Automatic rectum limit detection by anatomical markers correlation.

Several diseases take place at the end of the digestive system. Many of them can be diagnosed by means of different medical imaging modalities together with computer aided detection (CAD) systems. These CAD systems mainly focus on the complete segmentation of the digestive tube. However, the detection of limits between different sections could provide important information to these systems. In this paper we present an automatic method for detecting the rectum and sigmoid colon limit using a novel global curvature analysis over the centerline of the segmented digestive tube in different imaging

Visualization of Lumbar Artery Variations by Contrast-Enhanced Multi-detector Row Computed Tomography.

PURPOSE: This study was designed to identify the anatomical variations of lumbar arteries on contrast-enhanced multi-detector row computed tomography (MDCT). MATERIALS AND METHODS: Consecutive 100 colon cancer patients underwent preoperative 3-dimensional navigation studies, which include CT colonography (CTC) and CT angiography (CTA) for evaluation of anatomical relationship between the colon cancer and mesenteric vasculatures. After exclusion of inappropriate cases, 84 cases (33 women and 51 men; mean age, 64 years) were finally analyzed. The visualization of lumbar arteries from the CTA wa

Evaluation of submucosal lesions of the large intestine: part 2. Nonneoplastic causes.

Various nonneoplastic entities may manifest as submucosal abnormalities at colorectal evaluation, and it may be difficult to distinguish between those with an intramural origin and those with an extramural origin on the basis of optical colonoscopy alone. Cross-sectional radiologic imaging, which allows evaluation of the entire bowel wall and the surrounding tissues, plays an important role in the localization and characterization of these abnormalities. However, some superficial submucosal lesions that are initially detected at computed tomographic colonography or barium enema studies may be

CTC technique: methods to ensure an optimal exam.

CT colonography (CTC) has demonstrated equivalent accuracy to optical colonoscopy in the detection of clinically relevant polyps and tumors but this is only possible when technique is optimized. The two most important features of a high-quality CTC are a well-prepared colon and a distended colon. This article will discuss the dietary, bowel preparation, and fecal/fluid tagging options to best prepare the colon. Strategies to optimally distend the colon will also be discussed. CT scan techniques including patient positioning and radiation dose optimization will be reviewed. With proper techniqu

Preoperative T staging using CT colonography with multiplanar reconstruction for very low rectal cancer.

BACKGROUND: Preoperative T staging of lower rectal cancer is an important criterion for selecting intersphincteric resection (ISR) or abdominoperineal resection (APR) as well as selecting neoadjuvant therapy. The aim of this study was to evaluate the accuracy of preoperative T staging using CT colonography (CTC) with multiplanar reconstruction (MPR), in which with the newest workstation the images can be analyzed with a slice thickness of 0.5 mm. METHODS: Between 2011 and 2013, 45 consecutive patients with very low rectal adenocarcinoma underwent CTC with MPR. The accuracy of preoperative T s

Detection of High-Risk Sessile Serrated Lesions: Multitarget Stool DNA Versus CT Colonography.

BACKGROUND. The serrated pathway for colorectal cancer (CRC) development is increasingly recognized. Sessile serrated lesions (SSLs) that are large (≥ 10 mm) and/or have dysplasia (i.e., high-risk SSLs) are at higher risk of progression to CRC. Detection of SSLs is challenging given their predominantly flat and right-sided location. The yield of noninvasive screening tests for detection of high-risk SSLs is unclear. OBJECTIVE. The aim of this study was to compare noninvasive screening detection of high-risk SSLs between the multitarget stool DNA (mt-sDNA) test and CT colonography (CTC). METHOD

In vivo MRI virtual colonography in a mouse model of colon cancer.

We have developed a reliable noninvasive method for monitoring colonic tumors and mucosal inflammation in a mouse model of colon cancer using magnetic resonance colonography (MRC). After a mild cleansing enema, the colon is filled with Fluorinert, a perfluorinated liquid that does not produce a proton MR signal. The mouse is placed in a dedicated volume MR receiver coil, and high-resolution images are acquired in three planes. The Fluorinert enema distends the mouse colon, creating an artifact-free black homogeneous background, allowing clear delineation of the inflamed colonic wall and visual

Atypical abdominal pain: post-traumatic transverse colon stricture.

