: are Locked Plates Cost Effective?

BACKGROUND: Cost effective implant selection in orthopedic trauma is essential in the current era of managed healthcare delivery. Both locking and non-locking plates have been utilized in the treatment of displaced fractures of the olecranon. However, locking plates are often more costly and may not provide superior clinical outcomes. The primary aim of the present study is to assess the clinical and functional outcomes of olecranon fractures treated with locked and non-locking plate and screw constructs while providing insight into the cost of various implants. METHODS: We performed a retros

Primary Retrograde Tibiotalocalcaneal Nailing For Fragility Ankle Fractures.

BACKGROUND: Ankle fragility fractures are difficult to treat due to poor bone quality and soft tissues as well as the near ubiquitous presence of comorbidities including diabetes mellitus and peripheral neuropathy. Conventional open reduction and internal fixation in this population has been shown to lead to a significant rate of complications. Given the high rate of complications with contemporary fixation methods, the present study aims to critically evaluate the use of acute hindfoot nailing as a percutaneous fixation technique for high-risk ankle fragility fractures. METHODS: In this stud

Three Methods Of Guided Growth For Pediatric Lower Extremity Angular Deformity Correction.

BACKGROUND: Different methods of guided growth are used for correction of angular deformity in growing children. The differences between these different methods are not well described in the literature. METHODS: A retrospective review was undertaken comparing the effectiveness and complication rates of titanium staples, titanium eight-plates, and the stainless steel Pedi-plate at a tertiary pediatric hospital after IRB approval. RESULTS: 77 patients were included in the analysis. Average follow up was 18 months after implantation (range 7-22). Stainless steel implants showed significantly lo

Delayed Proximal Migration of the Radius Following Radial Head Resection for Management of a Symptomatic Radial Neck Nonunion Managed with Radial Head Replacement: A Case Report and Review of the Literature.

BACKGROUND: Long-term outcomes of radial head resection for radial head fracture have shown mixed outcomes, depending on the integrity of the soft-tissue stabilizers of the elbow, forearm, and wrist. METHODS: We report a case of a symptomatic delayed proximal migration of the radius after radial head excision for radial neck nonunion which was managed with a staged radial head replacement. Informed consent was obtained from the individual in this case report. RESULTS: At 7 months after radial head replacement, the patient had 90 degrees of forearm supination and 85 degrees of pronation. Elbo

Clinical Accuracy of J-Sign Measurement Compared to Magnetic Resonance Imaging.

BACKGROUND: The J-sign is defined as lateral patellar translation over the anterolateral femur proximal to the trochlear groove during active leg extension. Dynamic magnetic resonance imaging (MRI) techniques allow for quantification of the J-sign using a variety of published indices. However, to date, clinical quantification of the J-sign has not been reliably described. The purpose of the present study is to assess the accuracy of clinically quantifying the J-sign compared with objective MRI data. METHODS: All patients in this case series were indicated for Fulkerson osteotomy due to recurr

A Biomechanical Comparison Of Pin Configurations Used For Percutaneous Pinning Of Distal Tibia Fractures In Children.

BACKGROUND: Percutaneous pin fixation is often used in conjunction with closed-reduction and cast immobilization to treat pediatric distal tibia fractures. The goal of this procedure is to maintain reduction and provide improved stabilization, in effort to facilitate a more anatomic union. We conducted a biomechanical study of the torsional and bending stability of three commonly used pin configurations in distal tibia fracture fixation. METHODS: A transverse fracture was simulated at the metaphyseal/diaphyseal junction in 15 synthetic tibias. Each fracture was reduced and fixed with two Kirs

Protrusio After Medial Acetabular Wall Breach in Total Hip Arthroplasty.

BACKGROUND: Medial protrusio is a recognized complication of total hip arthroplasty, but it is not known if a medial wall breach during cup implantation increases the risk. We thus investigated the effect of up to a 2 cm defect in the medial acetabular wall in a cadaveric model. Separately, we investigated the ability of acetabular screws to rescue the construct. METHODS: Nine human fresh-frozen hemipelves were reamed medially to create the defect, implanted with acetabular cups, and then loaded to failure. The nine contralateral hemipelves were reamed in a standard fashion and served as cont

Dysplasia Epiphysealis Hemimelica Treated with Osteochondral Allograft: A Case Report.

BACKGROUND: Dysplasia epiphysealis hemimelica (DEH), or Trevor's disease, is a developmental disorder of the pediatric skeleton characterized by asymmetric osteochondral overgrowth. METHODS: We present the case of a five year old boy with a two year history of right knee pain and evidence of DEH on imaging who underwent initial arthroscopic resection of his lesion with subsequent recurrence. The patient then underwent osteochondral allograft revision surgery and was asymptomatic at two year follow-up with a congruent joint surface. RESULTS: To our knowledge, this is the first reported case o

Outcomes Following Low-Energy Civilian Gunshot Wound Trauma to the Lower Extremities: Results of a Standard Protocol at an Urban Trauma Center.

BACKGROUND: Lower extremity injuries secondary to low-energy gunshot wounds are frequently seen in the civilian populations of urban areas. Although these wounds have fewer complications than high-energy gunshot injuries, the functional and psychological damage is still significant making appropriate timely orthopaedic treatment and follow-up imperative. PURPOSE: The purpose of this study is to present our outcomes in the treatment of low-energy gunshot wounds in a civilian population at an urban, level one trauma center in patients treated by a standard protocol. METHODS: One hundred and th

Fractures Due to Gunshot Wounds: Do Retained Bullet Fragments Affect Union?

