Penile blood gas determination for the evaluation of priapism.

INTRODUCTION: Priapism is a penile erection that is sustained beyond or is unrelated to sexual stimulation. Depending on the etiology of the priapism, there may be low blood flow to the tissue, causing ischemic priapism, or high blood flow, called nonischemic priapism. Ischemic priapism is assessed in part by corporal blood gas analysis and should be treated emergently to preserve tissue health and function. Ischemic priapism is a common morbidity associated with sickle cell disease (SCD). METHODS: A 16-year-old male individual presented at the emergency department with priapism. His medical …

[The effect of preischemic blood sugar concentration on hemodynamics and regional organ blood flow during and following cardiopulmonary resuscitation (CPR) in swine].

UNLABELLED: Blood glucose alterations prior to cerebral ischaemia are associated with poor neurologic outcome, possibly due to extensive lactic acidosis or energy failure. Cerebral effects of hyper- or hypoglycaemia during cardiopulmonary resuscitation (CPR) are less well known. In addition, little information is available concerning cardiac effects of blood glucose alterations. The aim of this study was to evaluate the effects of pre-cardiac-arrest hypo- or hyper-glycaemia compared to normoglycaemia upon haemodynamics, cerebral blood flow (CBF) and metabolism (CMRO2), and regional cardiac blo

Continuous blood gas monitoring in haemodialysis using an electrode inserted in the extracorporeal dialysis circulation.

The aim of this study was to evaluate the usefulness of continuous gas monitoring during haemodialysis using an oxygen- and carbon dioxide-sensitive electrode inserted in the 'arterial' line from the arteriovenous fistula. We investigated 15 patients with chronic renal failure, treated with regular bicarbonate haemodialysis. The PO2 and PCO2 were measured by the new method and compared to conventional blood gas analysis of the 'arterial' blood from the dialysis circulation. We found the in situ gas sensors were stable and there was no systematic difference between gas tensions measured by in s…

Analytical evaluation of a direct ion-selective-based analyser: Still gaps to close.

INTRODUCTION: Discrepancies between electrolyte concentrations determined by blood gas analysers (BGA) and core-lab chemistry analysers may create confusion in clinical practice. This problem is rooted in the different ion-selective electrode (ISE) methodologies that are used. Whilst most available chemistry analysers use indirect ISE, we evaluated the analytical performance of the new automated chemistry analyser Biossays™ E6 (Snibe), equipped with direct ISE, for the determination of sodium (Na+), potassium (K+), chloride (Cl-), ionized calcium (iCa2+) and pH. MATERIALS AND METHODS: Total p

Clinical indicators of blood gas disturbances, elevated L-lactate concentration and other abnormal blood parameters in newborn beef calves.

Blood gas disturbances, commonly resulting from dystocia, are associated with failed transfer of passive immunity, morbidity and mortality in newborn calves. Modified APGAR scores intended to identify compromised calves are not widely adopted due to lack of practicality and inconsistent associations with blood parameters. The objective of this study was to determine clinical indicators of acidemia in newborn beef calves. Blood parameters at 10 min and 24 h after birth were compared to at-birth clinical examination parameters and calving characteristics in 77 commercial beef calves. There were

Hypothermic effects on gas exchange performance of membrane oxygenator and blood coagulation during cardiopulmonary bypass in pigs.

INTRODUCTION: Whether hypothermic cardiopulmonary bypass could attenuate both blood coagulation and platelet activation compared to normothermic cardiopulmonary bypass remains elusive. METHODS: Biocompatibility of a polymer-coated cardiopulmonary bypass circuit was comparatively assessed by plasma proteomics between juvenile pigs undergoing hypothermic (23°C) cardiopulmonary bypass and those undergoing normothermic (37°C) cardiopulmonary bypass (n = 6, respectively). Plasma samples were taken three times: 5 minutes after initiation of cardiopulmonary bypass (T5, before cooling), just before d

Utility of arterial blood gas, CBC, biochemistry and cardiac hormones as evaluation parameters of cardiovascular disease in nonhuman primates.

Cardiovascular disease (CVD) has a tremendous impact on the quality of life of humans. While experimental animals are valuable to medical research as models of human diseases, cardiac systems differ widely across various animal species. Thus, we examined a CVD model in cynomolgus monkeys. Laboratory primates are precious resources, making it imperative that symptoms of diseases and disorders are detected as early as possible. Thus, in this study we comprehensively examined important indicators of CVD in cynomolgus monkeys, including arterial blood gas, complete blood count (CBC), biochemistry

Arterial blood gas control in the upright versus recumbent Asian elephant.

In the elephant, there is concern that lateral recumbency (LR) impairs respiratory muscle and lung function resulting in clinically significant arterial hypoxemia. Using healthy adult female Asian elephants (Elephas maximus, n=6), the hypothesis was tested that, given the O(2) binding characteristics of elephant blood, substantial reductions in arterial O(2) pressure (Pa(O(2))) in LR could be tolerated without lowering arterial O(2) content appreciably. Fifteen minutes of LR decreased Pa(O(2)) from 103+/-2 (upright, U) to 77+/-4 mmHg (P<0.05) and hemoglobin O(2) saturation (U, 97.8+/-0.1, LR,

Tissue pO2 of human brain cortex--method, basic results and effects of pentoxifylline.

