Application and prospects of brain-computer interface technology for motor function reconstruction after brachial plexus injury.

BACKGROUND: Brachial plexus injury (BPI) is a severe peripheral nerve disorder leading to significant upper limb motor dysfunction. While traditional surgeries like nerve grafting and tendon transfer exist, functional outcomes are often suboptimal due to biomechanical limitations and slow neural recovery. Brain-computer interface (BCI) technology has emerged as a promising innovative pathway for motor function reconstruction. OBJECTIVE: This review systematically evaluates the current applications, physiological mechanisms, and technical challenges of BCI technology specifically within the cl…

Spinal nerve origins of the muscular branches of the radial nerve: an electrophysiological study.

BACKGROUND: In injuries of the lower brachial plexus, finger flexion can be restored by nerve or tendon transfer. However, there is no technique that can guarantee good recovery of finger and thumb extension. OBJECTIVE: To determine the spinal nerve origins of the muscular branches of the radial nerve and identify potential intraplexus donor nerves for neurotization of the posterior interosseous nerve in patients with lower brachial plexus injuries. METHODS: An intraoperative electrophysiological study was carried out during 16 contralateral C7 nerve transfers. The compound muscle action pot…

[Evaluation of late results of reconstructions of the thumb by trimmed great toe method].

Twelve patients with posttraumatic defects of the thumb were treated in the years 1990-1999 by the trimmed great toe method. Eight of them had additional injuries or loss of long fingers (two patients lost all fingers). Reconstruction methods were chosen basing on the amputation level. A fragment of the trimmed great toe of an appropriate length was used distally to the distal part of the first metacarpal according to the technique described by Wei (9 patients). In cases of a more proximal amputation only a distal part of the great toe was used. Pollicisation of other elements of the index wit

Free flap reconstruction of the foot.

Free flap reconstruction of the foot has been widely performed in the last 20 years, but choice of a free transferred substitute for the soft tissue of the particular defect remains controversial. The authors present a series of 77 free flaps to the foot performed in 68 patients during October 1976 and September 1997. Long-term follow-up ranged from 12 months to 18 years (median, 44.4 months). Seventy-three flaps were transferred successfully (95%). The indications for a specific flap depended on the localization and extension of the foot defect. In weight-bearing areas the authors favored the

Challenges and Controversies in Treating Massive Rotator Cuff Tears.

Massive rotator cuff tears present several challenges for orthopaedic surgeons. Many rotator cuff tears can be repaired; however, some chronic rotator cuff tears require advanced reconstructive techniques. Repair, if possible, is the optimal treatment for rotator cuff tears. In general, muscle transfers are an option for patients younger than 60 years who do not have pseudoparalysis. Arthroplasty is an option for older patients who have concomitant arthritis and for patients who have pseudoparalysis. Biologic augmentation in the setting of rotator cuff tears continues to evolve, and the applic

Free Flap Reconstruction of Traumatic Skin Defects of the Entire Hand Dorsum.

Background/Objectives: The reconstruction of hand defects, especially involving the dorsal region of the hand, has remained a challenge for surgeons because of its anatomical features and complex functions. The goal of reconstruction should include functional restoration as well as being esthetically pleasing. The flap transfer reconstruction strategy is essential for satisfying these requirements. Methods: Free flaps were used to cover traumatic defects of the hand dorsum in eleven patients from 2016 to 2022. Eight males and three females with a mean age of 41 years were enrolled. The size of

Ballistic peripheral nerve injuries: basic concepts, controversies, and proposal for a management strategy.

Ballistic injuries to peripheral nerves are devastating injuries frequently encountered in modern conflicts and civilian trauma centers. Suc … nerve transfers, especially at the brachial plexus level. Additionally, combined or early secondary tendon transfers are helpful in certain injuries. Finally, ideal timing for nerve repair is proposed, based on the defect length, associated injuries, and risk of infection, which correlate intimately to the projectile velocity.

The Marshall R. Urist Young Investigator Award. Orthopaedic applications of gene therapy. From concept to clinic.

Gene therapy offers new possibilities for the clinical management of orthopaedic conditions that are difficult to treat by traditional surgical or medical means. To bring the potential of this novel technology into the clinic, a research program was initiated that aimed to identify orthopaedically useful genes and develop methods for delivering them to suitable sites under conditions in which gene expression remains at therapeutic levels for the appropriate periods of time; this program is now 10 years old. Rheumatoid arthritis was selected as the lead disease. Preclinical studies evaluating t

Efficacy and Safety of Split Peroneal Tendon Lateral Ankle Stabilization.

Chronic lateral ankle instability is a common condition. Split peroneal tendon lateral ankle stabilization, a modification of the Chrisman-Snook procedure, is biomechanically stable and often used for severe and/or recurrent chronic lateral ankle instability. The purpose of the present study was to evaluate the efficacy and safety of this technique. Specifically, the midterm recurrence of instability and postoperative complications, such as stiffness, neurologic pain, and wound healing complications, were evaluated. We evaluated 30 consecutive procedures with a minimal follow-up period of 1 ye

What proportion of patients need extensive surgery after failure of the Ponseti technique for clubfoot?

