Silicone punctal plug extrusion resulting from spontaneous dissection of canalicular mucosa: A clinical and histopathologic report.

PURPOSE: To report on an uncommon, previously unidentified, progressive silicone punctal plug extrusion. DESIGN: Small, retrospective case series. PARTICIPANTS: Five cases of punctal plug extrusion were identified among a series of 424 consecutive implantations. Histopathologic analysis was obtained in one case. One additional referred case was included in this report. RESULTS: The plugs were completely extruded from the punctal lumen 6 to 14 months after insertion after an otherwise uncomplicated postoperative course. All plugs were retained at the punctal margin by an encircling band of v

Alloplastic implants for orbital wall reconstruction.

Nonabsorbable alloplastic implants for orbital wall reconstruction have been widely accepted by surgeons because of their ready availability, stability, and biocompatability. Many complications have arisen with this class of implants because the lack of host tissue integration allows for implant migration, implant extrusion, recurrent hemorrhage, and infection. Porous polyethylene implants provide a welcome alternative as they have the unique properly of supporting tissue ingrowth in vivo. Their semirigid structure provides structural stability when used around the orbit, and their malleabilit

Heavy metal: beware.

A 19-year-old Crouzon's syndrome patient with a history of multiple craniofacial procedures presented with severe bilateral temporal and frontal depressions and metal implants protruding through the scalp in multiple locations. Preoperative analysis revealed an extensive cranial defect that had been reconstructed with multiple large metallic mesh implants. The mesh required complete removal with an autograft cranial reconstruction. We present this case to urge that caution and forethought be exercised when contemplating the use of metallic alloplasts for major craniofacial reconstructions.

Soft lens movement: effects of humidity and hypertonic saline on lens settling.

Hydrogel contact lenses require a settling period before lens fit stabilizes, but the process underlying the initial reduction in lens mobility is poorly understood. Explanations for this phenomenon include base curve steeping with dehydration, expulsion of postlens tear fluid, and osmotic flow of hypotonic tears into the cornea. We conducted two randomized, single-masked interocular comparisons for ten subjects wearing HEMA lenses for 1 h. We investigated potential mechanisms of dehydration-dependent lens tightening, by limiting dehydration in one eye using a high humidity environment (experi

[Reconstruction of the orbit floor using a resorbable polydioxanone (PDS degree) cup. Analysis of a series of 71 cases].

A 71 patients series is presented, concerning primary orbital floor reconstruction by a polydioxanone cup (PDS degree) which has been used for five years. The technical data and surgical procedure are exposed. No infection either intolerance manifestation has been observed until the materiel was resorbed. Two early displacements of the implant are reported, due to a technical fault during the insertion of the cup. Only one late enophtalmy was observed for a patient with a large orbital floor defect. The loss a the rigidity of the polydioxanone seems to be relatively fast (probably 2-3 months)

Early migration following revision total knee arthroplasty with tibial metaphyseal cones: a 2-year prospective RSA cohort study of 25 patients.

BACKGROUND AND PURPOSE: In revision total knee arthroplasty (rTKA), cones can enhance stable fixation in the case of suboptimal metaphyseal bone stock. Combined with stems of sufficient length, cones can offer support and promote osseointegration without graft-related complications. However, their mechanical stability remains underexplored. We aimed to evaluate the stability of tibial rTKA constructs with metaphyseal cones using radiostereometric analysis (RSA) over a 2-year follow-up. METHODS: 25 patients undergoing rTKA with a tibial cone were included. Tantalum markers were placed in the c

Similar 1-year tibial implant migration, but higher continuous migration with cementless fixation: a retrospective RSA study of 990 patients with moderate to severe knee osteoarthritis.

BACKGROUND AND PURPOSE: Aseptic loosening is a major cause of revision surgery. The grade of osteoarthritis (OA) may influence tibial implant migration and the risk of aseptic loosening. We aimed to investigate the association between preoperative OA grade and tibial implant migration in cemented and cementless total and unicompartmental knee arthroplasty. METHODS: We performed a retrospective analysis on a prospective clinical cohort of 990 patients (990 knees). Preoperative OA was graded with the Kellgren-Lawrence (KL) scale and categorized as severe (KL 4) or moderate (KL 1-3). Tibial impl

5-year migration and inducible displacement of the uncemented LCS and ATTUNE rotating platform knee systems: a secondary report of a randomized controlled RSA trial.

BACKGROUND AND PURPOSE: Early migration of the uncemented cruciate-sacrificing rotating platform ATTUNE and Low Contact Stress (LCS) tibial components was classified as at-risk for aseptic loosening rates exceeding 6.5% at 15 years based on recent fixation-specific migration thresholds. In this secondary report of a randomized controlled trial (RCT) we aimed to evaluate whether the 5-year migration, inducible displacement, and the clinical outcome of the ATTUNE components were comparable to those of the LCS. METHODS: Patients from the initial 2-year radiostereometric analysis (RSA) RCT were r

A novel method to manage a dislodged self-expanding transcatheter aortic valve.

Despite advancements in technology, operator experience, and procedural planning, transcatheter aortic valve replacement (TAVR) procedures are complex, and complications remain inevitable. Valve embolization may prove to be fatal and conventional rescue techniques are dependent on the anatomy of the aorta. We describe a case of postimplant embolization of a self-expanding valve during valve-in-valve application where the valve could not be stabilized due to the anatomy of the aorta and a novel technique was utilized to stabilize the valve in the aortic arch using a wire fixed to the left axill

Left ventricular perforation from a dislodged needle migrating via a pulmonary artery branch in an intravenous drug user.

Embolised needles causing injury to the right heart and cardiac tamponade has been reported before in intravenous drug users, but to our knowledge, this is the first reported case of a needle migrating via the pulmonary arterial system to cause perforation of the left ventricle. Appropriate utilisation of imaging modalities such as plain X-ray and point-of-care focused cardiac ultrasound can be vital and life-saving in the emergency setting, and the value of gated multidetector CT as a powerful tool for imaging moving structures is highlighted.

Multiple graft injury due to migrated nonbroken sternal wires 2 years after cardiac surgery.

The migration of sternal wires into vital structures is a rare but potentially life-threatening complication and associated with infection in some cases. While a few cases have been reported the sternal wires were broken in those cases. To our knowledge, this is the first report of multiple, nonbroken, migrated sternal wires stabbing vascular grafts.

Impact of Histopathologic Changes Induced by Polyethylene Glycol Hydrogel Pleural Sealants Used During Transthoracic Biopsy on Lung Cancer Resection Specimen Staging.

Patients undergoing transthoracic needle core lung biopsy (TTNB) are at risk for biopsy-related pneumothorax. Instilling pleural sealant at the pleural puncture site reduces this risk. The impact of histologic changes associated with pleural sealant on assessing the histologic type and pathologic stage in lung cancer resection specimens has not been previously evaluated. All lung cancer resection specimens from 2015 to 2018 in which polyethylene glycol hydrogel pleural sealant was instilled during TTNB were reviewed. Thirty-three cases were identified. TTNB preceded lobectomy by an average of
