Subtotal esophagectomy with retrosternal gastric tube reconstruction alters upper gastrointestinal anatomy and can create two obstacles to endoscopic retrograde cholangiopancreatography (ERCP): displacement and rotation of the pylorus and proximal duodenum, which impede side-viewing duodenoscopy, and ampullary carcinoma, which may obscure the bile duct orifice and prevent selective cannulation. When endoscopic drainage fails, endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) offers an alternative route for internal biliary decompression. Reports of EUS-HGS performed from within a retr

A 70-year-old male with lung cancer and interstitial pneumonia was diagnosed with ampullary carcinoma, causing obstructive jaundice. After the failure of endoscopic retrograde cholangiopancreatography, endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) was performed with a 7-Fr plastic stent (PS) into the B2 bile duct. Three months later, mild bleeding was observed during stent exchange, but was stopped by stent replacement. The patient developed recurrent cholangitis, and 1 month later, when the PS was removed to add supplementary drainage, massive bleeding occurred from the endosonog

BACKGROUND: While endoscopic ultrasound (EUS) is well established as a standard diagnostic and therapeutic tool in adults, its use in children remains limited. This study assessed the indications, safety, and clinical impact of diagnostic and therapeutic EUS in children with pancreatobiliary and gastrointestinal disorders.
METHODS: We retrospectively analyzed pediatric patients with pancreatobiliary and gastrointestinal disorders who underwent EUS at our institution between January 2022 and June 2025.
RESULTS: Fifty-three EUS procedures were conducted in children with a median age of 9.6 ± 3

We report the use of endoscopic cholecystoduodenostomy in a 6-year-old child to manage postanastomotic stricture of the common bile duct (CBD) secondary to an intraoperative injury sustained during the resection of an abdominal neuroblastoma (NB). The patient was diagnosed with stage M NB, characterized by dissemination to the bone marrow and vertebrae, and MYCN amplification. Following multiple cycles of chemotherapy and subsequent hematopoietic stem cell transplantation, the patient was scheduled for surgical resection. Preoperative imaging identified several image-defined risk factors, incl

It is known that the presence of associated anomalies has become one of the most critical prognostic factor for infants born with oesophageal atresia (OA). Associated VACTERAL group of abnormalities are common, but other associations are rare. Anomalies of the hepatobiliary system are even more rare among other groups. There are only a few such cases in the literature including adults and paediatric population. We describe the findings and management of one infant born with OA with tracheoesophageal fistula (TEF) and Ano rectal malformation (ARM), who was later shown to have features of obstru

A 67-year-old man presented with obstructive jaundice in March 2022 and was diagnosed with borderline resectable pancreatic head cancer. Gemcitabine+nab-paclitaxel(GnP)was started as preoperative chemotherapy, and tumor shrinkage was observed. In August of the same year, he underwent a subtotal stomach-preserving pancreaticoduodenectomy, combined portal vein resection and reconstruction. The pathological evaluation of surgical specimen demonstrated that it was anaplastic pancreatic cancer and he was treated with adjuvant chemotherapy with GnP. In April 2023, a CT scan confirmed that there was

Pancreatic solid pseudopapillary epithelial neoplasm (SPEN) is a rare pancreatic tumor with low-grade malignant potential. They often present in young women in their second and third decade of life, with only a small minority concerning children. It has a good prognosis, with a five-year survival rate of up to 97%. A common hepatic artery (CHA) arising from the superior mesenteric artery (SMA) is a rare occurrence. A 12-year-old girl, admitted with features of obstructive jaundice, was evaluated to have a heterogeneously enhancing mass lesion of size 7.5 × 7.2 cm involving the head of the panc

Surgical clip migration into the common bile duct (CBD) with subsequent stone formation is an exceedingly rare complication following both laparoscopic and open cholecystectomy, with fewer than 100 cases reported in the literature. Herein, we present the case of a 78-year-old female who presented with abdominal pain and dark urine six years after an open cholecystectomy. Her abdominal ultrasonography revealed no abnormalities, with only mild derangements noted in liver function tests. However, computed tomography of the abdomen unveiled a single metallic surgical clip lodged within the CBD, su