UNLABELLED: In 1948, Professor Ignacio Ponseti began a nonoperative management form of treatment for severe talipes equinovarus. This method of manipulative treatment became attractive because long-term outcomes demonstrated the majority of feet were pain-free, plantigrade, and functioning at a high level of activity without evidence of degenerative arthrosis. We retrospectively reviewed the charts of 51 children (31 boys and 20 girls; 72 feet) with idiopathic clubfeet deformity treated with the Ponseti method from January 5, 2002, to January 5, 2007. The median age at treatment was 2 weeks (9

Long-term comparative results in patients with congenital clubfoot treated with two different protocols.

BACKGROUND: Long-term follow-up studies of adults who had been treated for congenital clubfoot as infants are rare. The purpose of this study was to review and compare the long-term results in two groups of patients with congenital clubfoot treated with two different techniques. In both groups, treatment was started within the first three weeks of life by manipulation and application of toe-to-groin plaster casts, with a different technique in each group. At the end of the manipulative treatment, a posteromedial release was performed when the patient was between eight and twelve months of age

Three-Dimensional Preoperative Planning of Corrective Osteotomies for Distal Radius Malunions: A Systematic Review of Clinical and Radiographic Outcomes.

Distal radius malunions (DRMs) are painful and functionally impairing, often necessitating surgical intervention to restore wrist anatomy and function. Traditional corrective osteotomies, which rely on orthogonal radiographs, may overlook complex deformities. This study aims to evaluate the techniques and effectiveness of 3-dimensional (3D)-planned corrective osteotomies, along with their clinical and radiographic outcomes. We conducted a systematic review of literature across PubMed, Ovid, EMBASE, and Web of Science for studies that implemented 3D planning in corrective osteotomies for DRM. W

Accessory flexor carpi ulnaris muscle with associated anterior interosseous artery variation: case report with the definition of a new type and review of concomitant variants.

PURPOSE: Knowledge of accessory flexor carpi ulnaris (AFCU) is not only important for proper orientation in the surgical field but it can be used for tendon transfer as well. AFCU commonly occurs with concomitant variants, and its presence should rise caution in order to prevent iatrogenic injury. METHODS: During a routine dissection for research data collection at the Institute of Anatomy, a AFCU with concomitant variants was observed in a European cadaver fixed with Thiel's method. A thorough review of the literature concerning all the encountered variants was performed. RESULTS: AFCU was …

Restoration of shoulder function with nerve transfers in traumatic brachial plexus palsy patients.

Shoulder stabilization is of utmost importance in upper extremity reanimation following paralysis from devastating injuries. Although secondary procedures such as tendon and muscle transfers have been used, they never achieve a functional recovery comparable to that following successful reinnervation of the supraspinatus, deltoid, teres minor, and infraspinatus muscles. Early restoration of suprascapular and axillary nerve function through timely brachial plexus reconstruction offers a good opportunity to restore shoulder-joint stability, adequate shoulder abduction, and external rotation func

Thumb deformity in the spastic hand: classification and surgical techniques.

Surgery based on an accurate determination of deforming forces, muscle weakness, and joint instability achieves reasonably predictable results in patients with cerebral palsy. Thumb deformities can be classified into those of mainly intrinsic nature, extrinsic nature, and those of a combined nature. This allows identification of which muscles are released most appropriately and which muscle function requires augmentation. Joint instability needs to be taken into consideration because tendon transfers across unstable joints will be ineffective. The various surgical procedures are outlined in th…

[A case of popliteal pterygium syndrome. Considerations of timing and method of therapy].

A case of popliteal pterygium syndrome is presented and the various possibilities of treatment so far reported in the literature are discussed. The medical history of the 22-year-old male patient shows that there is no uniform concept in its therapy and that even multiple operations, which dealt only with the skin contracture and the shortened achilles tendon, were not successful. An extensive neurolysis of the sciatic nerve, its transposition into deeper muscle layers and slow extension of the knee joint with a dynamic splint did improve the patient's condition and this seems to be the concep

The Kager Triangle: An Anatomic Review and Potential Implications for Lymphatic Surgery.

Vascularized lymph node transplant (VLNT) is widely performed for chronic upper and lower extremity lymphedema. However, ideal recipient sites for the transplant are still under debate. The placement of VLNTs distally in an extremity can be challenging as the small cross-sectional area of the limb at this level does not allow for flap inset without gross contour deformity, which can adversely impact aesthetic outcomes and preclude fitting of an adequate compression garment. In this article, we introduce the Kager triangle as a potential distal lower extremity VLNT recipient site for the lower
