Antithrombin-III plasma activity during and after prolonged use of heparin in unstable angina.

Heparin effectively prevents the complications of unstable angina but disease reactivation has been documented following its discontinuation. To investigate whether this could be related to antithrombin-III depletion, 50 patients with unstable angina had serial determinations of activated partial thromboplastin time and of the plasma levels of heparin, antithrombin-III activity and of the thrombin-antithrombin-III complex before, during and, in a subgroup of 8 patients, 4 hours after heparin discontinuation. Heparin was administered intravenously at therapeutic doses for a mean of 7.6 +/- 4.1

[Therapeutic management of pregnancy complicated by congenital antithrombin III deficiency associated with pregnancy nephropathy].

Thrombo-embolic complications in pregnant women who have congenital antithrombin III deficiency are usually prevented by giving injections of sub-cutaneous heparin from the beginning to the end of pregnancy and with the administration of concentrated doses of antithrombin III (A.T. III) at delivery and in the following days. When we discovered a severe pre-eclampsia at the 28th week of pregnancy in a patient who had congenital deficiency of A.T. III and who had a past history of very severe thrombo-embolic complications we administered concentrated A.T. III as well as continuing the heparin tr

Plasma elimination of antithrombin III (heparin cofactor activity) is accelerated in term newborn infants.

Antithrombin III (AT III) levels are markedly increased in newborn infants following exchange transfusion with adult blood, and subsequently return to pre-exchange values. This transient rise in AT III (heparin cofactor activity), was used to estimate its plasma elimination half-life. AT III activities were measured serially, before and after double-volume exchange transfusions with heparinised blood in newborn infants requiring therapy for severe hyperbilirubinaemia. The plasma elimination half-life of AT III activity was calculated to be 3.9 +/- 1.4 h (mean +/- SEM). Compared with published

Low antithrombin III levels in neonates with idiopathic respiratory distress syndrome: poor prognosis.

Automated microanalytic chromogenic coagulation assays allow serial monitoring of critically ill newborn infants. In this study 84 premature infants [26 healthy prematures and 58 neonates with idiopathic respiratory distress syndrome (IRDS)] were studied daily during the first week of life, to investigate the possible significance of hemostatic abnormalities in IRDS. In neonates with IRDS, coagulation factors II and X, antithrombin III (AT-III), plasminogen, and alpha 2-antiplasmin were significantly lower than control values. Recovery of the initially low AT-III levels was delayed relative to

The effect of total hip replacement and general surgery on antithrombin III in relation to venous thrombosis.

This investigation was undertaken to identify and correlate one factor that makes patients undergoing total hip replacement more susceptible to venous thrombosis and pulmonary embolism than those who have almost any other elective orthopaedic procedure, and to determine why the operation of total hip replacement has proved to be relatively resistant to antithrombotic prophylaxis compared with general surgical procedures. Using the depletion of antithrombin III as a marker of activation of the coagulation system, two groups of patients were compared: twenty-one who were subjected to hip arthrop

The binding of 1-anilino-8-naphthalenesulfonate, heparin, salicylate and caprylate by human antithrombin III.

The binding of 1-anilino-8-naphthalenesulfonate to human antithrombin III was studied by fluorescence enhancement of the fluorophor and fluorescence quenching of the protein emission. Two molecules of 1-anilino-8-naphthalenesulfonate were found to bind per antithrombin molecule with an average dissociation constant of 4.4-10(-5) M. The binding of heparin to antithrombin was studied by ultraviolet difference spectroscopy. The stoichiometry of the heparin binding indicated 1.8 binding sites with an average dissociation constant of 4.3 - 10(-6) M. Further the fluorometric competition experiments

Identifying dynamic antithrombin Ⅲ trajectories to predict clinical outcomes in intra-abdominal sepsis.

BACKGROUND: Intra-abdominal infection (IAI) is the leading cause of sepsis and is often complicated by disseminated intravascular coagulation (DIC), leading to increased mortality. Antithrombin Ⅲ (AT Ⅲ), a crucial endogenous anticoagulant, becomes significantly depleted during sepsis due to increased consumption and reduced synthesis. Its levels are closely associated with disease severity and clinical outcomes. Currently, there is a lack of evidence on the dynamic changes of AT Ⅲ and their relationship with the severity and prognosis of sepsis caused by IAI. METHODS: This was a prospective o

Thrombin-Antithrombin Complex and Fibrinogen Predict Microangiopathic Acute Kidney Injury in Injured Patients.

Acute kidney injury (AKI) develops in 20-40% of critically injured patients and contributes to poor outcomes. In sepsis and critical illness, dysregulated coagulation and fibrinolysis are linked to microangiopathic kidney dysfunction, but similar relationships in trauma are not well characterized. We performed a secondary analysis of a single-center randomized controlled trial of severely injured adult trauma patients. Plasma thrombin-antithrombin (TAT), plasmin-2-antiplasmin (PAP), and fibrinogen were measured within 2 hours of injury and 6 hours later. The primary outcome was AKI, defined b

Site-specific glycosylation of serum antithrombin-III as a potential biomarker for AFP-negative HCC.

