A multicenter randomized trial of high frequency oscillatory ventilation as compared with conventional mechanical ventilation in preterm infants with respiratory failure.

A multicenter randomised trial was conducted in nine neonatal centers in Japan to re-evaluate the safety and the efficacy of high frequency oscillatory ventilation using the piston type oscillator (Hummingbird) in the treatment of respiratory failure in preterm infants weighing between 750 and 2000 g at birth. A total of 92 infants were enrolled in the study. Forty-six infants were allocated to high frequency oscillatory ventilation and 46 infants to conventional mechanical ventilation. There were no differences in sex, birth weight, gestation and Apgar score between groups. The study was begu

Using very high frequencies with very low lung volumes during high-frequency oscillatory ventilation to protect the immature lung. A pilot study.

OBJECTIVE: High-frequency oscillatory ventilation (HFOV) has been described as a rescue therapy in severe respiratory distress syndrome (RDS) with a potential protective effect in immature lungs. In recent times, HFOV combined with the use of volume guarantee (VG) strategy has demonstrated an independent effect of the frequency on tidal volume to increase carbon-dioxide (CO2) elimination. The aim of this study was to demonstrate the feasibility of using the lowest tidal volume on HFOV+VG to prevent lung damage, maintaining a constant CO2 elimination by increasing the frequency. STUDY DESIGN:

[A comparative evaluation of the efficacy of the traditional combination of high-frequency and intermittent high-frequency artificial ventilation of the lungs in patients with parenchymatous acute respiratory insufficiency].

Conventional controlled lung ventilation (CLV) with positive end expiratory pressure (PEEP) (Con CLV), combined high-frequency CLV (Com HF CLV) and intermittent high-frequency CLV with PEEP (Int HF CLV) have been performed in 43 patients with parenchymatous acute respiratory failure (ARF). It has been established that Int HF CLV significantly increases PaO2 in patients with focal damages of the lung parenchyma and effective compliance (Ceff) > 0.033 l/cm H2O. Com HF CLV increases significantly PaO2 and lung compliance in patients with disseminated lung damages and Ceff < 0.030 l/cm H2O and has

Lung pathology after high frequency jet ventilation combined with low rate intermittent mandatory ventilation in a canine model of meconium aspiration.

The microscopic appearance of lungs subjected to six hours of conventional ventilation (CV), standard high frequency jet ventilation (HFJV), and HFJV combined with low rate intermittent mandatory ventilation (CHFJV) was studied in 22 mongrel puppies. A lung injury model was created by insufflating 4 ml/kg of a 20% mixture of meconium into the trachea. The animals were initially ventilated at the same mean airway pressure (Paw). Subsequently, if PCO2 fell below 20 torr, peak and mean airway pressures were reduced. A scoring system was devised which assigned 0-3 points for each of eight lung inj

Changes in lung volume and ventilation during lung recruitment in high-frequency ventilated preterm infants with respiratory distress syndrome.

OBJECTIVES: To assess global and regional changes in lung volume and ventilation during lung recruitment in preterm infants with respiratory distress syndrome. STUDY DESIGN: Using electrical impedance tomography, changes in lung volume and ventilation were measured in 15 high-frequency oscillatory ventilated preterm infants during oxygenation-guided recruitment maneuvers. The inflation and deflation limbs were mapped, and the lower and upper inflection points were calculated using both oxygenation and impedance data. The impedance data were also used to determine recruitment-related changes i

Early activation of inflammation and clotting in the preterm lamb with neonatal RDS: comparison of conventional ventilation and high frequency oscillatory ventilation.

In neonatal respiratory distress syndrome activation of inflammation and clotting is demonstrated. High frequency oscillatory ventilation (HFOV) is considered to be less damaging to the human preterm lung, resulting in less activation of inflammation and clotting compared with conventional ventilation (CV). To assess the sequence of events of activation of inflammation and clotting and to compare the impact of HFOV to CV, we ventilated preterm lambs delivered by cesarean section at 132 d gestational age (term 145 d) for 8 h by CV (n = 10) or HFOV (n = 11). Fifteen minutes after birth and at 2-

Using Dexcom G7 in Critically Ill Infant With Hyperinsulinemia on High Frequency Oscillatory Ventilation Support.

INTRODUCTION: Life threatening hypoglycemia can occur during critical illness in infants with underlying hyperinsulinemia or metabolic conditions, such as glycogen storage diseases. Using a continuous glucose monitor (CGM) can provide insight into glycemic trends with this high-risk patient population in the intensive care setting. Barriers to usage of CGMs in the intensive care unit include familiarity with CGMs devices among staff, safety, and cost. CASE PRESENTATION: We describe the first reported case of safely utilizing the newest Dexcom G7 continuous glucose monitor in an infant with hy

Role of nasal high-frequency oscillatory ventilation in a premature infant with severe bronchopulmonary dysplasia.

