Incidence and Outcomes of Surgical Bailout for Transcatheter Aortic Valve Replacement.

OBJECTIVES: Intraprocedural complications of transcatheter aortic valve replacement (TAVR) continue to necessitate conversion to emergent open cardiac surgery. The need for on-site surgical backup remains debated. This study investigates the current incidence and outcomes of surgical conversions during TAVR in the United States. METHODS: We queried the National Inpatient Sample Database to identify patients aged 18 and older who underwent percutaneous TAVR as the primary procedure between 2017 and 2021. Conversion was defined as any open cardiac surgery performed within 1 day following TAVR.

Laparoscopic treatment of intestinal malrotation in neonates and infants: retrospective study.

BACKGROUND: Intestinal malrotation in neonates or infants may require urgent surgical treatment, especially when volvulus and vascular compromise of the midgut are suspected. Successful laparoscopic management of malrotation has been described in a number of case reports. It remains unclear, however, whether laparoscopy for the treatment of malrotation has a success rate equal to that of open surgery and what relative risks exist in terms of conversion and redo surgery in larger numbers of patients. This report describes a retrospective analysis of the clinical outcome for 45 children who unde

Superiority of robotic over laparoscopic distal pancreatectomy in surgical outcomes evaluated by textbook outcome.

BACKGROUND: The comparative advantages of robot-assisted distal pancreatectomy (R-DP) over laparoscopic distal pancreatectomy (L-DP) remain to be fully established. This study aimed to evaluate the achievement of textbook outcomes (TO), a composite metric reflecting optimal perioperative results, in patients undergoing R-DP versus L-DP. METHODS: A retrospective analysis was conducted in 180 patients who underwent minimally invasive distal pancreatectomy between February 2011 and July 2025, including 116 L-DP and 64 R-DP cases. TO was defined as the absence of conversion to open surgery, clini

Comparative outcomes of robotic versus laparoscopic total proctocolectomy.

Total proctocolectomy (TPC) with end ileostomy is performed for inflammatory bowel disease (IBD) or diffuse polyposis/malignancy, but evidence regarding robotic versus laparoscopic TPC remains limited. We hypothesized that robotic TPC is associated with lower conversion rates. We queried the National Surgical Quality Improvement Program's proctectomy database (2016-2023) for adult patients undergoing elective TPC for IBD or polyposis/malignancy. Cases were categorized by intended approach (robotic versus laparoscopic). Conversion to open surgery served as the primary endpoint, with operative t

ArtiSential® vs. straight-stick in laparoscopic left-sided colorectal resection: a prospective randomized study.

BACKGROUND: This prospective randomized study compared the efficacy of articulated ArtiSential® instruments with that of conventional straight laparoscopic instruments in minimally invasive left-sided colorectal resections. The primary endpoint was conversion to open laparotomy. Secondary endpoints included intraoperative and postoperative complications, estimated blood loss, operating time, lymph node yield, and length of hospital stay. METHODS: 78 patients (37 males) were randomized to conventional straight instruments (n = 40) or ArtiSential® instruments (n = 38). Categorical variables wer

Perioperative Outcomes and Complications of Laparoscopic Cholecystectomy in End-Stage Renal Disease Patients: A Prospective Comparative Study.

BACKGROUND: End-stage renal disease (ESRD) patients have increased gallstone disease prevalence and unique perioperative challenges. In ESRD patients awaiting renal transplantation, asymptomatic gallstones identified during pre-transplant evaluation carry additional clinical significance given the disproportionate risk of post-transplant biliary emergencies. Limited literature exists regarding laparoscopic cholecystectomy (LC) outcomes in this population. METHODS: True prospective observational study, CTRI-registered (CTRI/2016/12/007489) was conducted from December 2016 to March 2020 at a te

Factors associated with and impact of open conversion on the outcomes of minimally invasive left lateral sectionectomies: An international multicenter study.

