Minimally invasive implantation of epidural spinal cord neurostimulator electrodes by using a tubular retractor system. Technical note.

The authors describe a novel technique for the implantation of multipolar epidural spinal cord neurostimulator electrodes with the aid of a tubular retractor system. Spinal cord neurostimulation is used as a neuroaugmentive tool for treating chronic intractable pain syndromes. Minimally invasive placement of the multipolar neurostimulator electrodes may allow for shorter hospital stays and less postoperative pain associated with the incision.

Assessment of rat sciatic nerve function following acute implantation of high density Utah slanted electrode array (25 electrodes/mm(2) ) based on neural recordings and evoked muscle activity.

INTRODUCTION: High density Utah slanted electrode arrays (HD-USEAs) have been developed recently for intrafascicular access to submillimeter neural structures. Insertion of such high electrode density devices may cause nerve crush injury, counteracting the intended improved selective nerve fiber access. METHODS: HD-USEAs were implanted into sciatic nerves of anesthetized rats. Nerve function was assessed before and after HD-USEA implantation by measuring changes in evoked muscle and nerve compound action potentials and single unit neuronal recordings. RESULTS: Neural activity was recorded wi

Long-Term Neural Recording Performance of PEDOT/CNT/Dexamethasone Coated Electrode Array Implanted in Visual Cortex of Rats.

Implantable neural electrode arrays can be inserted in the brain to provide single-cell electrophysiology recording for neuroscience research and brain-machine interface applications. However, maintaining signal quality over time is complicated by inflammatory tissue responses and degradation of electrode materials. Organic electrode coatings offer a solution by enhancing recording and stimulation capabilities, including reduced impedance, increased charge injection capacity, and the ability to incorporate and release anti-inflammatory drugs. In this study, acid-functionalized multi-walled car

Platinum corrosion products from electrode contacts of human cochlear implants induce cell death in cell culture models.

Despite the technological progress made with cochlear implants (CI), impedances and their diagnosis remain a focus of interest. Increases in impedance have been related to technical defects of the electrode as well as inflammatory and/or fibrosis along the electrode. Recent studies have demonstrated highly increased impedances as the result of corroded platinum (Pt) electrode contacts. This in vitro study examined the effects of Pt ions and compounds generated by corrosion of the electrode contacts of a human CI on cell metabolism. Since traces of solid Pt in surrounding cochlear tissues have

Successful Cochlear Implantation of a Split Electrode Array in a Patient With Far-advanced Otosclerosis Assisted by Electromagnetic Navigation: A Case Report.

BACKGROUND: Incomplete electrode insertion is frequent when implanting ossified cochleae with conventional linear electrodes. If split electrode arrays (SEA) are used, this complication occurs less frequently resulting in improved audiological performance (AP). Additional implementation of electromagnetic navigation systems (EMNS) may add additional safety to this procedure and may further improve AP. However, previously performed SEA cochlea implantations rarely implemented EMNS. The few cases reported a laborious approach with customized, three-dimensional-printed microstereotactic headframe

Selective Stimulation of Facial Muscles Following Chronic Intraneural Electrode Array Implantation and Facial Nerve Injury in the Feline Model.

BACKGROUND: Our group has previously shown that activation of specific facial nerve (FN) fiber populations and selective activation of facial musculature can be achieved through acute intraneural multichannel microelectrode array (MEA) implantation in the feline model. HYPOTHESIS: Selective stimulation of facial muscles will be maintained in the setting of 1) chronic and 2) acute MEA implantation after FN injury and subsequent recovery. METHODS: This study included seven cats. In three cats with normal facial function, 4-channel penetrating MEAs were implanted chronically in the FN and teste

Electrical stimulation by semi-implantable electrodes decreases the levels of proteins associated with sciatic nerve injury-induced muscle atrophy.

Muscle atrophy is a disease that is usually caused by denervation. The aim of the present study was to determine whether electrical stimulation by semi-implantable electrodes is capable of decreasing the levels of specific proteins associated with sciatic nerve injury-induced muscle atrophy. Male Sprague Dawley (SD) rats with damaged sciatic nerves were maintained on a 12‑h light/dark cycle. Thirty-two SD rats were randomly allocated into 4 groups (each group, n=8). The rats in group C received no electrical stimulation; the rats in groups D, N and DN received electrical stimulation by semi-im

Percutaneous cephalocaudal implantation of epidural stimulation electrodes over sacral nerve roots--a technical note on the importance of the lateral approach.