A driver presented to the emergency department 1 day after an accident driving his excavator with abdominal pain and vomiting. He was admitted to the surgical ward 2 days later, after reattending. A CT scan revealed wall thickening and oedema in the transverse colon. This was supported by a subsequent CT virtual colonoscopy which raised the suspicion of neoplasia. A follow-up colonoscopy was not carried further than the transverse colon due to an indurated, tight stricture. Biopsies from that area showed ulceration and inflammatory changes non-specific for ischaemia, drug-induced changes or in

Implications of new colorectal cancer screening technologies for primary care practice.

Colorectal cancer (CRC) screening reduces the risk of CRC mortality but is currently not well utilized, with adherence only 50% in the eligible U.S. population and rates that lag behind those for breast and cervical cancer. The primary care physician has the pivotal role of facilitating patient adherence to CRC screening by informed choice of the screening tests, follow up of positive tests, and coordination of medical resources when diagnostic intervention is required. Consequently, the primary care setting is where significant improvements can be made in CRC screening adherence. This article

Virtual colonoscopy training and accreditation: a national survey of radiologist experience and attitudes in the UK.

AIM: Expert consensus recommends directed training and possibly in the future, formal accreditation before independent virtual colonoscopy (VC) reporting. We surveyed radiologists' experience of VC training, compared with barium enema, and assessed attitudes towards accreditation. MATERIALS AND METHODS: A questionnaire was sent to 78 consultant radiologists from 72 centres (65 National Health Service hospitals; seven independent primary screening centres) offering a VC service. RESULTS: Fifty-four (69%) eligible radiologists responded. They had interpreted 18,152 examinations (range 3-1500)

CT colonography in a Korean population with a high residue diet: comparison between wet and dry preparations.

AIM: To compare wet and dry preparation methods for computed tomography colonography (CTC) in terms of preparation quality, interpretation time, and diagnostic performance for polyp detection in a population with a high residue diet. MATERIALS AND METHODS: Eighty-six patients were divided into two groups. Group 1 (n=24) received a wet preparation of 4l polyethylene glycol (PEG) solution, and group 2 (n=62) received a dry preparation of phosphor-soda. Abnormal findings, including polyps, and the time required to interpret the CTC images in both groups were documented by a radiologist. CTC find

Magnetic resonance colonography for the detection of colorectal neoplasia in asymptomatic adults.

BACKGROUND & AIMS: Colonoscopy is the preferred screening test for colorectal neoplasia; the fecal occult blood test (FOBT) detects neoplasias with low levels of sensitivity. Computed tomographic colonography detects neoplasias with high levels of sensitivity but involves exposure to radiation. We investigated whether magnetic resonance colonography (MRC) can be used to screen for colorectal adenomas and cancers. METHODS: We analyzed data from 286 asymptomatic adults (40-82 years old) who underwent 3 Tesla MRC and colonoscopic examinations on the same day. FOBT was performed before bowel prep

Diagnostic performance of CT colonography in a population with high prevalence of large bowel disease.

OBJECTIVE: This study examined the relationship between psychopathy and subtypes of aggression and firearm violence among a high-risk, community-based sample of adults. Specifically, it assessed whether the four-facet model of psychopathy (interpersonal, affective, lifestyle, and antisocial) was differentially associated with reactive and proactive aggression and reactive and proactive gun violence. Additionally, a confirmatory factor analysis (CFA) was conducted to evaluate the factor structure of the Self-Report Psychopathy Short Form (SRP-SF) in this population. METHOD: A total of 458 viol

Ascending colon rotation following patient positional change during CT colonography: a potential pitfall in interpretation.

OBJECTIVE: To investigate the degree and pattern of ascending colonic rotation as patients moved from supine to prone positions during CTC. METHODS: A search of our CTC and colonoscopy database found 37 patients (43 eligible lesions) who fulfilled the following criteria: colonoscopy-proven sessile polyps ≥ 6 mm in the straight mid-ascending colon, lesion visualisation in both supine and prone CTC, and optimal colonic distension. A coordinate system was developed to designate the polyp radial location (°) along the luminal circumference, unaffected by rotation of the torso. The degree/directio