BACKGROUND: Liver involvement in adults with acute myeloid leukemia is uncommon. Most of the case reports describe acute liver failure or obstructive jaundice, while acute hepatitis is rarely mentioned. We report a patient with acute myeloid leukemia who presented with clinical, biochemical, and radiological signs of acute hepatitis that totally regressed after chemotherapy.
CASE PRESENTATION: A 38-year-old Caucasian man presented with fever, cough, and mild fatigue. Laboratory workup showed anemia, thrombocytopenia, severe leukocytosis, transaminitis, and hyperbilirubinemia. Imaging of the a

INTRODUCTION: Adenosquamous carcinoma (ASC) is a rare subtype of the conventional adenocarcinoma of the bile duct. The clinico-pathological characteristics of this entity are poorly understood partly due to its rarity.
CASE SUMMARY: A 67-year-old ASA II male presented with obstructive jaundice subsequently complicated by cholangitis. CT abdomen showed dilatation of the intra and extrahepatic biliary tree. Endoscopic retrograde cholangiopancreatography revealed a stricture with a mucosal growth at the ampulla of Vater. He had a pancreaticoduodenectomy and the distal common bile duct tumour ide

BACKGROUND: Situs inversus totalis is a rare anatomical variation of both the thoracic and the abdominal organs. Common bile duct strictures can be caused by malignant and benign diseases as well. 7-18% of the latter ones are 'malignant masquerade' cases, as pre-operative differentiation is difficult.
CASE PRESENTATION: We present the case of a 68y male patient with known situs inversus totalis and a recent onset of obstructive jaundice caused by a malignant behaving common bile duct stricture. Technically difficult endoscopic retrograde cholangiopancreatography, brush cytology, magnetic reso

This study describes an autopsy case of pancreatic/peripancreatic myeloid sarcoma in a 70-year-old man, initially presenting with obstructive jaundice. Pathologically, diffuse infiltration of round cells containing atypical nuclei with marked cleavage was observed mainly in the pancreas head, peripancreatic lymph nodes, spleen, bilateral lung, and bone marrow. Immunohistochemically, the tumor cells were negative for CD20, CD79a, CD3, CD5, c-kit, CD34, and TdT and positive for myeloperoxidase, CD33, CD68, and CD163. Flow cytometry of the peripheral blood showed underexpression of CD11c and aber

We report a case of borderline resectable(BR)pancreatic cancer, which was eligible for R0 resection following preoperative chemotherapy with GEM plus nab-PTX. A 77-year-old woman presented with brown urine and clay-colored stool. After further examination, she was diagnosed with obstructive jaundice due to pancreatic head cancer. Because the tumor was in contact with the region attached to the SMA nerve plexus, she was also diagnosed with BR-A pancreatic cancer. After 6 courses of preoperative GEM plus nab-PTX combination chemotherapy, she underwent subtotal stomach-preservingpancreaticoduoden

Mirizzi syndrome (MS) is a rare complication of chronic cholelithiasis, which is always caused by a calculus in the cystic duct or neck of the gallbladder, resulting in mechanical compression of common bile duct and the gallbladder. It is clinically characterized by abdominal pain, fever, as well as obstructive jaundice. During cholecystectomy, MS is seen as a dangerous adherent and inflammatory tissue in the area of Calot's triangle. In the general population, aberrant right posterior hepatic duct, one of the causes of bile duct injury during duct surgery, is present in 4.8%-8.4% of people. H

Carcinoma of the papilla of Vater is a relatively rare tumor and its coexistence with other primary sporadic cancers is very exceptional. Here we report the case of a 76-year-old man who presented with painless obstructive jaundice, pathologically elevated liver function tests and increased serum levels of carbohydrate antigen 19-9 and carcinoembryonic antigen. Endoscopic retrograde cholangiography revealed a large polypoid mass in the ampulla of Vater. A large tumor in the ascending colon was also incidentally detected by abdominal computed tomography. Endoscopic biopsies from both lesions sh
