OBJECTIVE: The long-term efficacy of posterior foraminotomy compared with anterior cervical decompression and fusion (ACDF) for the treatment of degenerative disc disease with radiculopathy has not been previously investigated in a population-based cohort.
METHODS: All patients in the national Swedish Spine Register (Swespine) from January 1, 2006, until November 15, 2017, with cervical degenerative disc disease and radiculopathy were assessed. Using propensity score matching, patients treated with posterior foraminotomy were compared with those undergoing ACDF. The primary outcome measure wa

STUDY DESIGN: Retrospective Cohort Study.
OBJECTIVE: Cervical radiculopathy meeting operative criteria has traditionally been managed using anterior cervical discectomy and fusion (ACDF). However, cervical disc arthroplasty (CDA) and posterior cervical foraminotomy (PCF) are also reasonable options. This study aimed to assess differences in postoperative outcomes among patients undergoing multi-level ACDF, CDA, or PCF comparing medical/surgical complications and healthcare utilization parameters.
METHODS: Patients who underwent multi-level ACDF, CDA, or PCF between 2012 and 2019 were identif

BACKGROUND: The number of surgical trials is increasing but such trials can be complex to deliver and pose specific challenges. A multi-centre, Phase III, RCT comparing Posterior Cervical Foraminotomy versus Anterior Cervical Discectomy and Fusion in the Treatment of Cervical Brachialgia (FORVAD Trial) was unable to recruit to target. A rapid qualitative study was conducted during trial closedown to understand the experiences of healthcare professionals who participated in the FORVAD Trial, with the aim of informing future research in this area.
METHODS: Semi-structured interviews were conduc

Anterior cervical decompression and fusion (ACDF) is the standard procedure for operation of cervical disc herniations with radicular arm pain. Mobility-preserving posterior foraminotomy is the most common alternative in the case of a lateral localization of the pathology. Despite good clinical results, problems may arise due to traumatization of the access. Endoscopic techniques are considered standard in many areas, since they may offer advantages in surgical technique and rehabilitation. These days, all disc herniations of the lumbar spine can be operated in a full-endoscopic technique. The

BACKGROUND: Synovial cysts are rare degenerative lesions arising from facet joints that likely arise due to hypermobility, trauma, and inflammation. These lesions can calcify, making resection challenging when adjacent to critical neurovascular structures. Operative intervention classically warrants a two-level partial or complete laminectomy, or a unilateral "open" or minimally invasive medial facetectomy/foraminotomy; notably, the latter is largely equivalent to the procedure described here as a "midline sparing para-articular approach."
CASE DESCRIPTION: This two-dimensional operative vide

OBJECTIVE: To compare the clinical outcomes and biomechanical effects of total disc replacement (TDR) and posterior cervical foraminotomy (PCF) and to propose relative inclusion criteria.
METHODS: Thirty-five patients who underwent surgery between 2006 and 2008 were included. All patients had single-level disease and only radiculopathy. The overall sagittal balance and angle and height of a functional segmental unit (FSU; upper and lower vertebral body of the operative lesion) were assessed by preoperative and follow-up radiographs. BACKGROUND: Posterior cervical expansive open-door laminoplasty (EODL) may cause postoperative C5 palsy, and it can be avoided by EODL with bilateral C4/5 foraminotomy. However, prophylactic C4/5 foraminotomy can compromise cervical spine stability. To prevent postoperative C5 palsy and boost cervical stability, We propose a new operation method: EODL combined with bilateral C4/5 foraminotomy and short-segment lateral mass screw fixation. However, there are no studies on the biomechanical properties of this surgery.
PURPOSE: Evaluating the biomechanical characteristics of EODL combined with b

STUDY DESIGN: A retrospective 1-year cost-utility analysis.
OBJECTIVE: To determine the cost-effectiveness of anterior cervical discectomy and fusion with plating (ACDFP) in comparison with posterior cervical foraminotomy (PCF) for patients with single-level cervical radiculopathy.
SUMMARY OF BACKGROUND DATA: Cervical radiculopathy due to cervical spondylosis is commonly treated by either PCF or ACDFP for patients who are refractory to nonsurgical treatment. Although some have suggested superior outcomes with ACDFP as compared with PCF, the former is also associated with greater costs. The p

BACKGROUND: Posterior cervical foraminotomy against anterior osteophyte is an indirect decompression procedure but less invasive compared to anterior cervical discectomy and fusion. Residual compression to the nerve root may lead to poor surgical outcomes. Although clinical results of posterior cervical foraminotomy for osteophytes are not considered better than those of disk herniation, osteophyte size and the association of the decompression area with poor surgical outcomes remain unclear. This study aimed to identify the limitations of minimally invasive posterior cervical foraminotomy for

STUDY DESIGN: We quantified the segmental biomechanics of a cervical total disc replacement (TDR) before and after progressive posterior decompression. We hypothesized that posterior decompressive procedures would not significantly increase range of motion (ROM) at the index TDR level.
OBJECTIVE: To quantify the kinematics of a cervical total disc replacement (TDR) before and after posterior cervical decompression.
SUMMARY OF BACKGROUND DATA: A reported yet unaddressed issue is the potential for the development of same-segment disease after implantation of a cervical TDR and the implications

BACKGROUND: Cervical radiculopathy is a spine ailment frequently requiring surgical decompression via anterior cervical discectomy and fusion (ACDF) or posterior foraminotomy/discectomy. While endoscopic posterior foraminotomy/discectomy is gaining popularity, its financial impact remains understudied despite equivalent randomized long-term outcomes to ACDF. In a cohort of patients undergoing ACDF vs endoscopic posterior cervical foraminotomy/discectomy, we sought to compare the total cost of the surgical episode while confirming an equivalent safety profile and perioperative outcomes.

RATIONALE: Although C5 palsy is a common complication of cervical spine surgery, its cause has not been confirmed. There are various hypotheses for its mechanism, including spinal cord impairment and nerve involvement. Therefore, prophylactic foraminotomy is one of the methods recommended for preventing C5 palsy. However, we describe a patient who experienced C5 palsy after microendoscopic foraminotomy between the left C5/6 and C6/7 levels.
PATIENT CONCERNS: A 43-year-old man presented with a 14-month history of progressive numbness in the left upper limb. We performed microendoscopic left fo
