BACKGROUND: The number of Rwandan women of reproductive age receiving mechanical heart valve prostheses for rheumatic heart disease is increasing. Effective anticoagulation management during pregnancy is essential to prevent valve thrombosis but poses significant risks to both mother and foetus if not carefully tailored.
OBJECTIVE: This article describes the multidisciplinary development of national guidelines for anticoagulation management in pregnant women with mechanical heart valves in Rwanda, integrating local clinical realities with international best practices.
METHODS: Guideline deve…

BACKGROUND AND AIMS: Bioprosthetic heart valves (BHVs) are inherently susceptible to structural degeneration, driven by a combination of mechanical stress, lipid infiltration, glutaraldehyde-induced crosslinking instability, and progressive calcification. Recent evidence has implicated the αGal antigen (galactose-α-1,3-galactose) as an additional contributor to BHV deterioration through activation of innate immune pathways. The present study aims to: 1) perform a quantitative assessment of the residual presence of xenoantigens, specifically αGal, in a range of commercial BHV models; 2) evaluat…

Editorial note: On 28 May 2025, the manufacturer announced the global discontinuation of sales of its ACURATE neo2 and ACURATE Prime aortic valve systems. The following correspondence, submitted and accepted prior to the market withdrawal, discusses clinical experience with this device. Though no longer relevant for current practice, the Editorial Board believes it is important to document these findings in the interest of transparency and completeness of the scientific record. Accordingly, we are publishing this work as a Research Correspondence, with acknowledgement of the device's discontin

Partial heart transplantation is a novel approach to deliver a growing donor valve in pediatric recipients needing valve replacements. Objective data on the rate of growth of semilunar valves in patients following orthotopic heart transplantation (OHT) are necessary to set expectations for partial heart transplant semilunar valve growth. A retrospective cohort study was performed, which included twelve infants who underwent OHT and twelve controls with ventricular septal defects (VSD). Semilunar valve annulus absolute dimension over serial echocardiograms was recorded, Z-scores were calculated

Accurate information on pregnancy outcomes in women with prosthetic heart valves (PHVs) is essential for preconception counseling and prenatal care. This study aims to determine the maternal and fetal outcomes in women with PHVs. A total of 138 pregnant women with PHVs admitted into a tertiary center between November 2007 and February 2020 were included in the study, and the data were analyzed retrospectively. Patients were divided into the mechanical heart valves (MHVs) group, 118 patients with MHVs, and the tissue heart valves (THVs) group, 20 patients with THVs. The 2 groups were compared. …

Invasive aspergillosis (IA) is a life-threatening infection that mainly affects immunocompromised hosts. Cardiac involvement is rare but can be the sole presentation of IA. It is associated with a high mortality rate and mostly occurs in patients with pre-existing cardiac disease. It can also be seen in immunocompetent patients with a structurally normal heart. The reported cases of cardiac involvement are usually due to infectious endocarditis (IE) caused by Aspergillus species (most commonly Aspergillus fumigatus). However, there is limited data on non-valvular cardiac aspergillosis (NVCA).

INTRODUCTION: Surgical pulmonary valve replacement is commonly required to palliate patients with CHD affecting the right ventricular outflow tract; however, concerns remain about mid- and long-term durability. Post-operative short-term anticoagulation has been hypothesised to improve valve durability.
METHODS: This is a single-centre, retrospective study of paediatric patients who underwent surgical pulmonary valve replacement and received a direct oral anticoagulant in addition to aspirin post heart valve insertion. The primary objective was a composite safety score consisting of clinically

INTRODUCTION: Transcatheter aortic valve implantation (TAVI) is becoming the standard of care for severe symptomatic aortic stenosis (AS). Yet, some patients with AS are not indicated/eligible for TAVI. Several noninvasive, catheter-based or surgical alternatives exist, and other therapeutic options are emerging.
AREAS COVERED: This review provides an overview of non-TAVI options for severe AS. Non-invasive, transcatheter, and alternative surgical strategies are discussed, emphasizing their backgrounds, techniques, and outcomes.
EXPERT OPINION: Alternative therapies to TAVI, whether device-b

