Jin bu huan toxicity in adults--Los Angeles, 1993.

Jin Bu Huan (JBH) is a traditional Chinese herbal product used as a sedative and analgesic. During 1993, public health and health-care providers in Colorado reported three children with unintentional overdoses of JBH that caused central nervous system and respiratory depression with rapid onset of life-threatening bradycardia. Subsequently, the first cases of acute hepatitis attributed to use of JBH were diagnosed in three women in Los Angeles during July and August 1993. Patients 1 and 2 were referred to a Los Angeles hepatology clinic by their physicians; patient 3 was identified by patient

[Medical complications connected with the use of drugs].

Aside from disease induced by the direct pharmacological effect of heroin or cocaine, the occurrence of several medical complications not directly related to the drug itself is becoming an increasingly serious problem. In addition to the well-known occurrence of infective diseases, including AIDS, related to the i.v. use of heroin, heroin addicts also seem significantly more at risk for chronic nephropathy. Amyloidotic nephropathy is especially frequent in addicts who use heroin by subcutaneous route (skin popping); it seems to be mediated by an immunologic mechanism. Amyloidotic nephropathy i

Medroxyprogesterone acetate as a contraceptive for female drug addicts.

A study of 69 female drug addicts is presented who have been given depomedroxyprogesterone acetate (DMPA) as a contraceptive for 1 680 woman-months. No pregnancies were reported during the treatment period with DMPA. After stopping treatment, 60 pregnancies with 43 births were recorded, 12 with low birth weight. In 3 cases pregnancy ended with spontaneous abortion, and in 14 cases legal abortion were performed. During the study, regular liver function tests revealed no hepatotoxic effects from DMPA in this group of patients where one would expect a high frequency of a serum hepatitis. Neither

Major system toxicities and side effects of anticonvulsants.

The use of anticonvulsants is expanding in the treatment of bipolar and related disorders. Although they have characteristics in common, the anticonvulsants currently used are quite diverse and vary in their spectrum of activity, quality of supporting evidence, and organ toxicities. Common side effects of anticonvulsants that can limit tolerability but are not physiologically severe include sedation and other cognitive impairments, tremor, and gastrointestinal side effects. Possibly less common, but of more physiologic significance, are effects on body weight and metabolism and dose-related he

Photoletter to the editor: Acquired idiopathic generalized anhidrosis.

Anhidrosis is a failure in sweat production in response to physiological thermal or chemical stimuli. Acquired idiopathic generalized anhidrosis is a rare disorder without sweat gland pathology and without neurologic symptoms. Most cases have been reported from Far East. We report a case of a 58-year-old Caucasian male who suffered from heat intolerance, heat-induced cutaneous burning and failed to sweat even in sauna for five years. A skin biopsy disclosed no pathologies. He had no neurologic disorders. The diagnosis of acquired idiopathic generalized anhidrosis was confirmed and treatment wi

Intestinal gases: influence on gut disorders and the role of dietary manipulations.

The inner workings of the intestines, in which the body and microbiome intersect to influence gut function and systemic health, remain elusive. Carbon dioxide, hydrogen, methane and hydrogen sulfide, as well as a variety of trace gases, are generated by the chemical interactions and microbiota within the gut. Profiling of these intestinal gases and their responses to dietary changes can reveal the products and functions of the gut microbiota and their influence on human health. Indeed, different tools for measuring these intestinal gases have been developed, including newly developed gas-sensi

Phthalates and uterine disorders.

Humans are ubiquitously exposed to environmental endocrine disrupting chemicals such as phthalates. Phthalates can migrate out of products and enter the human body through ingestion, inhalation, or dermal application, can have potential estrogenic/antiestrogenic and/or androgenic/antiandrogenic activity, and are involved in many diseases. As a female reproductive organ that is regulated by hormones such as estrogen, progesterone and androgen, the uterus can develop several disorders such as leiomyoma, endometriosis and abnormal bleeding. In this review, we summarize the hormone-like activities

Stop-codon read-through for patients affected by a lysosomal storage disorder.

Lysosomal storage disorders are a group of inherited diseases that can result in severe and progressive pathology due to a specific lysosomal dysfunction. Current treatment strategies include bone-marrow transplantation, substrate reduction, chemical-chaperone and enzyme-replacement therapy. However, each of these treatments has its limitations. Enhanced stop-codon read-through is a potential alternative or adjunct therapeutic strategy for treating lysosomal-storage-disorder patients. Premature stop-codon mutations have been identified in a large cohort of patients with a lysosomal storage dis

A case of occupational hypersensitivity pneumonitis associated with trichloroethylene.

Trichloroethylene (TCE) is a toxic chemical commonly used as a degreasing agent, and it is usually found in a colorless or blue liquid form. TCE has a sweet, chloroform-like odor, and this volatile chlorinated organic chemical can cause toxic hepatitis, neurophysiological disorders, skin disorders, and hypersensitivity syndromes. However, the hypersensitivity pneumonitis (HP) attributed to TCE has rarely been reported. We hereby describe a case of HP associated with TCE in a 29-year-old man who was employed as a lead welder at a computer repair center. He was installing the capacitors on compu

Neuroleptic-induced priapism, hepatotoxicity and subsequent impotence in a patient with depressive psychosis.

