The Impact of Femoral Anteversion Correction on Clinical Outcomes in Total Hip Arthroplasty for Adult Developmental Dysplasia of the Hip.

Background: Total hip arthroplasty (THA) in adult patients with developmental dysplasia of the hip (DDH) presents unique surgical challenges due to altered hip anatomy and biomechanics. One of the most critical factors influencing surgical success is correcting femoral anteversion. This study evaluates the effects of different anteversion corrections (increased, unchanged, and decreased) during THA on clinical outcomes in adult Crowe type 3-4 DDH patients. Methods: A retrospective analysis was conducted on 130 patients who underwent THA with anteversion correction between 2008 and 2017 at a te

Surgeon-Applied Stress and a Ligament Tensor Instrument Provide a Similar Assessment of Preresection Flexion Laxity During Robotic Total Knee Arthroplasty.

BACKGROUND: Robotic-assisted total knee arthroplasty (RA-TKA) allows surgeons to perform intraoperative soft tissue laxity assessments prior to bone resections and is used to alter resections to achieve gap balance. This study compared 2 techniques for flexion gap laxity assessment during RA-TKA. METHODS: A prospective study of 50 primary RA-TKAs performed by a single surgeon was conducted between February and October 2023. Following full exposure, anterior tibial dislocation, and osteophyte removal, maximal medial and lateral compartment flexion laxity was quantified to the nearest 0.5 mm by

Decrease in Global Offset Leads to Complaints of Worsening of Valgus Knee Deformity After Total Hip Arthroplasty in Patients With Crowe Type IV Developmental Dysplasia of the Hip.

BACKGROUND: Conflicting evidence exists regarding the impact of total hip arthroplasty (THA) on valgus knee deformities in patients with developmental dysplasia of the hip (DDH). The aim of this retrospective study was to identify the factors potentially contributing to the worsening of valgus knee deformities in DDH patients after THA. METHODS: This retrospective case-control study included 51 patients (69 hips) with Crowe type IV DDH who underwent primary THA between January 2018 and January 2020. The anatomic lateral distal femoral angle (aLDFA), mechanical lateral distal femoral angle (mL

Ultrasound evaluation of the femoral trochlea in newborns: incidence of trochlear dysplasia and associated risk factors.

This study aimed to describe the femoral groove morphology using ultrasound in children under 6 months, estimate the incidence of trochlear dysplasia, and evaluate associated risk factors. A prospective study included 298 patients who underwent universal ultrasound screening for hip dysplasia [developmental dysplasia of the hip (DDH)] and knee ultrasound. Measurements of sulcus angle (SA), trochlear depth (TD), and trochlear facet asymmetry (TFA) were analyzed. Trochlear dysplasia was considered present if the ASO was ≥159°. Reproducibility was assessed using the intraclass correlation coeffic

Patellar instability factors in isolated medial patellofemoral ligament reconstructions--what does the literature tell us? A systematic review.

BACKGROUND: Many studies have demonstrated good results after medial patellofemoral ligament (MPFL) reconstruction for patients with patellar instability. The applicability of published studies to the clinical decision-making process for the individual patient with patellar instability, however, is not well elucidated. HYPOTHESIS: There is inconsistency in the reporting of preoperative and postoperative variables, which limits the applicability of current studies to patients with patellar instability. STUDY DESIGN: Systematic review. METHODS: A systematic review of the literature was conduc

[Course of recurrent dislocation of the patella, patellar syndrome without dislocation and femoropatellar osteoarthritis].

The frequency of recurrent dislocation of the patella is increased by the young age of onset, the presence of patella alta and permanent subluxation of the patella. The natural history of the patellar syndrome appears to be fairly favourable in everyday activities, but discomfort is often marked in the intermediate term in sports activities. Isolated femoropatellar osteoarthritis rarely causes major functional impairment of gait. Medial or lateral femorotibial osteoarthritis occurs secondarily. Painful cases often present with subchondral anomalies of the lateral patellar facet, lateral tibial

Recovery expectations predict recovery in workers with back pain but not other musculoskeletal conditions.

STUDY DESIGN: Prospective cohort study. OBJECTIVE: We examined whether recovery expectations predict future return-to-work in workers filing injury claims for a variety of musculoskeletal conditions. SUMMARY OF BACKGROUND DATA: Recovery expectations seem to influence recovery and return-to-work after back pain, but their role in other compensable conditions is uncertain. METHODS: All workers' compensation claimants receiving time-loss benefits for a musculoskeletal condition and undergoing return-to-work assessment were enrolled. Claimants completed a work-related recovery expectations ques

Arthroscopy of the knee in children.

Diagnostic arthroscopy was performed in 174 consecutive patients under the age of 18 years. Eight arthroscopies were bilateral. The clinical diagnosis was confirmed in only three of 17 knees in children under aged 14 years and in 44 per cent of the knees in older children. The most commonly disproved diagnoses were torn meniscus and chondromalacia patella. Arthroscopy seldom changed the diagnosis in cases of osteochondritis dissecans. In 38 per cent of the knees, arthroscopy revealed no surgical treatable abnormality, and unnecessary arthrotomy was avoided. Arthroscopy in acute hemarthrosis re

Total Femoral Replacement as a Salvage Procedure for a Periprosthetic Fracture After Multiple Previous Femoral Reconstructions: A Case Report.

BACKGROUND Periprosthetic femoral fractures and complications after joint replacement pose serious therapeutic challenges, especially in older patients with multiple comorbidities. In cases where conventional stabilization and revision procedures fail, implantation of a total femoral replacement (TFR) prosthesis may provide a limb-salvage option. CASE REPORT We present the case of a 77-year-old woman with multiple comorbidities who underwent repeated treatment for fractures and complications involving the left femur over a 20-year period. Due to poor bone quality resulting from multiple previo
