OBJECTIVE: To determine differential findings of patent foramen ovales (PFOs) and atrial septal defects (ASDs) demonstrating interatrial shunting on coronary computed tomography (CT) angiography.
METHODS: We retrospectively analyzed 15 cases of patients demonstrating a contrast jet from the left to the right atrium on CT and interatrial shunting on echocardiography. According to the echocardiography results, patients were divided into PFO (n = 10) or ASD (n = 5) groups. We report a modified technique of wire atrial septostomy (WAS) with a reverse transseptal puncture (TSP) in an infant case of pulmonary atresia with intact ventricular septum. A radiofrequency (RF) wire was advanced to the septum through a 4 Fr pigtail catheter hooked on the left side of atrial septum and RF energy was applied while advancing the wire across the septum. Lipomatous hypertrophy of the interatrial septum is a benign cardiac mass characterized by fat accumulation in the interatrial septum. Cardiac arrhythmias including atrial fibrillation, atrial premature contractions and atrioventricular block may occur as a consequence of involvement of the atrial wall and atrioventricular conduction pathways. We describe a patient with lipomatous hypertrophy of the interatrial septum associated with sick sinus syndrome.

Atrial septal aneurysm can be detected by subcostal echocardiography as a bulge of the intermediate interatrial septum, ballooning toward the right atrium. It is generally considered that the development of secundum atrial septal defect (ASDII) or oval fossa defect is the result of excessive resorption of the embryological atrial septum primum, but this does not seem to explain all defects. Lipomatosis of interatrial septum is a rare benign disorder characterized by fat accumulation into interatrial septum. It may cause atrial arrhythmias and hemodynamic complications. Atrial septal pouch is a recently described anatomical entity. We describe webbed left atrial septal pouch (LSP), a new variant in the spectrum of fusion of septum primum and septum secundum. We report an unusual case of an adult who underwent a mitral valve replacement with concomitant excision of the left atrial myxoma and thrombus. Echocardiography showed the presence of a large "thrombus" within the left atrial appendage, body and atrial septum. There was difficulty in trying to distinguish between the atrial thrombus and myxoma due to their morphological similarities. At time of surgery, frozen section confirmed the atrial septal component of the thrombus to be an atrial myxoma and the atrial septum was excised to obtain a clear margin.