OBJECTIVE: Idiopathic overactive bladder, urgency-frequency syndromes, interstitial cystitis, pudendal neuralgia, vulvodynia, prostadynia, and coccygodynia have been effectively treated with sacral nerve root modulation. This is most commonly performed with placement of electrodes via a transforaminal approach, predominately to the S3 foramen. This approach is limited by a high lead migration rate and the limitations of stimulating a single nerve root. Beginning in the 1990s, some centers began pursuing retrograde percutaneous placement from the lumbar spine, but adoption of this technique was

[The management of urinary and fecal incontinence due to a neurinoma of the cauda equina with an electrode implanted in the pelvic muscles (Caldwell) (author's transl)].

A woman, aged 49, had a complete sensory and motor deficit of the S2-5 segments with urinary and fecal incontinence due to a neurinoma of the cauda. A Caldwell electrode was implanted surgically into the muscles of the pelvic floor in April 1974. The patient was observed for 2 years thereafter and had an excellent result. The medical and technical problems of treating urinary and fecal incontinence due to a lower motor neuron lesion of S2-5 can be handled satisfactorily with an implanted electrode.

Comparative Long-Term Outcomes of Lateral Wall and Slim Modiolar Cochlear Implant Electrodes in Children With Normal Cochlear Anatomy.

BACKGROUND: Electrode design may influence neural stimulation efficiency, speech outcomes, and long-term stability. While both lateral wall and slim modiolar electrodes are widely used in pediatric cochlear implantation, comparative long-term outcome data in children with normal cochlear anatomy remain limited. METHODS: This retrospective comparative study included children with bilateral prelingual severe-to-profound sensorineural hearing loss who underwent cochlear implantation with either a lateral wall electrode (CI522/622) or a slim modiolar electrode (CI532/632) at a tertiary academic c

Dynamics in the Transimpedance Matrix and Electrically Evoked Compound Action Potentials in Cochlear Implant Users With a Lateral-Wall Electrode Array.

OBJECTIVES: In patients with cochlear implants, tools for measuring intracochlear electric environment as well as neural responses to electrical stimulation are widely available. This study aimed to investigate the possible correlation of changes in the responsiveness of the auditory nerve measured by neural response telemetry with changes in the peak and spread of the intracochlear electric field measured by transimpedance matrix (TIM) in patients implanted with straight electrode arrays. DESIGN: In this retrospective study, we analyzed a cohort of 144 ears of 113 consecutive patients who we

Access and Polarization Electrode Impedance Changes in Electric-Acoustic Stimulation Cochlear Implant Users with Delayed Loss of Acoustic Hearing.

Acoustic hearing can be preserved after cochlear implant (CI) surgery, allowing for combined electric-acoustic stimulation (EAS) and superior speech understanding compared to electric-only hearing. Among patients who initially retain useful acoustic hearing, 30-40 % experience a delayed hearing loss that occurs 3 or more months after CI activation. Increases in electrode impedances have been associated with delayed loss of residual acoustic hearing, suggesting a possible role of intracochlear inflammation/fibrosis as reported by Scheperle et al. (Hear Res 350:45-57, 2017) and Shaul et al. (Oto

Identifying Cochlear Implant Channels With Relatively Poor Electrode-Neuron Interfaces Using the Electrically Evoked Compound Action Potential.

OBJECTIVES: The primary objective of this study was to quantify local (within ear) and global (between ear) variation in the cochlear implant (CI) electrode-neuron interface (ENI) using the electrically evoked compound action potential (ECAP). We tested the hypothesis that, within an ear, ECAP measures can be used to identify channels with presumed good and poor ENIs, which may be influenced by a combination of spiral ganglion neuron (SGN) density, electrode position, and cochlear resistivity. We also hypothesized that ECAP responses would reflect age-related differences in the global quality