BACKGROUND: Many types of projectiles, including modern hollow point bullets, fragment into smaller pieces upon impact, particularly when striking bone. This study was performed to examine the effect on time to union with retained bullet material near a fracture site in cases of gunshot injury. METHODS: All gunshot injuries operatively treated with internal fixation at a Level 1 Trauma Center between March 2008 and August 2011 were retrospectively reviewed. Retained bullet load near the fracture site was calculated based on percentage of material retained compared to the cortical diameter of

A Finite Element Analysis of Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: The medial patellofemoral ligament is the primary soft-tissue restraint to lateral patella translation. Medial patellofemoral ligament reconstruction has become a viable surgical option to provide patellar stability in patients with recurrent instability. The primary goal of this study was to determine the effect of medial patellofemoral ligament reconstruction on the lateral force-displacement behavior of the patella using finite element analyses. METHODS: A finite element model of the knee was created using cadaveric image data. Experimental testing was performed to validate the

Lateral Epicondylitis and Tobacco Use: A Case-Control Study.

BACKGROUND: Although lateral epicondylitis (LE) is a very common tendinopathy, we understand little about the etiology of the disease. Tobacco use has been associated with other tendinopathies, and the purpose of this study is to determine if there is an association between the incidence of lateral epicondylitis and tobacco use. METHODS: We performed a retrospective cohort study of adult patients diagnosed with lateral epicondylitis. Patients from a single orthopaedic surgeon's practice with LE were matched to control patients with other common upper extremity conditions based on age, gender,

Progressive adult spinal deformity following placement of intrathecal opioid pump: a report of four cases.

INTRODUCTION: Placement of intrathecal opioid pumps (ITOP) for chronic pain is a rare, but described cause of progressive spinal deformity. Over the last two decades there has been several suspected cases at our institution. In this case series, we described the apparent association between placement of an intrathecal opioid pump and progression of spinal deformity. METHODS: The medical records of a single surgeon working at a single institution were retrospectively queried for patients seen between 1995-2010 to identify patients with spinal deformity and an ITOP. All hospital records includi

Patella fracture fixation with suture and wire: you reap what you sew.

INTRODUCTION: Operative fixation of displaced inferior pole patella fractures has now become the standard of care. This study aims to quantify clinical, radiographic and functional outcomes, as well as identify complications in a cohort of patients treated with non-absorbable braided suture fixation for inferior pole patellar fractures. These patients were then compared to a control group of patients treated for mid-pole fractures with K-wires or cannulated screws with tension band wiring. METHODS: In this IRB approved study, we identified a cohort of patients who were diagnosed and treated s

Minimizing alteration of posterior tibial slope during opening wedge high tibial osteotomy: a protocol with experimental validation in paired cadaveric knees.

INTRODUCTION: The High Tibial Osteotomy (HTO) is a reliable procedure in addressing uni- compartmental arthritis with associated coronal deformities. With osteotomy of the proximal tibia, there is a risk of altering the tibial slope in the sagittal plane. Surgical techniques continue to evolve with trends towards procedure reproducibility and simplification. We evaluated a modification of the Arthrex iBalance technique in 18 paired cadaveric knees with the goals of maintaining sagittal slope, increasing procedure efficiency, and decreasing use of intraoperative fluoroscopy. METHODS: Nine pair

Patient perceptions and preferences when choosing an orthopaedic surgeon.

PURPOSE: Information regarding patient preferences is important to develop more diversity in healthcare providers. To our knowledge, no information exists regarding how patients choose their orthopaedic surgeon. The purpose of this study is to determine which demographic factors, if any, affect patient preferences when choosing an orthopaedic surgeon. METHODS: Five hundred new patients presenting to a large, urban, academic orthopaedic clinic from May 2011 to May 2013 were prospectively asked to participate in this study. Patients were asked to complete a survey designed with the help of the

Outcomes of total joint arthroplasty in HIV patients.

BACKGROUND: Advancement in human immunodeficiency virus (HIV) therapies has increased life expectancy. The need for joint replacement is expected to increase as this population develops degenerative changes from aging and avascular necrosis (AVN). Studies have shown a higher risk of peri-prosthetic joint infections (PJI) in HIV patients. However, these studies include a high percentage of hemophiliacs, which may be a confounding variable. With the advent of highly active anti-retroviral therapy (HAART) and evolving HIV demographics, we hypothesize the rate of PJIs in HIV patients are comparabl

Correlation of radiographic patellofemoral indices with tibial tubercle transfer distance in Fulkerson osteotomy procedures.

BACKGROUND: A laterally tracking patella is commonly seen in patients with chronic recurrent lateral patellar dislocations. Clinical appearance of the J-sign occurs when the patella is congruent with the trochlear groove in flexion and moves over the lateral border of the femoral condyle as the lower leg reaches complete extension. A Fulkerson osteotomy procedure corrects this maltracking of the patella by medially transferring the tibial tubercle. There are many radiographic patellofemoral indices that can be used describe this incongruence about the patelloformal joint. The current literatur

Nerve injury and pain after operative repair of calcaneal fractures: a literature review.

Peripheral nerve injury is a common problem in foot and ankle surgery. We look at evidence of nerve injury as it relates to different operative approaches to the fractured calcaneus. The direct lateral, extended lateral, smile, sinus tarsi, and percutaneous approaches are discussed and the reported incidence of nerve injury in each is identified. We expect to identify divergent rates of injury between approaches and stimulate further investigation into prevention and treatment.
