Symptomatic spinal extramedullary mass lesion secondary to chronic overdrainage of ventricular fluid--case report.

A 69-year-old man presented with progressive nuchal pain and spastic gait 2 years after undergoing ventriculoperitoneal (VP) shunting for a pineal astrocytoma with obstructive hydrocephalus. The neurological manifestations were compatible with radiculomyelopathy caused by an upper cervical lesion. Magnetic resonance imaging showed an enhanced extramedullary mass lesion tightly constricting the upper cervical spinal cord. The pressure of the shunt system was 150 mmH2O, and lumbar puncture revealed normal cerebrospinal fluid (CSF) pressure of 170 mmH2O. After removal of the shunt system, the cli

The clinical trials landscape of hydrocephalus: a scoping review of therapeutic priorities, innovation trends, and research gaps.

Hydrocephalus includes a diverse group of disorders with different causes, symptoms, and treatment options. Despite progress in surgery and diagnostics, the overall understanding of clinical research in hydrocephalus remains limited. We performed a scoping review to systematically examine registered clinical trials, aiming to identify trends in treatment approaches, research areas, and gaps in the current evidence. A comprehensive search of ClinicalTrials.gov was conducted to identify all registered clinical trials related to hydrocephalus. Trials were included regardless of study design, phas

The psychiatry of vertebro-basilar insufficiency with the report of a case.

Vertebro-basilar insufficiency produces a rich spectrum of psychological and neurological symptoms. Where psychological symptoms dominate the picture, the patient may be presented first to a psychiatrist. The phenomenology of vertebro-basilar insufficiency is discussed with special reference to hallucinatory syndromes, memory disturbance, affective disorders, akinetic mutism, 'unusual reports', cortical blindness, agitated delirium, the Capgras syndrome and normal pressure hydrocephalus. Finally, the case of a 61-year-old man illustrating a variety of the neurological and psychological feature

Neurophysiological insights into iNPH: ABR and CAEP responses to lumbar puncture and shunt surgery.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a neurological syndrome resulting from increased cerebrospinal fluid volume without significant increase in intracranial pressure. The diagnosis is difficult and investigating other tools that can assist in this process is essential. Auditory Evoked Potentials (AEPs) can be used to assess the auditory pathway integrity and monitor cognitive auditory functions. The objective of this study was to investigate the applicability of the AEPs as a potential monitoring tool for detecting clinical improvements associated with cerebrospinal

Dynamic multi-scale deep learning with mixture of experts for differentiating iNPH and PSP using MRI.

PURPOSE: Distinguishing idiopathic normal pressure hydrocephalus (iNPH) from progressive supranuclear palsy (PSP) presents a clinical challenge due to overlapping clinical symptoms such as gait disturbances and cognitive decline. This study presents a novel multi-scale deep learning framework that integrates global and local magnetic resonance imaging (MRI) features using a mixture of experts (MoE) mechanism, enhancing diagnostic accuracy and minimizing interobserver variability. METHODS: The proposed framework combines a 3D convolutional neural network (CNN) for capturing global volumetric f

Development of a Gait Analysis Application for Assessing Upper and Lower Limb Movements to Detect Pathological Gait.

Pathological gait in patients with Hakim's disease (HD, synonymous with idiopathic normal-pressure hydrocephalus; iNPH), Parkinson's disease (PD), and cervical myelopathy (CM) has been subjectively evaluated in this study. We quantified the characteristics of upper and lower limb movements in patients with pathological gait. We analyzed 1491 measurements of 1 m diameter circular walking from 122, 12, and 93 patients with HD, PD, and CM, respectively, and 200 healthy volunteers using the Three-Dimensional Pose Tracker for Gait Test. Upper and lower limb movements of 2D coordinates projected ont

Vascular risk factors in INPH: A prospective case-control study (the INPH-CRasH study).