To prevent the worsening of bronchopulmonary disease (BPD), early extubation is desirable. However, in extremely preterm infants, BPD tends to become severe, making early extubation difficult and leading to prolonged intubation. Even if the intubation period is prolonged, feasible respiratory strategies for extubation in extremely preterm infants during the chronic phase of severe BPD are necessary. In preterm infants, nasal high-frequency oscillatory ventilation (NHFOV) can support breathing after extubation immediately after birth, but whether NHFOV is effective as respiratory support after

Receipt of high-frequency ventilation is associated with acute kidney injury in very preterm neonates.

BACKGROUND: High-frequency ventilation (HFV) is frequently used in critically ill preterm neonates. We aimed to determine the incidence of acute kidney injury (AKI) in neonates less than 29 weeks gestation who received HFV in the first week of life and to determine if the rates of AKI differed in those who received other forms of respiratory support. METHODS: This retrospective cohort study of 24 international, level III/IV neonatal intensive care units (NICUs) included neonates less than 29 weeks gestation from the AWAKEN study database. Exclusion criteria included the following: no intraven…

High-Frequency Percussive Ventilation in Cystic Fibrosis Patients With Acute Respiratory Failure: A Case Series.

Acute respiratory failure in cystic fibrosis carries a high risk of mortality. The optimal mode of mechanical ventilation (MV) in this population is not well established. In this case series, we identified patients with cystic fibrosis who were ventilated with high-frequency percussive ventilation (HFPV) at our institution and describe their characteristics and outcomes. The use of high-frequency percussive ventilation has been sparsely described in the literature. This case series could serve as hypothesis-generating for future research.

Hemodynamic intolerance and pericardial effusion associated with high-frequency jet ventilation during pulmonary vein isolation.

BACKGROUND: High-frequency jet ventilation (HFJV) is used during pulmonary vein isolation (PVI) to increase catheter stability and improve outcomes. In prior studies, hemodynamic intolerance to HFJV was rare. OBJECTIVES: To evaluate the incidence of hemodynamic or respiratory intolerance of HFJV during PVI. METHODS: Retrospective observational analysis of consecutive patients undergoing PVI performed by 2 operators (PT, JW) at our institution between February 2019 and June 2020 who developed persistent hypotension or abnormal ventilatory parameters in association with HFJV. RESULTS: Among 1

Non-invasive high-frequency oscillatory ventilation in preterm infants: a randomised controlled cross-over trial.

OBJECTIVE: Non-invasive high-frequency oscillatory ventilation (nHFOV) has recently been described as a novel mode of respiratory support for premature infants. This study was designed to determine whether nHFOV decreases CO2 partial pressure (pCO2) in premature infants more effectively than non-invasive continuous positive airway pressure (nCPAP). DESIGN: Non-blinded prospective randomised controlled cross-over study. SETTING: University Medical Center tertiary neonatal intensive care unit. PATIENTS: 26 premature infants of 27±2 weeks of gestational age after extubation or non-invasive sur

Assessing Initial Response to High-Frequency Jet Ventilation in Premature Infants With Hypercapnic Respiratory Failure.

BACKGROUND: High-frequency jet ventilation (HFJV) has been used in conjunction with conventional ventilation for infants with respiratory failure. We sought to identify parameters that were associated with successful application of HFJV in patients with hypercapnic respiratory failure. METHODS: A single-center, retrospective review of infants who received HFJV was conducted. Subjects were enrolled if birthweight was ≤2,000 g and capillary PCO2 was ≥55 mm Hg. Ventilator parameters and physiologic data were recorded at 1 h before HFJV initiation and at hours 1, 4, and 6 following conversion. Su

Effect of Volume Guarantee in Preterm Infants on High-Frequency Oscillatory Ventilation: A Pilot Study.

Objectives Although adding volume guarantee (VG) to conventional ventilation has been a well-established respiratory management for preterm infants, the evidence of VG combined with high-frequency oscillatory ventilation (HFOV) has not been studied well. The aim of this study was to investigate the effect of VG added to HFOV on respiratory and other physiological parameters. Methods We conducted a pilot study in extremely low-birth-weight infants ventilated with HFOV + VG with stable pulmonary status after 28 days of age. VG was applied for 6 hours and removed for the following 6 hours, and da

[Pulmonary interstitial emphysema in a 15-month-old child treated with high frequency oscillatory ventilation].

Pulmonary interstitial emphysema (PIE) is a known complication of mechanical ventilation of preterm infants. PIE is associated with high mortality and treatment is controversial as both surgery and conservative treatment are possibilities. We present a case with a 15-month-old previously healthy child, hospitalized with severe respiratory distress. A computed tomography (CT) of the chest revealed localized PIE. He was successfully treated.

Elective high-frequency oscillatory ventilation in preterm neonates: a preliminary investigation in a developing country.

BACKGROUND: Elective high-frequency oscillatory ventilation (HFOV) in preterm infants is known to be as effective as conventional ventilation in the prevention of bronchopulmonary dysplasia without the risk of increased mortality or brain damage. OBJECTIVE: To document the feasibility and safety of early use of HFOV in preterm neonates with respiratory distress syndrome (RDS) in a resource-limited setting. The primary outcome was survival or death, and the secondary outcome included complication rates observed among the two groups, HFOV and synchronised intermittent mandatory ventilation (SIM