A 59-year-old man experienced priapism five days after taking neuroleptics and abnormal liver function after 28 days. These side-effects are possibly explained as hypersensitivity reactions to the drug.

Structural basis for agonist and heat activation of nociceptor TRPM3.

Detecting noxious heat is vital for survival, triggering protective pain responses. The TRPM3 channel is a key nociceptor and a promising therapeutic target for pain and neurological disorders. Here we show that the rabbit TRPM3 is intrinsically dynamic, with its intracellular domain (ICD) sampling both resting and activated states, but favoring the resting state in the absence of stimulation. We reveal that heat and the synthetic agonist CIM0216 shift the equilibrium toward activation by inducing a similar ICD rearrangement. Mutations that facilitate ICD movement enhance sensitivity to both t

Animal models for hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) represents ~90% of all cases of primary liver cancer and occurs predominantly in patients with underlying chronic liver disease and cirrhosis. Establishing appropriate animal models for HCC is required for basic and translational studies, especially the models that can recapitulate one of the human disease settings. Current animal models can be categorized as chemically-induced, genetically-engineered, xenograft, or a combination of these with each other or with a metabolic insult. A single approach to resemble human HCC in animals is not sufficient. Combining pa

Inhibitory Effects of Emodin, Thymol, and Astragalin on Leptospira interrogans-Induced Inflammatory Response in the Uterine and Endometrium Epithelial Cells of Mice.

Leptospirosis is a systemic infection that causes, among others, acute kidney injury, acute liver disease, muscle pain, vasculitis, bleeding disorders, and reproductive loss. In an effort to reduce uterine inflammatory responses induced by Leptospira, we evaluated the anti-inflammation effects of emodin, thymol, and astragalin in a mouse model. Our results showed that treatment with emodin, thymol, and astragalin alleviated uterine inflammation induced by leptospira infection via suppression of pro-inflammatory cytokine expression and prevented tissue damage. Furthermore, we used primary endom

A metabolomics study unravels the hepatotoxic mechanism of diuron in zebrafish: Disruption of glutathione synthesis and mitochondrial energy metabolism.

The liver plays a central role in xenobiotic metabolism and is consequently highly vulnerable to chemical-induced injury. Nevertheless, the mechanisms underlying diuron-induced hepatotoxicity remain poorly understood. Zebrafish (Danio rerio) were exposed to diuron at concentrations of 50 and 500 μg/L for 21 days, with subsequent analysis of the induced hepatotoxicity employing a combination of physiological, biochemical, and metabolomic techniques. Results showed that diuron significantly bioaccumulated in zebrafish, with bioconcentration factor (BCF) values ranging from 14.0 to 40.49 L/kg. Ti

Development of novel azabenzofuran TRPA1 antagonists as in vivo tools.

The transient receptor potential ankyrin 1 (TRPA1) channel is activated by noxious stimuli including chemical irritants and endogenous inflammatory mediators. Antagonists of this channel are currently being investigated for use as therapeutic agents for treating pain, airway disorders, and itch. A novel azabenzofuran series was developed that demonstrated in vitro inhibition of allyl isothiocyanate (AITC)-induced (45)Ca(2+) uptake with nanomolar potencies against both human and rat TRPA1. From this series, compound 10 demonstrated in vivo target coverage in an AITC-induced flinching model in r

[Chlorofluorocarbons--a new environmental health problem].

A review of health problems caused by the use of chlorofluorocarbons--chemical substances of widespread and varied applications is presented. There is a recent evidence that fully halogenated chlorofluorocarbons are air pollutants inducing hazardous health effects. Direct effects of chlorofluorocarbons on respiratory and cardiac functions are described among which some with lethal outcome. Recent data are given and models described forecasting indirect effects of chlorofluorocarbons induced by their ozone depleting reactions in the stratosphere which bring about a higher exposure of the popula

Multifactorial Genetic and Environmental Hedgehog Pathway Disruption Sensitizes Embryos to Alcohol-Induced Craniofacial Defects.

BACKGROUND: Prenatal alcohol exposure (PAE) is perhaps the most common environmental cause of human birth defects. These exposures cause a range of structural and neurological defects, including facial dysmorphologies, collectively known as fetal alcohol spectrum disorders (FASD). While PAE causes FASD, phenotypic outcomes vary widely. It is thought that multifactorial genetic and environmental interactions modify the effects of PAE. However, little is known of the nature of these modifiers. Disruption of the Hedgehog (Hh) signaling pathway has been suggested as a modifier of ethanol teratogen

Milk-derived extracellular vesicles protect intestinal barrier integrity in the gut-liver axis.

Milk-derived extracellular vesicles (mEVs) have been proposed as a potential nanomedicine for intestinal disorders; however, their impact on intestinal barrier integrity in gut inflammation and associated metabolic diseases has not been explored yet. Here, mEVs derived from bovine and human breast milk exert similar protective effects on epithelial tight junction functionality in vitro, survive harsh gastrointestinal conditions ex vivo, and reach the colon in vivo. Oral administration of mEVs restores gut barrier integrity at multiple levels, including mucus, epithelial, and immune barriers, a