OBJECTIVE: To assess the complete vascular risk factor (VRF) profile of idiopathic normal pressure hydrocephalus (INPH) using a large sample of representative patients with INPH and population-based controls to determine the extent to which vascular disease influences INPH pathophysiology. METHODS: All patients with INPH who underwent shunting in Sweden in 2008-2010 were compared to age- and sex-matched population-based controls. Inclusion criteria were age 60-85 years and no dementia. The 10 most important VRFs and cerebrovascular and peripheral vascular disease were prospectively assessed u

Neurological symptoms in a patient with isolated adrenocorticotropin deficiency: case report and literature review.

BACKGROUND: Isolated adrenocorticotropic hormone (ACTH) deficiency is a pituitary disorder characterized by reduction only in the secretion of ACTH. Although the underlying mechanism remains to be elucidated, numbers of cases with this entity have been increasing. We experienced a case presenting with gait disturbance necessitating differential diagnosis from idiopathic normal pressure hydrocephalus (iNPH). CASE PRESENTATION: A 69-year-old female with a complaint of difficulty walking and suspected to have iNPH at a prior hospital was referred to our department. For the prior three years, she

Twelve-year hospital outcomes in patients with idiopathic hydrocephalus.

OBJECTIVE: The aim of this study was to examine patients who were admitted for the first-ever shunting for idiopathic normal pressure hydrocephalus (INPH) during a 12-year period, in terms of variation rate, patient demographic characteristics, shunt procedures, postoperative complications, and hospital outcome. METHODS: An electronic database which included all shunted patients (1998 to 2009) was used to retrieve demographic, clinical, and hospital outcome data. INPH patient identification was based on clinical and imaging diagnostic criteria. RESULTS: INPH patients (n = 238) who had underg

A simplified method of xenon-enhanced CT for regional cerebral blood flow (rCBF) measurement with reference to clinical experiences.

We introduced a simplified method of xenon-enhanced CT (SXe-CT) for obtaining the topographic regional cerebral blood flow (rCBF) map. In the SXe-CT, we used the method of brief (three to four minutes) inhalation of 40% xenon. To shorten the period of xenon inhalation, we substituted a predetermined brain-blood partition coefficient (L) in the entire brain for the directly estimated L to calculate the rCBF value. Because of its non-invasiveness and simplicity, the SXe-CT can be applied to the patients with diseases in the central nervous system (CNS) without any difficulties. During the past t

[Degenerative cerebellar diseases and differential diagnoses].

CLINICAL/METHODICAL ISSUE: Cerebellar syndromes result in distinct clinical symptoms, such as ataxia, dysarthria, dysmetria, intention tremor and eye movement disorders. STANDARD RADIOLOGICAL METHODS: In addition to the medical history and clinical examination, imaging is particularly important to differentiate other diseases, such as hydrocephalus and multi-infarct dementia from degenerative cerebellar diseases. Degenerative diseases with cerebellar involvement include Parkinson's disease, multiple system atrophy as well as other diseases including spinocerebellar ataxia. ACHIEVEMENTS: In a

Cerebrospinal fluid proteomics identifies calcyphosine and follistatin-like 1 as exploratory candidate proteins of interest in hydrocephalus.

BACKGROUND: Hydrocephalus comprises etiologically heterogeneous disorders that converge on ventricular enlargement but may be associated with distinct protein-abundance patterns within the cerebrospinal fluid (CSF) compartment. This exploratory study compared CSF proteomic profiles in post-hemorrhagic hydrocephalus (PHH) and idiopathic normal pressure hydrocephalus (iNPH) to characterize CSF protein-abundance patterns associated with these two hydrocephalus and identify proteins for further validation. METHODS: Cerebrospinal fluid samples from 11 participants, including five patients with PHH

The light-flash-evoked response as a possible indicator of increased intracranial pressure in hydrocephalus.

