Dual Ureaplasma parvum arthritis: a case report of U. parvum septic arthritis following contralateral reactive arthritis in an immunosuppressed patient.

BACKGROUND: Ureaplasma parvum is usually part of the normal genital flora. Rarely can it cause invasive infections such as genitourinary infections, septic arthritis, or meningitis. CASE PRESENTATION: Here we present the first description of chronic ureterocystitis in a 56-year-old immunocompromised patient, complicated first by reactive arthritis and secondarily by contralateral septic arthritis due to U. parvum infection. U. parvum was detected in synovial fluid and in a urine sample. Treatment consisted of double-J stenting and targeted antibiotic therapy. Evolution showed resolution of ur

Albumin and Gamma-Glutamyl Transferase as Biomarkers for Differentiating Systemic Juvenile Idiopathic Arthritis from Reactive Arthritis.

OBJECTIVE: Systemic juvenile idiopathic arthritis (sJIA) and reactive arthritis (ReA) share overlapping clinical features, posing diagnostic challenges. Early differentiation is critical for treatment decisions but lacks reliable biomarkers. This study aims to identify simple clinical indicators and develop a clinical prediction model to distinguish sJIA from ReA. METHODS: This study retrospectively included clinical data of 397 sJIA patients and 290 ReA patients who attended the Children's Hospital of Chongqing Medical University from 2016-2024. Key predictors were identified by ANOVA, chi-s

HLA antigens in seronegative spondylarthropathies. Reactive arthritis and arthritis in ankylosing spondylitis: relation to gut inflammation.

The prevalence of HLA-A, B, C and DR antigens was determined and compared in 94 patients with reactive arthritis, 54 patients with ankylosing spondylitis (AS), 37 patients with inflammatory bowel disease (IBD) and in 1,010 apparently normal blood donors. The 185 patients all underwent ileocolonoscopy with biopsy of ileum, ileocecal valve and cecum. HLA-B27 was found elevated in the groups with reactive arthritis (48%, chi 2 = 82, p less than 0.0005) and the AS groups (78%, chi 2 = 157, p less than 0.0005), compared to healthy controls. HLA-Bw62 was significantly raised in the patients with rea

Metabolomics: A Viable Approach to Understand the Pathogenesis of Reactive Arthritis.

Reactive arthritis (ReA) is characterized by immune-mediated sterile synovitis brought on by an infection that enters the body through the gastrointestinal or urogenital tracts from a distance. The diseases known as seronegative spondyloarthropathy (SSA) include undifferentiated arthritis (uSpA) and reactive arthritis (ReA). Cytokines are crucial in orchestrating an effective immune response to eliminate bacterial infections, such as those seen in ReA (Reactive Arthritis) conditions. The balance between Th1 and Th2 cytokines is particularly important in determining the outcome of infections as

Bilateral dacryoadenitis: a new addition to the spectrum of reactive arthritis?

Reactive arthritis is a well-known sequel to both dysenteric and urethritic illnesses, classically occurring in combination with conjunctivitis, arthritis, and/or a variety of dermatologic conditions. Dacryoadenitis has not been previously described as part of the spectrum of reactive arthritis. The authors describe a case of biopsy-proven bilateral dacryoadenitis, which developed in the setting of chlamydial urethritis.

Detection of parvovirus in a patient with "reactive arthritis" by in situ hybridization.

We used in situ hybridization to search for the presence of viruses in synovial fluid cell preparations obtained from patients with various forms of knee arthropathies. One patient, presenting with acute reactive arthritis, was found to replicate parvovirus DNA in cells from synovial fluid whereas six other patients with various other forms of arthritis were negative for parvovirus infection. Five patients with osteoarthritis, constituting a control group in which an infectious etiology would not be expected, were consistently negative. In addition, no hybridization was found when synovial cel

[Reactive arthritis after treatment with immune checkpoint inhibitors: A case report].

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have transformed cancer therapy, but immune-related adverse events are common. Reactive arthritis-like syndromes remain rarely reported in this context. CASE REPORT: We report the case of a 73-year-old man with a history of clear cell renal carcinoma who developed conjunctivitis, urethritis, and asymmetric arthritis 48hours after receiving pembrolizumab. Synovial fluid analysis was sterile, the autoimmune panel was negative, and imaging excluded axial spondyloarthritis. Symptoms recurred during glucocorticoids tapering but improved after intra

Clostridioides difficile-related reactive arthritis: A scoping review of the literature.

Reactive arthritis (ReA) is an inflammatory arthritis triggered by preceding infection. While various bacterial pathogens are well-established as causative agents, the role of Clostridioides difficile remains less elucidated. Α scoping review was conducted to evaluate the existing literature on CDI-induced ReA to determine clinical features, management, and disease trajectory. Sixty-one CDI-related ReA cases were identified (61 % male, mean age 38.8 years). HLA-B27 positivity was 64 % (30/47 tested). Prior antibiotic use was reported in 89 % of cases, with arthritis onset 8.5 days post-CDI. Ol

Mendelian randomization analysis of C-reactive protein on septic arthritis risk.

Several observational studies have shown that C-reactive protein (CRP) is an effective biomarker for evaluating septic arthritis (SA). However, the inherent causal relationship of this connection has not yet been clarified. Thus, this study examined the potential causal association between CRP and SA risk using 2-sample Mendelian randomization (MR). The analysis used 2-sample MR, with the exposure and outcome data from the Genome-Wide Association Study Catalog and FinnGen, respectively. Throughout this study, inverse variance weighting has been used as the primary method. To assess the robustn
