Paraplegia Due to Spinal Cord Ischemia after Endovascular Treatment of a Type II Endoleak.

Type II endoleaks are a common complication after endovascular abdominal aortic aneurysm repair, with transarterial embolization using synthetic surgical glue being an established treatment option. We report a case of paraplegia due to spinal cord ischemia after lumbar arteries embolization by Glubran-lipiodol glue for a type II endoleak. Special attention must be given by interventional specialists when applying surgical diluted glues for the treatment of type II endoleaks to avoid distal embolization and subsequent spinal cord ischemia.

Secondary Endovascular Aneurysm (EVAS) Sealing in Combination With Chimney Grafts to Treat Failed Chimney EVAS.

PURPOSE: To describe a proximal extension of a failed chimney endovascular aneurysm sealing repair (chEVAS) using a chEVAS-in-chEVAS procedure in 2 cases with successful treatment outcome at 2-year follow-up. CASE REPORT: Two patients with an infrarenal abdominal aortic aneurysm were treated with an elective chEVAS procedure with 1 chimney stent for a unilateral renal artery. At 18 and 24 months, respectively, both patients showed aneurysm growth with an associated decrease in proximal seal. Both patients were treated with a secondary chEVAS procedure, consisting of chimney stent-graft placem

Inadvertent Migration of Umbilical Venous Catheters Often Leads to Malposition.

BACKGROUND: Migration of umbilical venous catheters (UVCs) has been described anecdotally. OBJECTIVES: The aim of this paper was to investigate migration of UVCs using ultrasonography (US). METHODS: In a prospective observational study, the position of UVCs was determined using serial US within 24 h, at midweek, and at the end of the week after umbilical catheterization. Migration was recorded in distance and direction. Malposition was defined as a position of the UVC in the heart (right atrium or more distal along the UVC-route), umbilicoportal confluence, or in the umbilical vein. UVC posi

Large Bariatric-Specific Stents and Over-the-Scope Clips in the Management of Post-Bariatric Surgery Leaks.

BACKGROUND: Endoscopic stents are successful in the management of surgical leaks; however, stent migration remains a significant problem. In this study, we present our approach depending on a large bariatrics-specific stent (Mega stent) and over-the-scope clips in the management of post-bariatric surgery leaks. METHODS: A retrospective analysis of all patients with post-bariatric surgery leaks treated at our institution using an approach reliant on Mega stents and over-the-scope clips was conducted. Potential factors associated with procedure success and occurrence of complications were also

A Delayed Appearance of the Secondary Bladder Stone 10 Years After Tension-free Vaginal Tape Procedure.

The occurrence of bladder calculi in women is uncommon, and the evaluation of bladder stone in women with any history of previous pelvic surgery demands careful attention. Bladder erosion due to previous anti-incontinence surgery formed the nidus for stone formation, and often presented as recurrent cystitis or refractory urinary incontinence since the early postoperative period. We hereby report a rarely delayed appearance of the symptomatic iatrogenic bladder stone occurring 10 years after previous tension-free vaginal tape procedure.

Case of a missed airway stent migration.

This case report describes a rare complication to stent management of airway compression. An 84- year-old man was admitted to the outpatient clinic with haemoptysis, cough and dyspnoea. A CT scan showed an intraluminal, non-occluding tumour close to the right side of the main carina. A stent was inserted in the right main bronchus. The patient was, 7 months later, admitted to the emergency room with severe cough and dyspnoea. The stent was produced during a coughing spell. Retrospective investigation of available imaging revealed that the stent had been dislodged to a nearly 'perfect' position

Delayed Superficial Migration of Retained Hyaluronic Acid Years Following Periocular Injection.

Cosmetic injection of hyaluronic acid (HA) and other fillers is increasingly common, and the late complications of these relatively new procedures are now coming to medical attention. Three patients with delayed periocular swelling that began years after injection of HA are described, with CT, MRI, and histopathologic characterization. While HA fillers are marketed as having a temporary effect of several months, the authors demonstrate that they may persist in the body for up to 9 years. Unlike most previous reports, there was no inflammatory reaction or encapsulation, simply infiltration into

[Efficacy and safety of endotracheal intubation performed in moving vs motionless environments].

OBJECTIVES: To compare the efficacy and safety of endotracheal intubation (ETI) in a simulated clinical environment in motion vs a motionless one. MATERIAL AND METHODS: Clinical simulation trial of ETI with 3 endotracheal tubes (Airtraq, Fast-trach, Macintosh laryngoscope) in mannequins with realistic physiological responses (MetiMan) in 2 scenarios: an environment in motion vs a motionless one. Thirty-six physicians expert in prehospital ETI participated. Outcome variables were successful intubation, effective intubation, number of attempts, maximum apnea time, and total maneuver time. The s

Intracranial Migration of Silicone Delaying Life Saving Surgical Management: A Mimicker of Hemorrhage.

We present a case in which intraocular silicone injection for complex retinal detachment resulted in migration and distribution of silicone along the intracranial visual pathway, and ultimately throughout the ventricular system. Misinterpretation of this material as intracranial hemorrhage on outside computed tomography imaging delayed emergent repair of a Type A aortic dissection until the diagnosis was made on repeat imaging. A discussion of this case and salient computed tomography and magnetic resonance imaging characteristics of silicone is provided.

Dislodgement and gastrointestinal tract penetration of bone cement used for spinal reconstruction after lumbosacral vertebral tumor excision: A case report.

INTRODUCTION: Polymethylmethacrylate (PMMA) cement is useful for spinal reconstruction, but can cause complications including new vertebral fractures, neurological disorders and pulmonary embolism. We report a case in PMMA cement used for spinal reconstruction after tumor curettage dislodged and penetrated the gastrointestinal tract. DIAGNOSES: The patient was diagnosed with a retroperitoneal extragonadal germ cell tumor at age 27 years. After chemotherapy and tumor resection, the tumor remained. It gradually increased in size and infiltrated lumbosacral vertebrae, causing him to present at a

Central venous catheter-induced pericardial effusion in a neonate: a case study and recommendations for practice.

Pericardial effusion (PCE) and tamponade as a complication of central venous catheters (CVCs) continues to occur in the neonatal population. It is imperative that clinicians managing neonates with CVCs practice vigilance regarding proper catheter tip location. Furthermore, it is of equal importance that clinicians have a high index of suspicion regarding the catheter tip location any time a neonate with a CVC has a clinical deterioration. It is clear that the ultimate outcome of PCE depends on rapid identifi cation and a quick response when pericardial tamponade occurs. PCE can occur any time

Spontaneous transanal protrusion of ventriculoperitoneal catheter: a case report.

Ventriculoperitoneal (VP) shunt placement that diverts the cerebrospinal fluid (CSF) into the peritoneal cavity is the most common method of treatment of hydrocephalus. This shunt has a high incidence of malfunction mainly due to catheter obstruction or infection. About 20% of these complications are abdominal that may occur at any time after shunt placement from 1 week to several years. This study reports a case of 2.5-year old child with a history of hydrocephalus who had a VP shunt placed which was protruded from the anus on the day of referral. The patient was treated successfully after ex

Unexpected complication with the new C3 excluder: cause and treatment.

The new C3 Gore Excluder delivery system enables both up/downward and rotational repositioning of the device before complete deployment. This contributes to more precise proximal landing and permits facilitation of the contralateral gate cannulation. During separate deployment, the position of the ipsilateral limb can also be readjusted. We have used the modified C3 delivery system in more than 50 patients, and in most cases, we were able to utilize the repositioning options of the device to achieve optimal fixation and sealing. However, we present a case where our attempt to readjust the posi