BACKGROUND: Despite the rapid advances that minimally invasive liver resection has gained in recent decades, open conversion is still inevitable in some circumstances. In this study, we aimed to determine the risk factors for open conversion after minimally invasive left lateral sectionectomy, and its impact on perioperative outcomes. METHODS: This is a post hoc analysis of 2,445 of 2,678 patients who underwent minimally invasive left lateral sectionectomy at 45 international centers between 2004 and 2020. Factors related to open conversion were analyzed via univariate and multivariate analys

Outcomes after laparoscopic anti-reflux surgery related to obesity: A systematic review and meta-analysis.

BACKGROUND: Laparoscopic Anti-Reflux Surgery (LARS) is an established alternative treatment to pharmacological therapy for patients with Gastro Osophageal Reflux Disease (GORD), yet its safety and efficacy in obese patients is controversial. A systematic review and meta-analysis was performed to compare LARS related to obesity. METHODS: Embase, MEDLINE and the Cochrane Library (January 1970 to July 2017) were searched for studies reporting clinical outcomes of LARS in patient cohorts stratified by Body Mass Index (BMI). Data was grouped according to BMI, <30 kg/m2 (non-obese) and ≥30 kg/m2 (o

Conversion during laparoscopic anterior resection for rectal cancer with a congenital solitary pelvic kidney: A case report.

A 54-year-old female patient was hospitalized with a chief complaint of anal discomfort. Based on biopsy results, she was diagnosed with highly differentiated adenocarcinoma, and colonoscopy findings indicated a type 3 rectal tumor. We observed a right pelvic kidney on enhanced abdominal CT. We began a laparoscopic operation but converted to an open operation after resecting the right pelvic renal artery by mistake. After low anterior resection, urologists performed angioplasty of the right renal pelvic artery. The patient was discharged on postoperative day 16, after the preservation of right

Impact of obesity and associated diseases on outcome after laparoscopic cholecystectomy.

Obesity is a risk factor for operative treatment. This study examined the impact of obesity and associated comorbidities on complications after laparoscopic cholecystectomy (LC). Altogether, 1581 consecutive patients with symptomatic gallstones underwent LC between the years 1995 and 2008. Preoperative data and operative outcome of the 437 obese patients [302 with body mass index (BMI) 30 to 35 kg/m² and 135 with BMI ≥ 35.1 kg/m²] and 1144 nonobese controls (BMI ≤ 29.9 kg/m²) undergoing LC were compared. The impact of obesity, diabetes, cholecystitis, coronary heart disease, pulmonary disease,

The optimal timing of laparoscopic cholecystectomy for acute cholecystitis according to symptom onset and patient admission: a meta-analysis of randomised controlled trials.

BACKGROUND: Acute cholecystitis (AC) is a common surgical emergency, and laparoscopic cholecystectomy (LC) remains its gold standard treatment. However, the optimal timing of LC, whether early during the index admission or delayed after conservative management, remains debated. Variability in defining "early" surgery, whether based on symptom onset or hospital admission, further complicates the interpretation of existing evidence. METHODS: This systematic review and meta-analysis followed PRISMA guidelines and was registered on PROSPERO (CRD420251081587). Only randomised controlled trials (RC

Standardization of single surgeon robotic pancreatoduodenectomy: lessons from 2 decades of stepwise robotic HPB surgery.

PURPOSE: The robotic approach has been increasingly used for complex pancreatic surgery, including Pancreatoduodenectomy (PD), yet variability in techniques limits reproducibility. This dynamic manuscript shows a standardized operative sequence for a single surgeon (without the need for a surgeon at the bedside) for robotic PD. METHODS: We retrospectively reviewed consecutive patients who underwent RPDs between January 2020 and July 2025, representing the plateau phase of a robotic program initiated in 2012. Demographic, operative, and postoperative variables were collected from a prospective

Multisociety research collaboration: timing of cholecystectomy following cholecystostomy drainage for acute cholecystitis.

BACKGROUND: Patients with acute cholecystitis (AC) presenting with unfavorable systemic or local conditions are often managed with percutaneous cholecystostomy (PC) as a temporary measure. The clinical outcomes of interval cholecystectomy following PC remain unclear. The aim of the study was to identify the association between the timing of cholecystectomy following PC for AC and perioperative complication rates at interval cholecystectomy. We hypothesized that there would be a specific time interval to cholecystectomy associated with lower risk for adverse events. METHODS: This was a retrosp
