Herpes Simplex Type 2 Virus: a new horizon.

Genital herpes or Herpes Simplex Type 2 Virus (HSC-2) is an incurable, venerable disease. Genital herpes infects more than 1 of 5 adolescents and adults according to the Center for Disease Control. Many people do not know that they have it and are not aware that they may be spreading it to others. Treatment and management of herpes simplex virus has been through pharmacotherapy and patient education. Recent studies have emerged identifying a vaccine for herpes simplex virus. The vaccine has thus far proven to be effective for women. Pending further investigation the target population is young …

Pyoderma gangrenosum complicated by herpes simplex virus infection.

High-dose immunosuppression used in the treatment of pyoderma gangrenosum predisposes patients to opportunistic infections. A 66-year-old man presented with recalcitrant pyoderma gangrenosum in which the ulcer itself became infected with herpes simplex virus type 1. This patient was immunosuppressed with multiple agents including topical and oral corticosteroids, cyclosporin, mycophenolate mofetil, intravenous immunoglobulin and infliximab. However, the patient's ulcer continued to extend despite this. It was not until the presence of this virus was detected using polymerase chain reaction on …

[Ulcerating Herpes simplex infections in intensive care patients].

Herpes simplex infections are potentially a life-threatening situation for immunocompromised as well as critically ill patients. The correct diagnosis is made more difficult in comatose patients by the fact that the characteristic symptom of extreme pain cannot be registered. The clinical dermatological findings (polycyclic configuration, easily bleeding ulcers) are thus especially important in patients under intensive care conditions. As examples, the cases of 3 critically ill patients (subarachnoid bleeding or head injury) developing therapy-resistant, flat sacral or perioral skin ulcers wit…

10: Herpes simplex and varicella-zoster virus infections.

Any new patient with suspected genital herpes should have diagnostic testing with virus identification. Type-specific serological tests that distinguish between antibodies for type 1 and type 2 herpes simplex virus (HSV) may be useful to determine previous exposure but cannot be used to diagnose recurrences of genital herpes. Initial episodes of genital herpes usually require antiviral therapy, while recurrences may be treated with continuous antiviral suppression (if frequent) or episodic therapy; patient counselling and education (including how to recognise lesions) are essential. Topical or…

Recrudescent herpes simplex infection mimicking primary herpetic gingivostomatitis.

We report six cases of recrudescent intraoral herpes simplex infection clinically indistinguishable from primary herpetic gingivostomatitis. All infections occurred in healthy children or young adults. Serological analysis demonstrated herpes simplex virus (HSV)-specific IgG at initial presentation, indicating that the infection was not a primary infection. Convalescent sera exhibited HSV-specific IgM and a rising HSV-specific IgG titre. These findings demonstrate that the initial clinical diagnosis of primary herpetic gingivostomatitis was erroneous and that what was actually being observed w…

Prolonged croup due to herpes simplex virus infection.

UNLABELLED: Herpes simplex virus (HSV) is an uncommon cause of acute laryngitis in immunocompetent patients since it mostly occurs in immunocompromised subjects. We present two previously healthy children with prolonged gingivostomatitis and stridor (lasting 3 and 4 weeks) in whom HSV-1 was isolated from subglottal ulcers. CONCLUSION: HSV should be considered a possible pathogen in cases of prolonged or atypical croup not only in immunocompromised or elderly patients but also in otherwise healthy children.

Herpes simplex blepharoconjunctivitis presenting as complete acquired ankyloblepharon.

PURPOSE: A 90-year-old woman was referred to us with a 1-month history of progressively worsening blepharoconjunctivitis. She had a complete acquired ankyloblepharon of the right lids, which resulted in the appearance of a right upper lid abscess on computed tomography. METHODS: Blepharotomy with separation and debridement of the lid margins was performed. RESULTS: Cultures were positive only for herpes simplex virus, type II. CONCLUSIONS: Herpes simplex blepharoconjunctivitis may take many clinical forms. Our patient's severe manifestations led to the clinical and radiologic appearance of …

Congenital infection with herpes simplex virus type 1.

We treated a newborn who had congenital herpes simplex virus (HSV) type 1 infection in which extensive skin lesions occurred with no evidence of involvement of other organs. The correct diagnosis was not made until the fifth day of life, but after effective treatment with intravenous acyclovir this child has remained in perfect health. At his last evaluation at age 18 months, he had no neurologic or ophthalmologic abnormalities. This report should alert pediatricians and dermatologists to the occasional occurrence of herpes simplex virus infection in apparently healthy neonates and the importa…

Herpes simplex type 2 meningitis treated with acyclovir.

A case of meningitis due to a primary genital herpes simplex type 2 infection, and its prompt response to therapy with intravenous acyclovir is reported. This complication of genital herpes is probably underdiagnosed and it is worthwhile searching for evidence of genital herpetic lesions in a young patient presenting with viral meningitis.

Congenital herpes simplex virus, type 2, bilateral endophthalmitis.

A set of dizygotic twins, both born prematurely, developed herpes simplex encephalitis shortly after birth. The second twin had unilateral keratoconjunctivitis and bilateral endophthalmitis and subsequently died from disseminated herpes infection. Herpes simplex, type 2, was isolated from conjunctiva of both eyes, cerebrospinal fluid, nasopharynx and trachea. Histologic examination of the eyes revealed bilateral endophthalmitis with a necrotizing retinitis. Intranuclear inclusion bodies were demonstrated by light microscopy in the retina, choroid and iris, and virus particles were demonstrated…

[Herpes simplex myelitis in an immunocompetent adult patient].

BACKGROUND: Herpes simplex-1 virus myelitis is a rare cause of myelitis in immunocompetent adults. CASE PRESENTATION: A healthy man in his sixties presented with acute chest pain, dyspnoea and cardiac arrest. Pre-hospital cardiopulmonary resuscitation was performed, and he underwent thrombolysis. Intrahospital he was diagnosed with bilateral pulmonary embolism. Twelve days after hospitalisation he developed acute paralysis of the lower extremities. MR findings were suggestive of myelitis, and herpes simplex-1 virus was identified in the spinal fluid. He was diagnosed with herpes simplex myeli…

Herpes simplex virus presenting as an oropharyngeal mass.

OBJECTIVES: Present a clinically challenging case of an immunocompetent 74-year-old male who presented with marked dyspnea and hemoptysis. After the airway was secured, direct laryngoscopy revealed a large, fungating, hemorrhagic mass of the left lateral pharyngeal wall and surrounding structures. METHODS: Chart review of a single patient. This patient provided consent for his case materials and images to be used for educational purposes and publication. RESULTS: The clinical appearance of the mass was suspicious for an aggressive neoplasm. Initial biopsy of the mass was nonspecific, reveali

An Atypical Case of Herpes Simplex 1 Meningitis.

Herpes simplex virus (HSV) is well known to cause Central Nervous System (CNS) infections, ranging from more common HSV-2 viral meningitis to the more rare and severe HSV-1 encephalitis. Here we present an atypical case of aseptic meningitis due to HSV-1. Intriguingly, despite the potential severity of HSV-1 infections, the patient had an uncomplicated course with only mild symptoms that resolved with conservative management alone. This case report aims to provide a frame of reference for the non-specific presentation of a patient with uncommon HSV-1 meningitis.
