[Eosinophilic Gastrointestinal Diseases: Diving beyond the Tip of the Iceberg].

Eosinophilic Gastrointestinal diseases (EGID) are group of chronic inflammatory diseases of the gastrointestinal tract, characterized by infiltration of eosinophils in its layers. These diseases can affect any organ in the GI tract, and can be classified to subgroups according to the anatomical organ involved: Esophagus - Esoniphilic Esophagitis (EoE), Stomach - Eosinophilic Gastritis (EoG), Small Intestine - Eosinophilic Enteritis (EoN) and Colon - Eosinophilic Colitis (EoC). The pathogenesis is not fully understood, but it is likely to involve a combined interaction between hereditary and en

Diving Medicine: An Exciting Journey Through Time and Future Prospects.

Humans, led by their eternal wish to explore the unknown, have always wanted to perfect their diving skills and conquer the sea world. The adverse conditions experienced by divers brought about medical problems and a new field of medicine. Diving medicine serves the identification, treatment, and precautions against illnesses that are related to diving activities. While the development of diving equipment is advancing, divers have had the chance to reach greater depths for a longer time. Along with this success, a novel medical condition under the term 'decompression illness' (DCI) was introdu…

Retrospective analysis of challenging cases for medical examiners of diving.

INTRODUCTION: Assessing a diver's fitness to dive enhances diving safety, with medical examiners of diving (MED) being entrusted with this responsibility. However, the effectiveness of MED training in preparing physicians for this task remains underexplored. In the Netherlands, where any physician can pursue MED qualification, challenging cases can be presented to a board of experts. METHODS: This retrospective analysis included all cases presented to a board of experts in the period 2013-2023. Aside from baseline information, cases were coded using the International Classification of Disease…

Scuba diving-related fatalities in New Zealand, 2007 to 2016.

INTRODUCTION: New Zealand (NZ) diving-related fatalities have been reported since the 1960s. The aim is to identify contributing risk factors, including medical, and to inform appropriate preventative strategies. METHODS: NZ scuba diving fatalities from 2007 to 2016 were searched from multiple sources - the National Coronial Information System (NCIS); the NZ Chief Coroner's office; Water Safety NZ Drownbase™ and the NZ Police National Dive Squad records. For inclusion, a victim must have been wearing a scuba set (which may include a rebreather). A key word search of the NCIS was made and the …

Haemoptysis following shallow breath-hold diving in a cannabis user.

We report the first case of a healthy 24-year-old male with a 6-year history of regular cannabis use, who presented with haemoptysis after a shallow 3 m breath-hold dive. Blood investigations showed mild neutrophilia. CT thorax revealed focal ground-glass changes in the superior segment of the lower lobe. With a suspicion of pneumonia, oral antibiotics were initiated to poor effect. Through bronchoscopic visualisation and lavage, a diagnosis of diffuse alveolar haemorrhage was established. The clinical course was benign with resolution of symptoms and changes on CT thorax within 6 weeks of sto

Diving Response in Rats: Role of the Subthalamic Vasodilator Area.

Diving response (DR) is a powerful integrative response targeted toward survival of the hypoxic/anoxic conditions. Being present in all animals and humans, it allows to survive adverse conditions like diving. Earlier, we discovered that forehead stimulation affords neuroprotective effect, decreasing infarction volume triggered by permanent occlusion of the middle cerebral artery in rats. We hypothesized that cold stimulation of the forehead induces DR in rats, which, in turn, exerts neuroprotection. We compared autonomic [AP, heart rate (HR), cerebral blood flow (CBF)] and EEG responses to the…

Insulin-dependent diabetes mellitus and recreational scuba diving in Australia.

Dive medicine bodies worldwide recognise that, with comprehensive screening and careful management, people with insulin-dependent diabetes (IDDM) can dive safely. Despite this, people with IDDM in Australia are generally denied access to dive training, an out-dated status quo that is not acceptable to the Australian diabetes community. This paper reflects upon the important advocacy work that has been done to progress this issue, and what is still required to open up access and bring Australia into line with more flexible and supportive international standards.

Voluntary apnea evokes diving responses in obstructive sleep apnea patients.

PURPOSE: Two potentially protective responses to apnea were studied in obstructive sleep apnea (OSA) patients; the diving response and the increase in Hb concentration [Hb] via spleen contraction. METHODS: Eight OSA patients and ten healthy controls performed apneas in air (A) and apneas with facial immersion in 15 °C water (FIA) after inspiration and without prior hyperventilation. In each condition, subjects performed three apneas of maximal voluntary duration spaced by 2 min of rest. Cardiorespiratory parameters were measured non-invasively, and venous blood samples for [Hb] analysis were …

Provisional report on diving-related fatalities in Australian waters 2008.

INTRODUCTION: An individual case review of diving-related deaths, reported as occurring in Australia in 2008, was conducted as part of the DAN Asia-Pacific dive fatality reporting project. METHOD: The case studies were compiled using reports from witnesses, the police and coroners. In each case, the particular circumstances of the accident and details from the post-mortem examination, where available, are provided. RESULTS: In total, there were 19 reported fatalities (the same as for 2007), 17 involving males. Twelve deaths occurred while snorkelling and/or breath-hold diving, and six while s…

Predicting performance in competitive apnea diving, part II: dynamic apnoea.

Part I of this series of articles identified the main physiological factors defining the limits of static apnea, while this paper reviews the factors involved when physical work is added in the dynamic distance disciplines, performed in shallow water in a swimming pool. Little scientific work has been done concerning the prerequisites and limitations of swimming with or without fins whilst breath holding to extreme limits. Apneic duration influences all competitive apnea disciplines, and can be prolonged by any means that increase gas storage or tolerance to asphyxia, or reduce metabolic rate,

["Deep purple": a case of hemoptysis during breath-hold diving].

INTRODUCTION: Breath-hold diving has been practised since the dawn of humanity but is open to many risks. The consequences mainly involve the respiratory system. The pathophysiological mechanisms are complex because of the multiple forces applied to the body during breath-hold diving. OBSERVATION: The case reported here describes the development of haemoptysis during a single breath-hold dive to a depth of 26 m, associated with acute alveolar haemorrhage. Recovery was spontaneous. CONCLUSION: This event is rarely described and, although often associated with the more frequently reported pulm…

Reduced taxonomic richness of lice (Insecta: Phthiraptera) in diving birds.

Avian lice occupy different habitats in the host plumage that the physical environment outside the host body may affect in several ways. Interactions between host plumage and water may be an important source of such effects. Here, we use a comparative approach to examine the effect of a host's diving behavior on the taxonomic richness of its lice. Louse genera richness was significantly lower in clades of diving birds than on their nondiving sister clades. Species richness of host and body mass did not differ significantly between these clades; thus, these factors did not bias our results. Thi…

[Breath-hold diving--an increasing adventure sport with medical risks].

Breath-hold diving as a recreational and competitive sports activity is on the increase. In this review physiological limitations and medical risks associated with breath-hold diving are discussed. Specific topics include hypoxia, ascent blackout, hyperventilation, squeeze or barotrauma of descent including effects on the pulmonary system, glossopharyngeal breathing, and decompression illness. It is also concluded that the health requirements for competitive breath-hold diving should follow essentially the same standards as used for SCUBA-diving.

Frontal recess surgery for diving-related frontal pain: case report.

We report the case of a professional scuba diver who was unable to dive because he began experiencing severe frontal pain on descent. Following endoscopic surgery to open the frontal recess, the man was able to resume diving unrestricted by pain. We discuss the causes and treatment of this complaint, and we suggest that this might be considered a new indication for surgery in a limited number of cases.
