Young children occasionally suffer from cataracts. Etiology may be trauma or heredity, but surgical removal and proper contact lens care are now considered imperative for visual development. This paper describes the author's method for fitting and caring for young aphakic children.

During a two-year period a randomly selected age-stratified sample of subjects 65 years old and older living in Gisborne, New Zealand, was examined by an ophthalmologist to investigate visual acuity and the prevalence of the major disorders of vision in old age. A total of 481 subjects (a response rate of 86.2%) completed the study. Scleral buckling was performed on 116 eyes with rhegmatogenous retinal detachments that spared macular function preoperatively. The retina was reattached in 97% of these eyes after one operation. The remaining cases were repaired successfully by a second procedure. Eight percent of the 90 eyes with preoperative vision of 20/30 or better experienced some visual loss postoperatively despite successful retinal reattachment. In 4% of these 90 eyes, the final vision was less than 20/40. Macular pucker was the most common cause of postoperative reduction in vision.

Four female patients with known unplanned filtering blebs following cataract surgery developed endophthalmitis, two weeks to 30 months after being fit with corneal contact lenses. Three patients were wearing hard contact lenses and one wore a soft lens. Significant blepharoconjunctivitis was present and treated in two patients, but intraocular infection was not averted. It was the purpose of this study to determine whether age could be used to predict central endothelial cell count in preoperative cataract patients aged 40 to 90. Three separate series of cell counts on a total of almost 3,000 eyes were performed and analyzed regarding age relationship. Although statistically endothelial cell count appears to decrease with age, the decrease is so small that a large range of cell counts was noted for each age. The results of this study indicate that age is not useful in estimating endothelial cell count in this population.

Chronic aphakic glaucoma can be surgically corrected with the Molteno plastic implant, which consists of a silicone tube attached to a plastic base. The technique described here avoids complications such as hypotony, blocking of the filtering mechanism and fibrosis of the filtration bleb, which can arise in conventional glaucoma operations. Out of 31 eyes with aphakic glaucoma 29 had normal intraocular pressure within six to 72 months without any medication whatsoever.

Because the significance of donor age in penetrating keratoplasty remains controversial, a study was undertaken to analyze the results of keratoplastics performed with older (greater than 70) and younger (less than 55) donor corneas. Fifty-seven eyes with extracapsular surgery and Copeland lenses and the same number of intracapsular cataract extractions without lenses are described. All properly chosen lens cases have 6/12 vision or better. Forty percent required capsulotomy because of 6/12 vision or less. Possible cystoid macular edema occurred in two cases. No endothelial changes were present that were not present preoperatively. Five lenses dislocated. Two glaucomas appeared. There was no retinal separation, endophthalmitis, or lens removal for lens-induced irritation.