Early diagnosis of hepatocellular carcinoma (HCC), particularly in alpha-fetoprotein (AFP)-negative patients, remains a significant challenge. Characterizing N-glycopeptides with site-specific glycan structural information enables a better understanding of the molecular pathogenesis of liver injury and cancer. Here, we performed an unbiased quantitative analysis of N-glycopeptides from serum in patients with hepatitis B virus (HBV)-related liver diseases using a stable isotope labeling-based glycoproteomic approach. Serum samples from patients with liver cirrhosis (LC) and AFP-negative HCC wer

Bridging laboratory assays, genetics, and clinical phenotypes in antithrombin deficiency: Rethinking the diagnostic paradigm.

BACKGROUND: Diagnosis of antithrombin deficiency (ATD), the most severe inherited thrombophilia, commonly relies on functional assays despite their uncertain sensitivity. Moreover, routine characterization of ATD remains uncommon due to limited supporting clinical data. OBJECTIVES: This study aimed to evaluate the diagnostic sensitivity of commercial antithrombin (AT) activity assays and assess clinical differences among ATD types to refine the current diagnostic approach. METHODS: Eighty-eight patients with decreased AT activity and 124 consecutive patients with unprovoked venous thromboemb

Risk stratification according to genotype and effect of thromboprophylaxis on obstetric outcomes in women with antithrombin deficiency.

BACKGROUND: Hereditary antithrombin (AT) deficiency is a risk factor for venous thromboembolism (VTE) and for obstetric complications and warrants thromboprophylaxis in certain situations. AT deficiency is divided into subtypes with different risk profiles. There is limited knowledge on the specific risks in pregnant women depending on the subtypes. OBJECTIVES: The study aim was to investigate whether genotyping pregnant women with AT deficiency could aid in determining complication risk and subsequently help to individualize antepartum and postpartum treatment. METHODS: This retrospective c

Facial Dysmorphism and Severe Vascular Phenotype in TRNT1 Deficiency with Concomitant Antithrombin III Deficiency.

TRNT1 deficiency (SIFD syndrome) is a rare inborn error of immunity characterized by sideroblastic anemia, immunodeficiency, periodic fevers, and developmental delay. We report two Romanian patients with genetically confirmed TRNT1 deficiency presenting with characteristic hematologic and immunologic abnormalities and a distinctive facial dysmorphism. One patient additionally developed severe gastrointestinal ischemia and intracranial hemorrhage in the setting of concomitant hereditary antithrombin III deficiency. These observations expand the phenotypic spectrum associated with TRNT1 deficien

Automated Antithrombin Activity Detection with Whole Capillary Blood Based on Digital Microfluidic Platform.

UNLABELLED: Antithrombin (AT) plays a crucial role in the human anticoagulant system and has extensive clinical applications. However, traditional detection methods often require large sample volumes, complex procedures, and lengthy processing times. METHODS: We integrated digital microfluidics technology with AT detection to develop a point-of-care testing (POCT) device that is user-friendly and fully automated for real-time AT testing. RESULTS: This device allows for automation and enhanced adaptability to various settings, requiring only a minimal sample volume (whole capillary blood), th

Danaparoid failure in heparin-induced thrombocytopenia due to acquired antithrombin deficiency: A case report.

Heparin-induced thrombocytopenia (HIT) is a severe immunological adverse effect of heparin therapy, characterized by thrombocytopenia and unpredictable thromboembolic complications. Rapid discontinuation of heparin and replacement by an alternative anticoagulant such as danaparoid is mandatory. We report the case of a 45-year-old woman with uterine sarcoma and acute HIT, who experienced treatment failure with danaparoid. Despite danaparoid dosage escalation, anti-Xa activity remained subtherapeutic, resulting in clinical deterioration. Acquired antithrombin (AT) deficiency in the context of ca

How to Manage Venous Thromboembolism Risk during Pregnancy in Patients with Inherited Antithrombin Deficiency?

Inherited antithrombin deficiency (ATD) is associated with a high risk of venous thromboembolic complications. Association of ATD with other conditions such as pregnancy obviously increases thromboembolic risk and may require anticoagulant therapy for prevention. Although there are several/heterogenous international guidelines regarding thromboprophylaxis in pregnant patients with ATD, data on anticoagulant prophylaxis in this context are scarce in the literature. Thus, this situation remains a challenge both in the antepartum period and during delivery. Physicians from the French Society of T

Antithrombin deficiency caused by SERPINC1 gene mutation in white matter lesions: A case report.

RATIONALE: White matter lesions (WMLs) are structural changes in the brain that manifest as demyelination in the central nervous system pathologically. Vasogenic WMLs are the most prevalent type, primarily associated with advanced age and cerebrovascular risk factors. Conversely, immunogenic WMLs, typified by multiple sclerosis (MS), are more frequently observed in younger patients. It is crucial to distinguish between these 2 etiologies. Furthermore, in cases where multiple individuals exhibit WMLs within 1 family, genetic testing may offer a significant diagnostic perspective. PATIENT CONCE