A polarographic multiwire surface electrode was used for measurement of local oxygen partial pressure (pO2) on human brain cortex during neurosurgical operations. The two major problems encountered in this application of the electrode involved sterility of the equipment and mounting of the electrode. The described method of sterilization does not alter the electrical properties of the electrode. A special mount was designed to allow free three-dimensional placement of the electrode without exerting pressure on the cortex. Basic results of this technique demonstrated that it is possible to dist

Asplenia syndrome with atypical cardiac anomalies.

Two infants with asplenia and complex cyanotic congenital heart disease showed unusual findings at cardiac catheterization. Each had a patent pulmonary outflow tract and a severe obstructive lesion of the aortic arch. These cases emphasize the heterogeneity of the asplenia-polysplenia syndromes. Pulmonary atresia or critical pulmonic stenosis is not an invariable accompaniment of the asplenia syndrome. Obstructive lesions of the aortic outflow tract, which occur in about 1/3 of patients with the polysplenia syndrome, also can occur with asplenia.

Continuous blood gas monitoring using an in-dwelling optode method: comparison to intermittent arterial blood gas sampling in ECMO patients.

INTRODUCTION: The ability to measure postmembrane arterial blood gases is essential in the management of critically ill neonates treated with extracorporeal membrane oxygenation (ECMO). A new technology using, the Paratrend 7 system (Diametrics Medical, High Wycombe,UK) allows for continuous measurement of pH, PCO(2) and PO(2), and calculates oxygen saturation, bicarbonate, and base excess. OBJECTIVE: To evaluate and compare the results of continuous blood gas measurement using the Paratrend 7 system with a standard system of blood gas analysis in our intensive care unit. Detection of disturbances in pulmonary gas exchanges during exercise from arterialized earlobe PO2.

Blood sampling from the arterialized earlobe is widely used in clinical exercise testing but Fajac et al. (1998) (Eur. Respir. J. 11, 712-715) have shown that arterialized P(O2) Pc(CO2) is not a valid surrogate for Pa(O2). In the present study, in order to detect disturbances in pulmonary gas exchanges during clinical exercise testing from the alveolar-arterial gradient of P(O2) (P[Ai-a](O2)), a correction factor for Pc(O2) was validated from data on a large cohort (107 patients at one or two levels of exercise: 172 pairs of samples). Pulmonary gas exchanges and pH, P(O2), P(CO2), PA(iO2) and

Comparison of early and late time-controlled adaptive ventilation on pulmonary gas exchange in anesthetized horses.

OBJECTIVE: To compare early versus late time-controlled adaptive ventilation (TCAV) for setting airway pressure release ventilation (APRV) on arterial oxygenation in dorsally recumbent anesthetized horses. STUDY DESIGN: A crossover, nonrandomized, experimental study. ANIMALS: Six healthy adult horses. METHODS: Each horse underwent two anesthetics: 1) volume-controlled ventilation (VCV) began for 30 minutes before switching to APRV using the TCAV method (late TCAV, TCAVL); and 2) ventilation began immediately with TCAV (early TCAV, TCAVE). The study lasted 180 minutes, with arterial blood ga

Circulatory Failure During Noninhaled Forms of Cyanide Intoxication.

Our objective was to determine how circulatory failure develops following systemic administration of potassium cyanide (KCN). We used a noninhaled modality of intoxication, wherein the change in breathing pattern would not influence the diffusion of CN into the blood, akin to the effects of ingesting toxic levels of CN. In a group of 300 to 400 g rats, CN-induced coma (CN i.p., 7 mg/kg) produced a central apnea within 2 to 3 min along with a potent and prolonged gasping pattern leading to autoresuscitation in 38% of the animals. Motor deficits and neuronal necrosis were nevertheless observed i

Dependence of oxygen consumption on oxygen delivery in patients with chronic congestive heart failure.

We previously have shown that in patients with adult respiratory distress syndrome (ARDS) oxygen consumption (VO2) is linearly related to oxygen delivery (DO2) below a threshold DO2 of 21 ml/min/kg. To evaluate this relationship in chronic congestive heart failure (CHF), we studied eight patients with chronic CHF at baseline and during treatment with nitroglycerin. The resting DO2 and VO2 were 10.7 +/- 2.3 ml/min/kg and 3.8 +/- 0.87 ml/min/kg, respectively. In our eight patients, we found a significant relationship between changes in VO2 and in DO2 (delta VO2 = 0.16 + 0.34 X delta DO2, r = 0.8

Can Venous Blood Gas Be Used as an Alternative to Arterial Blood Gas in Intubated Patients at Admission to the Emergency Department? A Retrospective Study.

OBJECTIVE: Blood gas analysis plays an important role in both diagnosis and subsequent treatment of critically ill patients in the emergency department and the ICU. Historically, arterial blood is predominantly used for blood gas analysis. The puncture is painful and complications may occur. The purpose of the present study was to evaluate the agreement between arterial and venous blood gas analysis and whether the sole use of venous blood gas analysis would have changed therapy. METHODS: Adult patients who were intubated in the field and received an arterial and venous blood gas analysis wit…

Effects of mean arterial pressure and needle size on arterial sampler filling time.

BACKGROUND: Arterial blood sampling is subject to numerous pre-analytical errors, one of which is inadvertent venous blood sampling. Especially when assessing oxygenation and titrating ventilation, accidental venous blood sampling may lead to inappropriate respiratory care and repeated percutaneous punctures. OBJECTIVE: To determine the effects of mean systemic blood pressure and needle size on vented arterial sampler filling times, to distinguish venous and arterial sampling. METHODS: We constructed an extracorporeal laboratory model to circulate whole blood at 4 L/min. We used hemostats to