Surgical treatment of infantile hydrocephalus by shunt implantation may result in suboptimal intracranial pressure. Major neurological impairments and death are usually prevented by shunt treatment, but minor sequelae may persist or develop. The introduction of adjustable shunts has improved the possibilities of optimizing shunt function and minimizing the risk of such impairments. However, it is still impossible to determine the intracranial pressure without invasive measurements. Clinical findings and procedures such as computed tomography (CT) are not always enough to allow a conclusion as

Unusual clinical manifestations of disturbed CSF dynamics in hydrocephalic children.

OBJECTS: Clinical symptoms and signs of increased intracranial pressure (ICP) may be nonspecific and unreliable, or even entirely absent, in hydrocephalic infants and children. Even with a radiological examination, it is often difficult to distinguish between "arrested hydrocephalus" and slowly progressive hydrocephalus requiring treatment. METHODS: We present two cases with unusual and initially misleading clinical manifestations of increased ICP. In both cases, the disturbed cerebrospinal fluid (CSF) dynamics, i.e. raised ICP, were not recognised until demonstrated by a long-term ICP monito

Evaluation of histopathologic changes in an animal model of mechanical corpus callosum impingement as seen in hydrocephalus.

RATIONALE AND OBJECTIVES: We evaluated histologic changes associated with chronic impingement of the corpus callosum. Similar callosal impingement has been postulated to be responsible for some of the symptoms in people who have hydrocephalus. METHODS: Eight rats with callosal impingement produced by surgical implantation of a blunt blade in the interhemispheric fissure and four control animals with no callosal impingement were evaluated by magnetic resonance (MR) imaging and by direct histologic evaluation after autopsy. The histologic evaluations occurred 1 month after surgery in half the a

Slow unsteady gait in a population-based cohort: links to ventriculomegaly and INPH-related imaging markers.

BACKGROUND: Although clinical evaluation, radiological findings, and supplementary tests are routinely used to diagnose idiopathic normal pressure hydrocephalus (INPH), the large variability in surgical outcomes raises the question of whether diagnostic radiological markers truly reflect the typical clinical gait impairment. Gait disorder (GD) and ventriculomegaly (VM) are both common in older adults and may coincide by chance, potentially contributing to this inconsistency in results. This study aimed to determine the prevalence of typical INPH-related higher-level gait disorder (HLGD), diagn

Spatiotemporal characterization of Alzheimer disease pathology in living human brain tissue.

Alzheimer disease begins in the brain many years before symptoms, but early changes are usually studied after death or indirectly through fluid and imaging biomarkers. Here we show that small brain biopsies collected during ventriculoperitoneal shunt surgery can be used to study Alzheimer disease pathology and protein turnover during life. We analysed biopsies from 18 individuals with suspected normal pressure hydrocephalus, alongside matched ventricular and lumbar cerebrospinal fluid and post-mortem control brain tissue. Amyloid plaques were present in 9 of 18 biopsies, while mature tau tangl

Particular CSF sphingolipid patterns identify iNPH and AD patients.

Idiopathic normal pressure hydrocephalus (iNPH) is characterized by reversible neurological symptoms due to an impairment in cerebrospinal fluid (CSF) clearance. In these patients, cognitive functions are severely impaired, with a scenario similar to Alzheimer's disease (AD), making the differential diagnosis difficult and highlighting the need of new markers. We analyzed the composition of sphingolipids (SLs) in serum, by combining a single phase extraction with a high-performance thin-layer chromatography (HPTLC) primuline-profiling, and, in CSF, by MALDI profiling and LC-MS. Ceramides and s

Selective impairment in the retrieval of family relationships in person identification: a case study of delusional misidentification.

We describe a 74-year-old, right-handed woman who exhibited a peculiar form of delusional misidentification due to Alzheimer's disease (AD) combined with idiopathic normal pressure hydrocephalus (iNPH). The patient's most distinctive symptom was that she often misidentified her daughters as her sisters. She had severe atrophy of the bilateral medial temporal lobe and right-hemisphere-dominant hypoperfusion in the fronto-temporo-parietal cortices. Detailed tests revealed that she had a selective deficit in retrieving the family relationships between herself and her daughters/husband (i.e., she