BACKGROUND: Although pregnancy outcomes in women with normally functioning bioprosthetic valves (BPVs) are often good, structural valve dysfunction (SVD) may adversely affect pregnancy outcomes, but this has not been studied.
OBJECTIVES: The aim of this study was to examine outcomes in pregnant women with BPVs and the association with SVD.
METHODS: Pregnancy outcomes in women with BPVs were prospectively collected. Adverse maternal cardiac events (CEs) included cardiac death or arrest, sustained arrhythmia, heart failure, thromboembolism, and stroke. Adverse fetal events were also studied. D

Cardiovascular diseases are the first cause of death worldwide. Among different heart malfunctions, heart valve failure due to calcification is still a challenging problem. While drug-dependent treatment for the early stage calcification could slow down its progression, heart valve replacement is inevitable in the late stages. Currently, heart valve replacements involve mainly two types of substitutes: mechanical and biological heart valves. Despite their significant advantages in restoring the cardiac function, both types of valves suffered from serious drawbacks in the long term. On the one …

Background In patients with mechanical heart valves, cerebral susceptibility-weighted imaging (SWI) lesions on magnetic resonance imaging, postulated to be caused by degenerative metallic abrasion, are frequently referred to as valve abrasion. It remains unclear whether valve implantation not requiring cardiopulmonary bypass or biological heart valves also shows those lesions. Methods and Results Two blinded readers rated SWI lesions and cerebral amyloid angiopathy probability according to established criteria on brain magnetic resonance imaging pre- and postinterventionally. We assessed the a…

This paper considers an anisotropic hyperelastic soft tissue model, originally proposed for native valve tissue and referred to herein as the Lee-Sacks model, in an isogeometric thin shell analysis framework that can be readily combined with immersogeometric fluid-structure interaction (FSI) analysis for high-fidelity simulations of bioprosthetic heart valves (BHVs) interacting with blood flow. We find that the Lee-Sacks model is well-suited to reproduce the anisotropic stress-strain behavior of the cross-linked bovine pericardial tissues that are commonly used in BHVs. An automated procedure …

Since there is no upper age limit for general organ donation, unlike heart valve donation, and since a quarter of all organ donors are 65 years and older, we examined whether the heart valves from these donors are suitable as allografts. In the period 1999-2004 the aortic valve and pulmonary valve of 100 organ donors above 65 years of age were examined to establish whether they would have been suitable as valve grafts. To compare the valve grafts above and below the age limit of 65 years, we used data on the aortic and pulmonary valves of 380 organ donors below the age limit in the same time p…

Fractures in Björk-Shiley convexoconcave (BScc) heart valves have raised questions about the feasibility of early diagnosis of technical defects by means of acoustic assessment. Three laboratory tests were conducted. To establish acoustic fingerprints, 66 valves with a defect, such as single-leg fracture (SLF) or single-leg separation (SLS), or without a defect were connected with a contact sensor and excited by dropping a small metal ball onto the outlet strut. In the second test, we simulated the valve sound propagation within the thorax. In the third test, intact, SLF, and SLS valves were p…

Hemodynamic performance of the CarboMedics heart valve in the aortic position and its clinical impacts were investigated in 126 consecutive patients. The actuarial survival rates of patients who had undergone isolated aortic valve replacement and concomitant aortic and mitral valve replacement were 82.6+/-5.7% and 71.0+/-9.2% at 8 years, respectively. Morbid events were rare, and almost all late survivors were free from evident cardiac symptoms regardless of the valve size. Echocardiography revealed suboptimal transvalvular pressure gradients and effective orifice areas of 19 mm and 21 mm valv

BACKGROUND: The management of women with prosthetic heart valves during pregnancy poses a particular challenge as there are no available controlled clinical trials to provide guidelines for effective antithrombotic therapy. Oral anticoagulants such as warfarin sodium cause fetal embryopathy; subcutaneous administration of heparin sodium has been reported to be ineffective in preventing thromboembolic complications.
OBJECTIVE: To identify the risks of maternal and fetal complications in women with mechanical heart valves treated with different anticoagulation regimens during pregnancy.

The hydrodynamic function of six size 19 mm bileaflet mechanical heart valves was investigated in a pulsatile flow simulator. The St. Jude Standard, St. Jude Haemodynamic Plus, CarboMedics, Sorin Biomedica, Duromedics and ATS valves were studied. The mean pressure drop and effective orifice area during forward flow, regurgitant volumes and total energy loss were analysed. The St Jude Haemodynamic Plus and Sorin Biomedica valves had significantly lower forward flow pressure drops and total energy losses than the other four valves. Assuming that all six valves can be implanted in the equivalent …
